Improving the Quality of Lateral Wrist Radiographs: A Closed-Loop Audit of Radiographic Compliance With Standard Positioning Criteria.

Background Wrist injuries represent a substantial proportion of acute musculoskeletal presentations, with distal radius and carpal fractures among the most frequently encountered upper-limb fractures in emergency and orthopaedic services. Accurate lateral wrist radiographs are essential for detecting osseous and carpal alignment abnormalities, guiding management, and preventing complications. However, radiographic quality is highly dependent on adherence to established positioning standards. Inadequate positioning may obscure key anatomical relationships, contributing to diagnostic errors, tre

Decreased Radial Inclination Without Dorsal Angulation Attenuates the Biomechanical Effects of Ulnar Abutment Syndrome: A Finite Element Analysis.

PURPOSE: To evaluate, using finite element analysis, how progressive loss of radial inclination (RI) influences ulnar abutment mechanics when volar tilt (VT) is maintained as normal in association with increasing ulnar variance. METHODS: Nine fresh-frozen cadaveric forearms were scanned using computed tomography and reconstructed into finite element models. Five models were generated from each specimen: anatomical alignment (RI 0°) and four malunion models with stepwise loss of radial inclination, while maintaining a normal VT; this deformation resulted in a corresponding increase in ulnar va

Predictors of ideal outcomes after reconstructive surgery for chronic Monteggia fracture in children.

BACKGROUND: We aimed to analyze surgical outcomes of reconstructive surgery for chronic Monteggia fracture, which is one of the most challenging problems for orthopaedic surgeons. METHODS: Twenty-eight patients were followed during 55.6 ± 32.0 months after open reduction of the radial head combined with ulnar osteotomy and annular ligament reconstruction. The mean interval from trauma was 15.1 ± 19.6 months, and the mean age at the surgery was 9.1 ± 3.5 years. In addition to the chronological age, elbow maturity was assessed using the Sauvegrain score. Surgical outcomes were primarily evaluat

Apparent Proximal Ulna Dorsal Angulation Variation Due to Ulnar Rotation.

OBJECTIVE: To investigate the effect of ulna rotation on the apparent proximal ulna dorsal angulation (PUDA). METHODS: Computed tomography images of 59 ulnas were included in this study, 48 being bilateral specimens and the remaining 11 were unilateral. Three-dimensional models of the entire ulna were obtained, and the ulnas were rotated in 5-degree increments in internal rotation or external rotation from neutral. PUDA, PUDA apex, varus angulation, and varus apex were measured on each ulna. RESULTS: With the ulna in neutral rotation, the mean (95% CI) PUDA was 3.7 (2.9-4.5) degrees, whereas

Micro-CT Structural Analysis of the Canine Medial Coronoid Disease.

OBJECTIVE: To use micro-computed tomography (CT) to evaluate the fissure-fracture pattern in dogs affected by medial coronoid disease (MCD). STUDY DESIGN: Prospective case-controlled study. SAMPLE POPULATION: Client owned (n = 21) and cadaver dogs (n = 5). METHODS: Segments were excised by subtotal coronoid ostectomy from dogs with MCD. Two categories were identified: fissured (Fi) or fractured (F). Three subcategories were contingent on fissure-fracture orientation: tip (T), radial incisure (RI), and radial incisure encroaching tip (RIT). Control segments were from nondiseased cadavers. Hi

Repair of thumb metacarpophalangeal joint ulnar collateral ligament injuries with microanchors.

INTRODUCTION: Ulnar collateral ligament (UCL) injuries of the metacarpophalangeal (MCP) joint of the thumb are common. Complete rupture can be a debilitating injury, resulting in decreased grip and pinch strength. PURPOSE: The present study evaluated prospectively the functional results of 27 patients who had open repair of the UCL of the thumb using microanchors either acutely or delayed (up to 9 weeks postinjury). METHODS: Through a standard S-shaped incision over the dorsoulnar aspect of the thumb, one or two 1.5-or 1.3-mm microanchors are fixed to the base of the proximal phalanx in the

The repair of large segmental bone defects in the rabbit with vascularized tissue engineered bone.

