Fragile X premutation in women: recognizing the health challenges beyond primary ovarian insufficiency.

Fragile X premutation carriers have 55-200 CGG repeats in the 5' untranslated region of the FMR1 gene. Women with this premutation face many physical and emotional challenges in their life. Approximately 20% of these women will develop fragile X-associated primary ovarian insufficiency (FXPOI). In addition, they suffer from increased rates of menstrual dysfunction, diminished ovarian reserve, reduction in age of menopause, infertility, dizygotic twinning, and risk of having an offspring with a premutation or full mutation. Consequent chronic hypoestrogenism may result in impaired bone health a

[Psychiatric disorders in pregnant and puerperal women].

The most frequently occurring pre- and postpartum psychiatric disorders are depression and, to a lesser degree, panic disorder and psychosis. Apart from the negative effects on the psychological well-being of the mother, these psychiatric disorders may also result in obstetric complications and an impaired mother-infant relationship. In order to prevent these negative effects, mothers who are at risk for major psychiatric disorders need to be identified early, preferably before or during pregnancy. The most important risk factor is a history of psychiatric disorders. Obstetric risk factors for

Counseling pregnant women treated with paroxetine. Concern about cardiac malformations.

QUESTION: I have always reassured my patients that taking selective serotonin reuptake inhibitors (SSRIs) during pregnancy would not increase their risk of having children with major malformations. A recent warning from Health Canada, based on results of a study from GlaxoSmithKline, stated that infants exposed to paroxetine might be at higher risk of congenital malformations, specifically cardiovascular defects. Some of my pregnant patients who are taking paroxetine heard the warning and asked me whether they should stop taking it. What should I tell them? ANSWER: The new warning is based on

Infant social withdrawal and rapid infant weight gain.

BACKGROUND: Rapid infant weight gain (RIWG) and infant social withdrawal are important early markers of risk of subsequent overweight/obesity and mental health problems, respectively. While overweight and mental health problems are linked later in life, it remains unclear if this association is present already in infancy. OBJECTIVE: We examine if infant social withdrawal is associated with subsequent RIWG independently of an array of known risk factors (gestational age, birth weight, sex, parity, breastfeeding, maternal postpartum depressive symptoms, maternal smoking during pregnancy, matern

Developing a statewide public health initiative to reduce infant mortality in Oklahoma.

The Preparing for a Lifetime, It's Everyone's Responsibility initiative was developed to improve the health and well- being of Oklahoma's mothers and infants. The development phase included systematic data collection, extensive data analysis, and multi-disciplinary partnership development. In total, seven issues (preconception/interconception health, tobacco use, postpartum depression, breastfeeding, infant safe sleep, preterm birth, and infant injury prevention) were identified as crucial to addressing infant mortality in Oklahoma. Workgroups were created to focus on each issue. Data and medi

Analysis of the Retinal Microcirculation in Chinese Patients With Vogt-Koyanagi-Harada Disease by Optical Coherence Tomography Angiography.

IMPORTANCE: Depressive symptoms during pregnancy influence the development and health of the offspring, underscoring the need for timely intervention. However, the course of depressive symptoms across the perinatal period remains unclear, thus complicating screening and referral guidelines. OBJECTIVE: To examine the course and stability of depressive symptoms across the perinatal period in multiple, ethnically diverse independent observational cohorts. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included self-reported depressive symptoms at multiple time points from 7 prospective co

Recent life events and adverse childhood experiences in predicting antenatal depression and anxiety: cross-sectional study.

BACKGROUND: Antenatal depression and anxiety are associated with preterm labour, low birth weight, and postpartum depression, and can impact the emotional and mental development of the child. Both adverse childhood experiences and recent stressful events are linked to negative health outcomes. However, certain events may be more impactful than others. OBJECTIVE: This study aims to investigate the most important types of adverse childhood events and recent stressors that may help in identifying pregnant women in most need of psychological support. METHODS: This observational study examines da

The Mental Health of Women with Gestational Diabetes During the COVID-19 Pandemic: An International Cross-Sectional Survey.

Background: There is evidence that women with gestational diabetes are at increased risk of the common mental disorders of anxiety and depression. The COVID-19 pandemic may have exerted an additional burden on the mental health of this population. The aim of this analysis was to compare levels of symptoms of common mental disorders and experiences during the COVID-19 pandemic between pregnant and postnatal women exposed and unexposed to gestational diabetes. Methods: Cross-sectional study utilizing quantitative data from an online survey administered across 10 countries to women who were pregn

Validation of the MGMQ in screening for emotional difficulties in women during pregnancy.

