[Treatment strategy in first traumatic anterior dislocation of the shoulder. Plea for a multi-stage concept of preventive initial management].

The purpose of this paper is to outline the treatment protocol for the first time traumatic anterior shoulder dislocator, with options including conservative, arthroscopic and open surgical treatment. Regarding the subclassification of the first time traumatic anterior dislocater, it is imparitive to differentiate between the unidirectional dislocator with and without hyperlaxity. This subclassification takes into account the structural quality of the stabilizing ligamentous structures of the glenohumeral capsule. The patient with hyperelastic ligaments exhibit elastic deformation of the gleno

Complication of the use of a radiofrequency device in arthroscopic shoulder surgery: second-degree burn of the shoulder girdle.

Radiofrequency ablation (RF) energy devices are increasingly being used in arthroscopic surgery. This is a case report of a 53-year-old man who had second-degree burns of the shoulder and chest wall as a result of fluid overheating due to RF. During an extensive arthroscopic bursal dissection of the left shoulder, the suction device on the RF wand and the valve was left open, allowing fluid to drip onto the chest and arm of the patient. Three days after his surgery, on his first postoperative visit, the dressings were removed, and the patient was noted to have severe blistering on the lateral

Massive rotator cuff tears repaired on top of humeral head by McLaughlin's procedure.

The clinical and radiographic outcomes of McLaughlin's procedure for massive rotator cuff tears were investigated in 25 shoulders, in which the cuff tears were so severe that the tendons were sutured on the top of the humeral head. The mean age at surgery was 62.2 years (range, 39-74 years). The mean follow-up period was 50 months (range, 24-80 months). The University of California, Los Angeles score significantly improved from 10.9 to 31.8 points postoperatively; the postoperative result was classified as excellent in 11 shoulders, good in 11, and poor in 3. Osteoarthritis progressed postoper

Minimum Effective Volume of Liposomal Bupivacaine for Interscalene Block in Arthroscopic Rotator Cuff Repair: A Prospective, Biased-Coin Up-and-Down Dose-Finding Study.

BACKGROUND: Establishing the minimum effective volume (MEV90) of liposomal bupivacaine for ultrasound‑guided interscalene brachial plexus block (ISB) is essential to optimize postoperative analgesia and minimize local anesthetic-related side effects in patients undergoing arthroscopic rotator cuff repair. Determining this optimal volume provides a clinically meaningful reference for achieving consistent nerve block success while reducing unnecessary drug exposure and the risk of systemic toxicity. Therefore, the present study aimed to identify the MEV90 of liposomal bupivacaine for ISB in pati

Effect of humeral lengthening on post-operative pain scores by three-dimensional measurements in patients undergoing reverse shoulder arthroplasty.

PURPOSE: Humeral lengthening (HL) has been reported to correlate with post-operative clinical outcomes of reverse shoulder arthroplasty (RSA). However, these are simple radiographic evaluations, and three-dimensional (3D) assessments using computed tomography (CT) images have not been considered. This study aimed to investigate the factors that influence post-operative clinical outcomes of RSA implant placement using a 3D evaluation system, specifically, the effect of HL on post-operative clinical outcomes. METHODS: This retrospective study included 49 patients who underwent RSA using the Exa

Preoperative handgrip strength can predict early postoperative shoulder function in patients undergoing arthroscopic rotator cuff repair.

BACKGROUND: Rotator cuff tears (RCTs) are a common musculoskeletal disorder, and arthroscopic rotator cuff repair (ARCR) is widely performed for tendon repair. Handgrip strength correlates with rotator cuff function; however, whether preoperative grip strength can predict functional outcomes in patients undergoing ARCR remains unknown. This study aimed to investigate the correlation between preoperative grip strength and postoperative shoulder function following ARCR. METHODS: A total of 52 patients with full-thickness repairable RCTs were prospectively enrolled. Baseline parameters, namely p

Use sonoelastography to predict the reparability of large-to-massive rotator cuff tears.

INTRODUCTION: Most symptomatic large-to-massive rotator cuff tears (RCTs) should be operated, but the surgical reparability depended on the degree of rotator cuff muscle atrophy or fatty infiltration. The orthopedic surgeons will decide whether the teared stump is reparable during the surgery, but preoperative evaluation can be done by some assessment tools. Magnetic resonance imaging (MRI) was used in recent studies to predict the reparability of large-to-massive RCTs, but the clinical availability was not as good as ultrasound. We hypothesize that the ultrasound elastography can predict the

Risk Factors for Rotator Cuff Disease: An Experimental Study on Intact Human Subscapularis Tendons.

Although several studies revealed a multifactorial pathogenesis of degenerative rotator cuff disorders, the impact and interaction of extrinsic variables is still poorly understood. Thus, this study aimed at uncovering the effect of patient- and pathology-specific risk factors that may contribute to degeneration of the rotator cuff tendons. Between 2015 and 2018, 54 patients who underwent arthroscopic shoulder surgery at three specialized shoulder clinics were prospectively included. Using tendon samples harvested from the macroscopically intact subscapularis (SSC) tendon, targeted messenger R