Management of segmental bone defects is a considerable challenge for orthopedic surgeons. Tissue engineering is a promising method for repairing bone defects, and vascularization is critical to the performance of a tissue engineered bone. We report herein the construction of a vascularized tissue engineered bone with mesenchymal stem cells (MSCs) and MSC-derived endothelial cells (ECs) co-cultured in porous beta-tricalcium phosphate ceramic (beta-TCP) to repair 1.5-cm ulnar defects in the rabbit. Examination by X-ray and single photon emission computed tomography (SPECT), histologic analysis,

Sauvé-Kapandji operation for disorders of the distal radioulnar joint after Colles' fracture. Good results in 12 patients followed for 1.5-4 years.

Controversies continue concerning the treatment of the posttraumatic caput ulna syndrome. We have treated 12 patients, mean age 42 (23-77) years, with arthrodesis ad modum Sauvé-Kapandji of the distal articulation between the radius and ulna, combined with resection of the ulnar neck. Before the operation, all patients had persistent ulnar wrist pain and restricted pronation-supination movement. At follow-up, after a mean of 2 (1.5-4) years, 8 patients had an excellent outcome, 3 a good, and 1 patient had a fair outcome. 10 patients had no wrist pain. The average grip-score improved from 53% p

[Operative long-term results in delayed radius head dislocation in childhood].

Only some cases and no long term follow-ups are reported in literature concerning the problem of delayed luxations of the radial head in children. 21 cases (19 acquired and 2 inherited) were treated in our hospital by operative procedure 12.5 months after the accident on the average. Disregarding one early and one late resection of the radial head we reconstructed the joint reposition and annular ligament plasty and if necessary correction osteotomy of the ulnar or shortening of the radius. Follow-up examination took place between 2 and 16 years after the operation, 7.9 years on the average. A

Influence of anteromedial facet defects of the coronoids process on the posterior bundle of the medial collateral ligament in human cadaveric specimens.

BACKGROUND: Anteromedial facet (AMF) fractures of the coronoid process are typically accompanied by tears of the lateral collateral ligament (LCL) and the posterior bundle of the medial collateral ligament (pMCL). Addressing the bone defect and repairing the LCL is essential in the acute setting to prevent persistent varus posteromedial rotatory instability (VPMRI) and subsequent progression to osteoarthritis. However, pMCL repair is not typically performed. Therefore, the present biomechanical study investigates the amount of stress experienced by the pMCL in cases of anteromedial facet defec

Effect of ulnar angulation and soft tissue sectioning on radial head stability in anterior Monteggia injuries: an in vitro biomechanical study.

BACKGROUND: Radial head instability continues to be a challenge in the management of anterior Monteggia injuries; however, there is a paucity of literature on the factors that contribute to this instability. The aim of this biomechanical investigation was to examine the effects of ulnar angulation and soft tissue insufficiency on radial head stability in anterior Monteggia injuries. METHODS: Six cadaveric arms were mounted in an elbow motion simulator. Radial head translation was measured during simulated active elbow flexion with the forearm supinated. After testing the elbows in the intact

Results of non-operative treatment of olecranon fracture in over 75-year-olds.

INTRODUCTION: Surgery is the gold-standard treatment of displaced olecranon fracture, but is associated with numerous complications, especially in the elderly. Functional results of non-operative treatment in this population have never been analyzed in a prospective study. STUDY HYPOTHESIS: Non-operative treatment of isolated olecranon fracture with stable elbow-joint in over 75-year-olds gives functional results comparable to those of surgery as reported in the literature, with fewer complications. MATERIAL AND METHODS: A prospective study analyzed functional results of non-operative treatm

Convergent type proximal radioulnar translocation complicating posterior elbow dislocation: watch out for dual dislocations in children's elbows.

Proximal radioulnar translocation associated with posterior dislocation of the elbow is an extremely rare combination of trauma to the elbow in children. We report on the successful and sequential closed reduction of both these dislocations in a 10-year-old girl in which the translocation of the forearm bones was initially missed. A concomitant fracture would have probably precluded an ideal outcome as we note from the literature review on this problem.

Metachronous and multiple aneurysmal bone cysts: a rare variant of primary aneurysmal bone cysts.

In 1942, Jaffe and Lichtenstein introduced the term aneurysmal bone cyst (ABC). Primary ABC is characterized by the presence of spongy or multi-cameral cystic tissue filled with blood. The process is benign, but it is locally destructive and has a high propensity for recurrence. In this paper, we present the third case of multiple metachronous primary ABCs as a rare variant of ABC. We describe the 10-year history of a 12-year-old boy with metachronous multiple primary ABCs at five different sites (right proximal humerus, right ulna, bilateral distal radius and right lateral clavicle).
