Effectiveness of mindfulness-based cognitive therapy via e-health on anxiety and depression in adults: A meta-analysis.

BACKGROUND: Anxiety and depression are highly prevalent mental health disorders, imposing significant burdens on individuals and healthcare systems. Mindfulness-based cognitive therapy (MBCT) has demonstrated effectiveness in reducing these symptoms. Electronic health MBCT (eMBCT) offers a scalable alternative, but its efficacy in alleviating anxiety and depression in adults remains unclear. OBJECTIVES: To examine the treatment efficacy of eMBCT for adult anxiety and depression and identify potential moderators associated with better treatment outcomes. METHODS: A systematic search was condu

Recovery from chronic fatigue syndrome: a reflexive thematic analysis of experiences of people before, during and after treatment.

INTRODUCTION: Chronic fatigue syndrome (CFS) is a chronic illness marked by severe, medically unexplained fatigue. Cognitive behavioural therapy (CBT) moderately improves fatigue and functioning. However, there is debate about recovery and how this should be operationalised. The impact of treatment on how recovery is viewed is also unclear. This study explored how people with CFS receiving CBT viewed recovery and whether these views differed at various stages of treatment. METHODS: A total of 19 people with CFS receiving CBT were recruited from a specialist service in the UK. Purposive sampli

Symptom Dimensions of Pediatric Obsessive-Compulsive Disorder: Exploratory Examination of Associations with Clinical Features and Response to Cognitive-Behavioral Therapy.

Several overarching symptom dimensions have been identified within pediatric obsessive-compulsive disorder (OCD), including doubting, obsessing, washing, ordering, neutralizing, and hoarding. These unique symptom presentations may have important clinical implications, especially in terms of potentially distinct associations with age, functional impairment, insight, family accommodation, and symptoms of anxiety and depression. Such symptom dimensions may also differ in their responsiveness to cognitive-behavioral therapy (CBT). Existing literature on the features associated with and the clinica

Dual prostacyclin infusions: A case report of a patient symptom-driven transition from high-dose intravenous epoprostenol to subcutaneous treprostinil for the treatment of pulmonary arterial hypertension.

PURPOSE: A case of successful transition from high-dose epoprostenol to high-dose subcutaneous treprostinil for treatment of pulmonary arterial hypertension (PAH) is reported. SUMMARY: PAH is a chronically progressive disease characterized by pulmonary artery luminal narrowing that causes increased pulmonary artery pressures leading to right ventricular failure. Parenteral prostacyclin analogues, such as epoprostenol and treprostinil, are direct vasodilators and are cornerstones of therapy for patients with severe disease that have been proven to reduce mortality and increase exercise toleran

Computer-supported spinal fusion rod shaping yields improved conformity compared to freehand bending.

During spinal fusion surgery, metallic rods must be shaped to conform to pedicle screw positions. Poor conformity can overload screws, causing pull-out or loosening. Manual freehand rod bending is perception-dependent, time-consuming, cognitively demanding, and variable. We propose and evaluate a workflow in which rod shape is computed virtually via multi-objective optimization and then manually realized with a custom tool. The algorithm minimizes bend number and angle to improve efficiency and preserve rod strength. Seven spine surgeons each bent rods for a 7-level screw configuration using b

Cannabidiol for Treatment-Resistant Anxiety Disorders in Young People: An Open-Label Trial.

Background: Treatment resistance is a significant problem among young people experiencing moderate-to-severe anxiety, affecting nearly half of all patients. This study investigated the safety and efficacy of cannabidiol (CBD), a non-intoxicating component of Cannabis sativa, for anxiety disorders in young people who previously failed to respond to standard treatment. Methods: In this open-label trial, 31 young people aged 12-25 years with a DSM-5 anxiety disorder and no clinical improvement despite treatment with cognitive-behavioral therapy and/or antidepressant medication were enrolled betwe

Neuropsychological Performance in Autoimmune Limbic Encephalitis: Evidence from an Immunotherapy-Naïve Cohort.

OBJECTIVE: Autoimmune limbic encephalitis (ALE) is characterized by memory impairment, psychiatric symptoms, and epileptic seizures. Though, the neuropsychological profile of ALE is not yet well defined. However, there is some evidence that neuropsychological impairments might exceed those related to the limbic system and that different autoantibodies (AABs) are associated with distinguishable pattern of neuropsychological impairments. We provide a comprehensive presentation of neuropsychological performance of ALE in an immune therapy-naïve sample. METHODS: We retrospectively analyzed 69 imm

Moderator effects in a randomized controlled trial of the Common Elements Treatment Approach (CETA) for intimate partner violence and hazardous alcohol use in Zambia.

