Tuberculous Encephalitis May Be Undetectable on Magnetic Resonance Imaging but Detectable on 18F-Fluorodeoxyglucose Positron Emission Tomography-Computed Tomography.

Neurotuberculosis (NT) continues to be a global health problem with severe morbidity and mortality. The manifestations of NT are well-known and encompass forms such as meningitis, tuberculomas, military tuberculosis, ventriculitis, and brain abscess. Data of all patients with central nervous system tuberculosis who underwent magnetic resonance imaging (MRI) and/or 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography-computed tomography (PET-CT) were analyzed. Over a 7-year period (2014-2021), we encountered three patients who had dense neurological deficits and 18F-FDG PET-CT results …

Extradural abscess secondary to Salmonella enteritidis in a child following fronto-orbital facial advancement and remodeling surgery.

Intracranial infections caused by Salmonella are rare. We describe the first case of a child undergoing craniofacial surgery for trigonocephaly and subsequently developing an extradural abscess secondary to likely community-acquired Salmonella enteritidis. He underwent surgical washout but returned to theater for a further 2, alongside a prolonged course of intravenous ciprofloxacin. We observed extensive anterior skull bone loss at 78 days postoperatively. At 1 year 11 months, extensive anterior skull bone remodeling had taken place, and the child is currently well.

Non-missile penetrating brain injury-surgical techniques for removing a long penetrating foreign body: a case report.

PURPOSE: Penetrating brain injury (PBI), a relatively uncommon injury, is associated with remarkable secondary complications such as vascula … etallic penetrating foreign body and eliminating the accompanying frontal ICH simultaneously. Focal brain abscess developed 8 days after the injury and resolved completely after antibiotics treatment. Dysphasia gradually improved but right distal limbs weakness with spasticity is still present. CONCLUSIONS: Our findings suggest prompt diagnosis by preoperative …

['I was murdered': Cotard's syndrome due to retrotonsillar abscess].

Cotard's syndrome is a rare and severe neuropsychiatric disorder characterized by nihilistic delusions, where patients believe they are dead or do not exist. It's often associated with depression or schizophrenia but may also occur secondary to somatic conditions such as epilepsy, brain tumors, and neurodegenerative diseases. This case involves a 30-year-old man with no prior psychiatric history, who was brought to the emergency department due to confusion and the delusion that he was dead. Examination revealed a retrotonsillar abscess. Although the patient initially lacked classic delirium fe

A case report of disseminated aspergillosis in an immunocompetent patient proven by choroid plexus biopsy.

We present a case of disseminated aspergillosis diagnosed by choroid plexus biopsy in an immunocompeten 69-year-old male with decreased consciousness and a fever for more than three months. Brain magnetic resonance imaging showed ventriculitis and phlegmon in the paravertebral space. Chest computerized tomography revealed an air-crescent lesion in the upper lobe of the left lung. No microorganism was identified from the blood or CSF cultures. A choroid plexus biopsy was performed and the pathology findings were consistent with fungal abscess caused by invasive aspergillosis. Since disseminated

Ulceronecrotic Eschar as a Cutaneous Clue to Disseminated Melioidosis.

Melioidosis is a potentially fatal infection caused by Burkholderia pseudomallei that is often underdiagnosed because of its diverse clinical presentations. We report the case of a 49-year-old female farmer from Tamil Nadu who presented with a 10-day history of fever, productive cough, and breathlessness followed by a rapidly progressive ulceronecrotic lesion on the forehead for 6 days. Examination revealed a 3 × 2 cm crusted eschar with surrounding edema. Blood, pus swab, and tissue bit grew B. pseudomallei, and imaging showed multiple abscesses in the lungs, liver, spleen, and brain. She was

A Complicated Case of Serratia marcescens Infective Endocarditis in the Era of the Current Opioid Epidemic.

While bacteremia due to Serratia marcescens is not uncommon, it rarely causes infective endocarditis. We report an isolated case of a 53-year-old male with history of intravenous drug abuse who presented with multiple acute pain symptoms and was found to have S. marcescens bacteremia with septic emboli in his spleen, brain, and testes, secondary to a large aortic vegetation, as well as aortic infective endocarditis with severe aortic regurgitation requiring aortic valve replacement. His course of disease was further complicated by epidural and psoas abscesses and a necrotic testicle requiring

Pulmonary arteriovenous fistula within a pulmonary cyst - evaluation with CT pulmonary angiography.

Pulmonary arteriovenous malformations (PAVMs) are abnormal direct communications between pulmonary arteries and pulmonary veins. These abnormal communications result in an anatomical right-to-left shunt that reduces the arterial oxygen saturation and may cause hypoxaemia and dyspnoea. Although PAVMs frequently remain undiagnosed, they are associated with severe morbidity in the form of ischaemic strokes and brain abscesses. We report a case of incidental CT angiography depiction of a PAVM within a pulmonary cyst. To the best of our knowledge, no such case has been described previously. On the …

Interventional treatment of pulmonary arteriovenous malformations.

Pulmonary arteriovenous malformations (PAVM) are congenital vascular communications in the lungs. They act as right to left shunts so that the blood running through these malformations is not oxygenated or filtered. These patients are typically hypoxaemic with exercise intolerance and are at high risk of paradoxical emboli to the brain and other organs. These malformations are most commonly seen in hereditary haemorrhagic telangiectasia (HHT) (Mb. Osler-Weber-Rendu syndrome). Nowadays, the generally accepted treatment strategy of first choice is embolization of the afferent arteries to the art

Clinical experience with linezolid for the treatment of nocardia infection.

Linezolid is an oxazolidinone that has activity against most gram-positive bacteria, including in vitro activity against all Nocardia species and strains. We describe 6 clinical cases of nocardiosis that were successfully treated with linezolid. Two patients had underlying X-linked chronic granulomatous disease, and 2 patients were receiving chronic corticosteroid therapy. Four of 6 patients had disseminated disease, and 2 of these 4 patients had multiple brain abscesses. Four patients primarily received monotherapy; for the fifth patient, linezolid was added to a failing multiple-drug regimen…
