We conducted a retrospective study of 636 eyes (624 patients, aged 65 years or more) to identify the clinical features of nontraumatic retinal detachment in the elderly. The incidence of retinal detachment due to a macular hole in our series (21%) was much higher than those described in previous reports, suggesting a racial difference between Japanese and Caucasian patients. We studied 573 patients with the Marfan syndrome for the presence of ocular misalignment, refractive errors, and amblyopia. A total of 110 patients (19.2%) had strabismus. Of 573 patients, exotropia occurred in 67 (11.7%), esotropia in 12 (2.1%), vertical deviations in eight (1.4%), and primary inferior oblique muscle overaction in three (0.5%). Of 67 patients with exotropia, 18 had anisometropia and 39 had amblyopia. Of the 12 patients with esotropia, four had amblyopia and none had anisometropia. In 15 eyes with uncontrolled aphakic/pseudophakic glaucoma, trabeculectomy with anterior vitrectomy was performed. Mean pre-operative intraocular pressure (IOP) was 38.2 +/- 6.7 mmHg and mean post operative IOP was 19.3 +/- 5.2 mmHg after follow-up of 11.2 2 months. The anterior chamber maintainer technique during vitrectomy has been employed. Success rate (IOP less than 21 mmHg with or without glaucoma medication) was 80%. The main emphasis in this study is on the three most common potentially blinding diseases in the age group 43 years and older, i.e. cataract, age related macular degeneration and open angle glaucoma. The effects of 4% topical epinephrine bitartrate on regional ocular blood flow were studied in normal, "sham-operated," and aphakic eyes of rabbits. Anterior uveal blood flow decreased in all cases, but reductions in the ciliary processes were less pronounced in aphakic eyes than in sham-operated eyes. Doubling the dose (from 2 to 4 drops) abolished this difference in the ciliary processes, but the increased dose was accompanied by the emergence of a similar (although lesser) effect in the iris. Visual rehabilitation by conventional aphakic spectacles and contact lenses has posed a serious problem in the pediatric population. While the use of intraocular lenses has not achieved widespread acceptance as a form of aphakic correction, we have obtained encouraging results in some of our patients. In this report, we discuss two categories: those patients with congenital cataracts, and those patients with traumatic cataracts.

A Cary, Model 118, spectrophotometer was used to measure spectral absorbence by the human lens capsule of wavelengths 200 nm to 400 nm. The lens capsule transmits greater than 90% of the incident UV light, and thus does not appear to provide significant protection for the retina from UV radiation. This suggests that the lower incidence of aphakic cystoid macula edema in the eye with an intact posterior capsule is not related to UV light absorption by the lens capsule.

A comparison of spheric and aspheric cataract lenses was carried out on 28 patients with uncomplicated surgery. Lenses were evaluated with respect to eccentric acuity, visual field, ring scotoma, magnification, distortion, and subjective preferences. The results suggest that design differences can be applied to specific visual tasks, thus improving clinical lens prescribing.