BACKGROUND: Screening for emotional health difficulties in pregnant or postpartum women is becoming routine practice in health services. This screening is often done using the EPDS, usually using a screen positive score just to identify possible depression. This and other such scales often have a myriad of screen-positive scores, making them impractical within clinical settings. The recent MGMQ screens for a variety of negative moods, is brief, simple, and has just a few screen-positive thresholds. METHOD: At recruitment 391 women attending routine antenatal clinics completed various mood que

Developmental fluoxetine exposure and prenatal stress alter sexual differentiation of the brain and reproductive behavior in male rat offspring.

Depression during pregnancy and postpartum is a significant health problem and affects up to 20% of women. While selective serotonin reuptake inhibitor (SSRI) medications are the drug of choice for treatment of maternal depression, the combined effect of maternal depression and perinatal SSRI exposure on offspring development is poorly investigated. Our aim was to determine the role of exposure to fluoxetine during development on sexual behavior and sexually dimorphic brain structures in male offspring using a rodent model of maternal adversity. Sprague-Dawley rat dams were stressed during ges

Changes in adaptability following perinatal morphine exposure in juvenile and adult rats.

The problem of drug abuse among pregnant women causes a major concern. The aim of the present study was to examine the adaptive consequences of long term maternal morphine exposure in offspring at different postnatal ages, and to see the possibility of compensation, as well. Pregnant rats were treated daily with morphine from the day of mating (on the first two days 5mg/kgs.c. than 10mg/kg) until weaning. Male offspring of dams treated with physiological saline served as control. Behavior in the elevated plus maze (EPM; anxiety) and forced swimming test (FST; depression) as well as adrenocorti

Children's appraisals of maternal depression and responses to emotional faces in early-adolescence: An Event Related Potential (ERP) study.

BACKGROUND: Parental depression represents a significant risk for depression development in offspring. While cognitive mechanisms represent a central risk pathway, children's appraisals of parental symptoms have been understudied. This study examined associations between children's self-blame, threat, and frequency/duration appraisals for maternal symptoms in relation to cognitive control and emotional response processes. METHODS: Sixty mother-daughter (aged 10-14-years) pairs participated. Affective processing was assessed by three Event Related Potential (ERP) components, the N2, P3, and LP

Antidepressant use in pregnant and postpartum women.

Women in their reproductive years are at risk of experiencing depressive and anxiety disorders. As such, it is likely that pregnant women will undergo treatment with antidepressants. We review the risk of adverse birth outcomes and neonatal complications subsequent to antidepressant use in pregnancy. An inconsistent literature shows that antidepressant exposure is associated with shortened gestations and diminished fetal growth; these effects are small. Transitory neonatal signs are seen in some neonates after exposure to antidepressants in utero. No specific pattern of malformations has been

Maternal use of SSRIs, SNRIs and NaSSAs: practical recommendations during pregnancy and lactation.

Selective serotonin reuptake inhibitors (SSRIs) are increasingly used during pregnancy and lactation, with 1.8-2.8% exposed pregnancies. Given the risks of untreated maternal depression for both mother and child, adequate treatment is essential. If pharmacological treatment with SSRIs is indicated, the fetal and neonatal effects of SSRIs have to be considered, as SSRIs cross the placenta and are excreted into breast milk. The overall risk of major congenital malformations during SSRI exposure in the first trimester does not appear to be greatly increased. Depending on the variability in pharma

Antidepressant Medication Use During Lactation: A Review for Providers.

This article presents a summary of information found within the existing medical literature on the pharmacological treatment options for maternal depression during lactation and the concurrent effects on the breastfeeding infant. Existing data on safety and efficacy varies by treatment modality. Medications used to treat depression are all secreted in breast milk to some extent; however, most antidepressants are considered relatively safe to use during breastfeeding. The selective serotonin reuptake inhibitors (SSRIs) sertraline and paroxetine are present in low levels and are considered prefe

Mental disorder in pregnancy and the early postpartum.

Perinatal mental illness is common, affecting up to 20% of women, but remains under-recognised and under-diagnosed. It may have adverse effects on pregnancy and neonatal outcomes, and mental disorder remains one of the leading causes of maternal death in the UK. Women with mental ill health face difficult decisions in balancing risks and benefits of treatment. Stigma related to mental disorder may lead to non-engagement with maternity care. Some disorders bring specific challenges for anaesthetists working in maternity settings and it is vital that anaesthetists have knowledge of these disorde
