Impact of a VLBW feeding guideline on central line usage and enteral feeding practices: A quality improvement project.

BACKGROUND: Feeding guidelines (FGs) reduce the incidence of necrotizing enterocolitis (NEC) and improve growth and enteral nutrition (EN) practices in very low birth weight (VLBW) infants. Our unit's baseline data revealed delayed initiation of EN, prolonged time to full EN, prolonged days on parenteral nutrition (PN), and frequent use of peripherally inserted central catheters (PICC). After introduction of this FG, our specific, measurable, attainable, realistic, and time-bound (SMART) aims were to reduce PICC placements in infants 1000-1500 g by 30% and to reduce the time to achieve full EN

Japan at the limits of viability: national experience and evolving outcomes of infants born at 22-23 weeks' gestation.

Infants born at 22-23 weeks' gestation represent the limits of viability with substantial international variation in outcomes. Japan has consistently reported some of the highest survival rates; however, the underlying healthcare systems and clinical practices have not been comprehensively synthesized. This review summarizes Japan's national experience using data from a nationwide network database, epidemiologic studies, clinical practice surveys, and international comparisons. Survival at discharge is approximately 60% and 80% for infants born at 22 and 23 weeks' gestation, respectively, with

Patency of the Ductus Arteriosus at 72 h in Extremely Preterm Infants Born at Two Altitudes in Colombia: A Preliminary Descriptive Study.

Patent ductus arteriosus (PDA) is common in preterm neonates, especially before 28 weeks' gestation, and is associated with morbidity such as bronchopulmonary dysplasia, hemorrhage, and necrotizing enterocolitis. Beyond immaturity, perinatal factors, postnatal interventions, and altitude, may affect ductal persistence; altitude is proposed as a modifier, though evidence is limited. The objective of the study is to describe the prevalence, the clinical, and echocardiographic characteristics of PDA at 72 h of life in neonates born before 28.6 weeks of gestation in two Colombian cities located at

Study of the gut microbiome profile in full-term infants with necrotizing enterocolits.

BACKGROUND: Necrotizing enterocolitis (NEC) is a severe gastrointestinal disease that primarily affects preterm. Recently, cases with similar clinical manifestations have been observed in full-term infants without Hirschsprung's disease. Dysbiosis suggested to have role in development of NEC. AIM: To compare the gut microbiome composition between full term infants with NEC and healthy controls and to evaluate the impact of feeding sources on microbial diversity. METHODS: Ten full term infants with NEC and six matched healthy breastfed control infants were enrolled in the study. Triplicate st

"Unveiling thyroid dysfunction in preterm neonates": A prospective observational study.

BackgroundThyroid dysfunction is common in preterm neonates due to immaturity of hypothalamic pituitary thyroid axis, increased demand and drug exposure. The dysfunctions seen in early days may get corrected later and may not require treatment. Guidelines recommend repeated screening for preterm. This study was planned to estímate the incidence of thyroid dysfunction requiring thyroxine replacement therapy within 40 weeks of corrected gestational age in preterm neonates ≤32 weeks.MethodologyThis prospective observational study included 421 preterm neonates born ≤32 weeks and admitted in the NI

Stapled versus hand-sewn intestinal anastomosis in pediatric surgery: A systematic review and meta-analysis.

BACKGROUND: Intestinal anastomosis is a core procedure in pediatric gastrointestinal surgery, performed for conditions such as necrotizing enterocolitis, intestinal atresia, Hirschsprung's disease, and inflammatory bowel disease. Although stapled anastomosis (SA) may improve operative efficiency, its safety and effectiveness compared with hand-sewn anastomosis (HA) in children remain uncertain. This meta-analysis compared clinical outcomes of SA and HA in pediatric patients. METHODS: The study followed PRISMA guidelines and was prospectively registered in PROSPERO (CRD420251177257). Previable Preterm Premature Rupture of the Membranes Reaching Viability: Canadian Outcomes.

OBJECTIVE: To determine timing of birth and neonatal outcomes after previable preterm premature rupture of the membranes (PPROM) <22 weeks versus no prolonged PPROM, to aid perinatal counselling at viability. METHODS: We conducted a retrospective cohort study of 8071 liveborn singletons born <32 weeks between 2018-2022, who received active care and died in the resuscitation room or were admitted to a neonatal intensive care unit in Canada. Associations between previable PPROM and birth timing (and latency period) were investigated. Outcomes were compared between previable PPROM and no prolong

Microbiota-Modulating Strategies in Neonates Undergoing Surgery for Congenital Gastrointestinal Conditions: A Narrative Review.

Background/Objectives: The gut microbiota (GM) is pivotal for immune regulation, metabolism, and neurodevelopment. Infants undergoing surgery for congenital gastrointestinal anomalies are especially prone to microbial imbalances, with a paucity of beneficial bacteria (e.g., Bifidobacteria and Bacteroides) and diminished short-chain fatty acid production. Dysbiosis has been associated with severe complications, including necrotizing enterocolitis, sepsis, and feeding intolerance. This narrative review aims to critically examine strategies for microbiota modulation in this high-risk cohort. Meth

Gut Health for Two: the Critical Role of Probiotics and Prebiotics in Pregnancy and Lactation.

PURPOSE OF REVIEW: This review summarizes current research on the effects of probiotics and prebiotics during pregnancy and lactation. RECENT FINDINGS: During pregnancy and lactation, various changes occur in maternal hormone levels and physiological processes to ensure the most favorable intrauterine environment. These changes may vary depending on factors such as maternal nutritional status and mode of delivery, and they can also lead to alterations in the maternal gut microbiota, breast milk microbiota, and neonatal gut microbiota. In this context, probiotic and prebiotic supplementation i

Antifungal prophylaxis-Where are we today?

In the past decades more infants of lower gestational ages (GAs) have developed invasive Candida infection (ICI) from nonmodifiable risk factors of immature immunity and underdeveloped immune barriers (skin, gastrointestinal and respiratory tracts). Rates of ICI vary secondary to modifiable clinical practices, resuscitation practices and the nonmodifiable factor of the number of infants cared for at the lowest GAs of 22-24 weeks. Standardization of ICI definitions and reporting by each GA are needed to optimally elucidate actual rates, outcomes and differences across units, countries and regio

Obstetric and neonatal outcomes in clinically diagnosed placental abruption with and without placental histopathologic confirmation-A retrospective study.

OBJECTIVE: Placental abruption is a major obstetrical complication bearing potentially life-threatening consequences. Though the diagnosis of placental abruption is a clinical one, the nonspecific nature of common presenting symptoms, mainly vaginal bleeding and contractions, pose a challenge, and the contribution of placental pathology has been debated. In this study, we assessed the clinical significance of related placental lesions by comparing obstetric and neonatal outcomes among cases of placental abruption with versus without supporting histopathologic placental lesions. METHODS: We re
