A controlled cortical impact model of traumatic brain injury in the rat.

Controlled cortical impact models produce brain injury by using a pneumatic impactor to impact exposed brain. This study systematically examined the effects of varying magnitudes of controlled cortical impact to the rat brain on neurological, cardiovascular, and histopathological variables. As the magnitude of injury increased, the duration of suppression of somatomotor reflexes and the duration of chronic vestibular motor deficits increased. The blood pressure response was observed to depend on injury levels; a moderate injury level produced a hypotensive response while a high injury level pr

[Epidemiology of benign paroxysmal positional vertigo].

Every ninth patient with a previous history of vertigo at the ENT Hospital of the University of Freiburg suffers from BPPV. The incidence, some important pathogenetic factors and the clinical picture of BPPV are discussed. In young men posttraumatic aetiology dominates, whereas idiopathic BPPV prevails in perimenopausal women. The affected ear tends to be hard of hearing in the high frequencies and is sometimes hypoexcitable by caloric stimulation. The diagnosis is best made by personal examination with Frenzel's glasses in darkness. Electronystagmography, brainstem evoked response audiometry,

[Combined computer-assisted neuro-otologic diagnosis of inner ear function in children].

Among 10 284 data files of neuro-otological patients of our neuro-otological data bank NODEC III, 8,6% pertain to children between 0 and 15 years of age. 350 (3,5%) of these are in the age group of 0-5 years, 261 (2,5%) in the group of 6-10 years and 267 (2,6%) in the group of 11-15 years. Modern inner ear functional diagnostic methods combine computerised neuro-otological audiometric as well as equilibriometric tests. Ranking of diagnostic applicability and efficiency comprises the following 6 steps of neuro-otological equilibriometric and audiometric tests: Child-adapted psychophysical heari

Hearing and vestibular abnormalities in primary fibrositis syndrome.

A prospective neurootological study of 30 cases of primary fibrositis syndrome (PFS) and 30 sex and age matched healthy volunteers was performed. Results showed that 70% of the PFS patients had decreased painful sound threshold, 40% hyperreactivity of perrotatory nystagmus without neurological or peripheral vestibular lesions, and 27% asymptomatic sensorineural hearing loss at low frequencies. Such abnormal data were not found in the matched controls. These findings suggest that otological problems may be part of the nonrheumatic features encountered in PFS.

If Not Now, When? An Absence of Neurocognitive and Postural Stability Deficits in Collegiate Athletes with One or More Concussions.

A history of concussion has been associated with decreased neurocognitive function and postural control. The purpose of our study was to compare neurocognitive function and postural control in collegiate athletes with and without varying histories of concussion. Collegiate athletes were divided into groups based on 0 (n = 129), 1 (n = 91), 2 (n = 52), and 3+ (n = 34) prior concussions. Participants in each group were carefully matched by sport, sex, height, weight, and age. Athletes were administered the Immediate Post-Concussion Assessment and Cognitive Test (ImPACT™) and the Sensory Organiza

Chronic idiopathic nausea.

Chronic nausea is a prevalent but poorly described symptom in adolescents. It often co-occurs with other functional gastrointestinal disorders (FGIDs) but may also present in isolation. A multitude of triggers and complex neural pathways underlie the sensation of nausea. These include gastrointestinal and blood-borne insults, dysmotility, vestibular or central nervous system pathways, an aberrant autonomic nervous system, and psychosocial factors. Although clinical algorithms are lacking, diagnosis is typically made on the basis of a thorough clinical history and without extensive testing. Tre

Etiological factors in space motion sickness.

We compared susceptibility to motion sickness during exposure to sudden-stop stimulation as a function of gravitoinertial force level. Our findings show that susceptibility is greatly enhanced, both with eyes-closed and eyes-open, for zero-g and 2-g conditions in parabolic flight compared with 1-g test conditions. The change in susceptibility is likely related to three factors: alterations in vestibulo-ocular function which result from variations in gravitoinertial force level (28,29); the altered pattern of otolithic activity resulting during variations in gravitoinertial force level; and the

A case-oriented approach exploring the relationship between visual and vestibular disturbances and problems of higher-level mobility in persons with traumatic brain injury.

Dizziness and impaired balance are common complaints after traumatic brain injury as a result of injuries to many systems. Ambient visual and vestibular system impairments are strong contributors to imbalance and dizziness, often leading to problems with spatial orientation and gaze stability during movement. The purpose of this review is to provide an overview of visual-vestibular impairments common to traumatic brain injury and summarize the contribution impairments in these systems will have on higher-level mobility. Standard visual and vestibular tests and measures are described and summar

Response Over Time of Vertigo Spells to Intratympanic Dexamethasone Treatment in Meniere's Disease Patients.

OBJECTIVE: To assess the effectiveness and response over time of intratympanic dexamethasone on the symptoms of Meniere's disease. MATERIALS AND METHODS: We performed a matched cohort study of 24 patients with Meniere's disease who were unresponsive to initial treatment and underwent 3 sessions of weekly intratympanic dexamethasone injections using a concentration of 16 mg/mL and 24 matched controls with the same characteristics with regard to vertigo spells. RESULTS: Compared with control subjects, intratympanic dexamethasone injections resulted in a decrease in the frequency of vertigo spe

[Vertigo and dizziness. Diagnostic algorithm from the perspective of emergency medicine].

Vertigo and dizziness are common symptoms in the acute care setting and have a wide diagnostic range. The most deleterious diagnosis is vertebrobasilar disease with brain infarction in the posterior fossa. Therefore, every patient with acute vestibular syndrome needs to be evaluated by a structured interview and a systematic physical examination for red flag symptoms which indicate vertebrobasilar infarction. Routinely, the physical examination should cover the head impulse (Halmagyi) test, test for nystagmus as well as the test of skew and in cases of benign paroxysmal positional vertigo, by

Recovery of vestibular imbalances after vestibular neuritis.

OBJECTIVES: To determine chronological characteristics of vestibular recovery after vestibular neuritis (VN) by using various clinical parameters. STUDY DESIGN: Prospective clinical study. METHODS: Twenty of 22 consecutive patients with acute VN underwent serial follow-ups of static (spontaneous nystagmus, subjective visual vertical, and ocular torsion) and dynamic (head-shaking nystagmus [HSN], vibration-induced nystagmus [VIN], head thrust test, and caloric test) vestibular imbalances for 1 year after symptom onset. RESULTS: Static vestibular imbalances improved more rapidly than dynamic

Superior semicircular canal dehiscence simulating otosclerosis.

This is a report of a patient with an air-bone gap, thought 10 years ago to be a conductive hearing loss due to otosclerosis and treated with a stapedectomy. It now transpires that the patient actually had a conductive hearing gain due to superior semicircular canal dehiscence. In retrospect for as long as he could remember the patient had experienced cochlear hypersensitivity to bone-conducted sounds so that he could hear his own heart beat and joints move, as well as a tuning fork placed at his ankle. He also had vestibular hypersensitivity to air-conducted sounds with sound-induced eye move

[Vestibular symptoms and ENG findings during periods of convalescence after endolymphatic sac drainage and steroid-instillation surgery (EDSS)].

Understanding the appearance of vestibular symptoms during periods of convalescence after surgery for the treatment of Meniere's disease is important for determining when a patient can return to work as well as the long-term results of the operation. We have treated 20 cases of intractable Meniere's disease with endolymphatic sac drainage & steroid-instillation surgery (EDSS) [Kitahara T, et al., Ann Otol Rhlnol Laryngol in press, 2000] and observed the subjective symptoms and objective vestibular findings using electronystagmogram (ENG) during the subsequent period of convalescence. The avera