BACKGROUND: Intimate partner violence (IPV) and hazardous alcohol use are prevalent and co-occurring problems in low- and middle-income countries (LMICs). While limited evidence suggests that cognitive behavioral therapy (CBT) interventions can help address these problems, few randomized trials in LMICs have investigated moderators of treatment effectiveness. This study explores moderating factors impacting responsiveness to a CBT-based intervention for IPV and hazardous alcohol use among couples in Zambia. METHODS: Data were obtained from a completed randomized trial of a CBT-based intervent

Vocal Outcomes in Vagus Nerve Stimulation: A Laryngeal Pattern-Based Objective Analysis.

The aim of our study is to evaluate objective and subjective vocal outcomes in patients undergoing vagus nerve stimulation (VNS) therapy for drug-resistant epilepsy and to assess the vocal outcome in the known laryngeal dysmotility patterns induced by VNS. We enrolled 16 adult patients without cognitive impairment who had undergone VNS implant for drug-resistant epilepsy at least 1 year prior. They were evaluated by flexible fibreoptic laryngeal examination and Voice Handicap Index questionnaire administration; acoustic and perceptual voice analysis was performed both at rest and during VNS ac

The effect of selective serotonin reuptake inhibitors on the frequency of psychogenic nonepileptic seizures: a critically appraised topic.

BACKGROUND: Psychogenic nonepileptic seizures (PNES) are commonly encountered problem in neurological practice and usually are accompanied by other psychiatric comorbidities. Despite its prevalence and profound impact on patients and families, there have been few trials addressing treatment. Cognitive behavioral therapy may be effective but the role of pharmacologic therapy remains unclear. OBJECTIVE: To critically evaluate evidence that PNES frequency may be reduced by treatment with selective serotonin reuptake inhibitors. METHODS: The objective was addressed through the development of a s

Intact bilateral resting-state networks in the absence of the corpus callosum.

Temporal correlations between different brain regions in the resting-state BOLD signal are thought to reflect intrinsic functional brain connectivity (Biswal et al., 1995; Greicius et al., 2003; Fox et al., 2007). The functional networks identified are typically bilaterally distributed across the cerebral hemispheres, show similarity to known white matter connections (Greicius et al., 2009), and are seen even in anesthetized monkeys (Vincent et al., 2007). Yet it remains unclear how they arise. Here we tested two distinct possibilities: (1) functional networks arise largely from structural con

Citalopram augmentation for subsyndromal symptoms of depression in middle-aged and older outpatients with schizophrenia and schizoaffective disorder: a randomized controlled trial.

BACKGROUND: Subsyndromal symptoms of depression (SSD) in older outpatients with schizophrenia are common and clinically important. While many physicians prescribe antidepressants to patients with schizophrenia and schizoaffective disorder who have SSD, evidence for their effectiveness and safety has been meager. We describe a randomized placebo-controlled trial of citalopram in 198 patients. METHOD: Participants in this 2-site study, conducted from September 1, 2001, to August 31, 2007, were men and women with DSM-IV schizophrenia or schizoaffective disorder who were 40 years of age or older

Influence of light-exposure during late incubation and early feeding on the performance of layer chicks in a multitasking test and step detour test.

During on-farm hatching, chicken embryos experience continuous light during late incubation, and have immediate access to feed and water post-hatch. While on-farm hatching compared to hatchery-hatching improves several welfare indicators in broilers, the combined effects of light exposure and early feeding on behavioral and cognitive development in broilers and layers remains unclear. On-farm hatching systems are expected to expand into the laying hen sector due to the advances regarding in ovo-sexing and vaccination technologies. We investigated effects of continuous light exposure from 18 un

Differential Myeloarchitecture of Subfields of the Mammalian Auditory Cortex: A Comparative Approach.

BACKGROUND: The mammalian auditory cortex can be parcellated into multiple functional subfields, with each subfield making a distinct contribution to sound processing. For example, primary fields consisting of primary auditory cortex and anterior auditory field are the first to receive information from the thalamus, and the neurons in each of these fields have different properties in terms of latency and response duration. Non-primary auditory fields consist of secondary auditory cortex, which is involved in object recognition and emotional conditioning, while dorsal auditory fields are more r

Flemish critical care nurses' experiences regarding the influence of work-related demands on their health: a descriptive interpretive qualitative study.

BACKGROUND: Critical care nurses (CCNs) around the globe face other health challenges compared to their peers in general hospital nursing. Moreover, the nursing workforce grapples with persistent staffing shortages. In light of these circumstances, developing a sustainable work environment is imperative to retain the current nursing workforce. Consequently, this study aimed to gain insight into the recalled experiences of CCNs in dealing with the physical and psychosocial influences of work-related demands on their health while examining the environments in which they operate. The second aim w

Effective approaches to public involvement in care home research: a systematic review and narrative synthesis.

BACKGROUND: Public involvement (often referred to as patient and public involvement or PPI) integrates the voices of the public in health and care research. However, groups such as care home residents are often excluded from involvement opportunities due to the complexities of involving people with additional care and communication needs. Despite a range of approaches being used, there is little understanding about how best to incorporate their experiences, and those of other care home stakeholders, into the design and conduct of research. OBJECTIVE: A systematic review was conducted to ident
