Electrophysiological predictors of super-response to left bundle branch area pacing in nonischemic cardiomyopathy: The PRECISION LBBAP study.

BACKGROUND: Left bundle branch area pacing (LBBAP) is an emerging alternative to biventricular pacing for cardiac resynchronization therapy (CRT), but variability in response remains poorly understood. OBJECTIVE: This study aimed to assess differences in cardiac activation between super-responders and partial responders to LBBAP and identify electrophysiological predictors of response using noninvasive electrocardiographic imaging. METHODS: Patients with nonischemic cardiomyopathy and left bundle branch (LBB) block (QRS >130 ms) who underwent LBBAP were enrolled. 10 super-responders (≥20% im…

An interactive assistant for patients with cardiac implantable electronic devices: A study protocol of the LUCY trial.

Patients with chronic heart failure (CHF) and reduced left ventricle ejection fraction benefit from cardiac resynchronization therapy (CRT) and implantable cardioverter defibrillator (ICD). However, increasing numbers of patient with CRT and ICD devices produce overload of cardiology centers where patients are admitted to ambulatory visits. This study aims to find multivariate model predicting the requirement for ambulatory follow-up of cardiac implantable electronic devices (CIEDs).The LUCY study is an observational, cohort, prospective, 2-stage trial. As equal number of patients (300) will b…

The prognostic significance of serum sodium in a population undergoing cardiac resynchronisation therapy.

PURPOSE: To determine the prognostic implications of changes towards hyponatremia at varying time-points in the treatment of patients undergoing cardiac resynchronisation therapy (CRT). METHODS: A retrospective series of 249 patients was studied from 2002 to 2013. The population was categorized on the basis of serum sodium profile at baseline, at 1 month and at 6 month follow up visits following successful CRT implantation. The composite endpoint was all-cause mortality and heart failure hospitalisation (defined by the need for intravenous diuretic therapy) following CRT implantation. RESULT

Improvement in Clinical Outcomes With Biventricular Versus Right Ventricular Pacing: The BLOCK HF Study.

BACKGROUND: Sustained right ventricular (RV) apical pacing may lead to deterioration in ventricular function and an increased risk of heart failure, especially in patients with pre-existing systolic dysfunction. The BLOCK HF (Biventricular Versus Right Ventricular Pacing in Heart Failure Patients With Atrioventricular Block) trial demonstrated that biventricular-paced patients had a reduced incidence of a composite endpoint of death, heart failure-related urgent care, and adverse left ventricular remodeling. OBJECTIVES: In a pre-specified analysis, this study examined clinical outcomes, inclu

Imaging topography and morphometry of persistent left superior caval vein and its variations, detected on cardiac implantable electronic device implantation.

BACKGROUND: Persistent left superior caval vein (PLSCV) is a rare, anatomically diverse developmental anomaly of systemic veins. Clinically asymptomatic PLSCVs are detected incidentally during medical procedures that utilise systemic veins, such as cardiac implantable electronic device (CIED) placement, and whose successful completion depends on favourable morphometric parameters of these vessels. The aim of this paper was to present topography and morphometry of PLSCV variations encountered during CIED implantation procedures. MATERIALS AND METHODS: We analysed a group of 5,010 patients for

Impact of left atrial appendage exclusion using an epicardial ligation system (LARIAT) on atrial fibrillation burden in patients with cardiac implantable electronic devices.

BACKGROUND: The left atrial appendage (LAA) is a well-known source of atrial arrhythmia and atrial fibrillation (AF). OBJECTIVE: The purpose of this study was to determine whether LAA exclusion using the LARIAT device would decrease AF burden. METHODS: A total of 50 patients with AF and cardiac implantable electronic devices who underwent successful LAA exclusion were enrolled in this prospective observational study. AF burden before LAA exclusion (baseline) and 3 and 12 months after exclusion was assessed by device interrogation. RESULTS: AF burden at 3-month follow-up (42% ± 34%) was sign

Use of a lateral infraclavicular puncture to obtain proximal venous access with occluded subclavian/axillary venous systems for cardiac rhythm devices.

BACKGROUND: Venous occlusion is not uncommon and total venous obstruction with more proximal patency may occur in as many as 10% of previous implants. Many techniques are available to obtain ipsilateral access; however, most require special equipment or skills. We describe a technique of infraclavicular cannulation of the brachiocephalic vein ipsilateral to the occlusion that is safe and feasible for most implanters. METHODS: Fourteen patients with subclavian/axillary occlusions ipsilateral to the implanted device and requiring revision or upgrade of their system or venous occlusion with cont

Three-dimensional speckle tracking echocardiography for the assessment of left ventricular function and mechanical dyssynchrony.

OBJECTIVE: Evaluation of left ventricular (LV) function and dyssynchrony is of vital importance. A newly-developed three-dimensional (3D) speckle tracking echocardiography (STE) has the potential to circumvent limitations of two-dimensional STE. Our study was to evaluate LV function and dyssynchrony by 3D-STE. METHODS AND RESULTS: Fifty-two non-ischaemic dilated cardiomyopathy patients (DCM) with complete left bundle-branch block and 55 healthy subjects were enrolled. Of these patients, 24 underwent cardiac resynchronization therapy (CRT). 3D-STE was performed before and 1 month after CRT. We…

Systemic Circulation in Advanced Heart Failure and Cardiogenic Shock: State-of-the-Art Review.

The integrative physiology of the left ventricle and systemic circulation is fundamental to our understanding of advanced heart failure and cardiogenic shock. In simplest terms, any increase in aortic stiffness increases the vascular afterload presented to the failing left ventricle. The net effect is increased myocardial oxygen demand and reduced coronary perfusion pressure, thereby further deteriorating contractile function. Although mechanical circulatory support devices should theoretically work in concert with guideline-directed medical therapy, cardiac resynchronization and inotropic and

Imaging in patients with cardiovascular implantable electronic devices: part 1-imaging before and during device implantation. A clinical consensus statement of the European Association of Cardiovascular Imaging (EACVI) and the European Heart Rhythm Association (EHRA) of the ESC.

More than 500 000 cardiovascular implantable electronic devices (CIEDs) are implanted in the European Society of Cardiology countries each year. The role of cardiovascular imaging in patients being considered for CIED is distinctly different from imaging in CIED recipients. In the former group, imaging can help identify specific or potentially reversible causes of heart block, the underlying tissue characteristics associated with malignant arrhythmias, and the mechanical consequences of conduction delays and can also aid challenging lead placements. On the other hand, cardiovascular imaging is

Daily Exposure to Air Pollution Particulate Matter Is Associated with Atrial Fibrillation in High-Risk Patients.

Several epidemiological studies found an association between acute exposure to fine particulate matter of less than 2.5 μm and 10 μm in aerodynamic diameter (PM2.5 and PM10) and cardiovascular diseases, ventricular fibrillation incidence and mortality. The effects of pollution on atrial fibrillation (AF) beyond the first several hours of exposure remain controversial. A total of 145 patients with implantable cardioverter-defibrillators (ICDs), cardiac resynchronization therapy defibrillators (ICD-CRT), or pacemakers were enrolled in this multicentric prospective study. Daily levels of PM2.5 an…
