A female patient with a chronic pain syndrome after multiple operation on her muskuloskeletal system presented with a displaced pain pump in her lower left abdominal region. After an excessive weight loss due to gastric banding the patient developed a massive pendulous abdomen causing the complication. An interdisciplinary dermatolipectomy together with a refixation of the pump was performed. Vascular injury is a rare but devastating complication of total hip arthroplasty. We present 2 cases of external iliac artery injury that complicated the removal of a chronically infected total hip arthroplasty where the acetabular component had migrated medial to Kohler's line. Small bowel evisceration through the rectum is extremely rare in childhood and has previously been reported in association with swimming pool suction injuries. We report a case of ileal evisceration resulting from a self-inflicted injury in a teenaged boy. Such a mechanism of injury leading to evisceration of the small bowel is previously unreported in children or adults.

Failure of total hip arthroplasty because of osteolysis caused by surface wear of acetabular polyethylene bearings has long been recognized. We report the case of a patient with an unusual presentation of a pelvic mass 20 years after he underwent total arthroplasty of his left hip.

Two cases are reported in which, after ACL reconstruction with autologous hamstring grafts, tibial polylactide interference screws migrated into the knee joint. Clinically, both patients presented with recurrent locking of the joint. In one case, a broken 15 mm-long tip of the screw was found intra-articularly. In the other case, the whole screw had migrated into the joint cavity. The degradation process of polylactic acid, operative technique and bone quality are discussed as possible reasons for these complications.

Five consecutive cases of prosthetic inter-vertebral disc displacement with severe vascular complications on revisional surgery are described. The objective of this case report is to warn spinal surgeons that major vascular complications are likely with anterior displacement of inter-vertebral discs. We have not been able to find a previous report on vascular complications associated with anterior displacement of prosthetic inter-vertebral discs. We present a 75-year-old man who, for 2 weeks, had progressive pain in both of his thighs when standing straight. MR imaging showed a sequestrated disk fragment, which had a signal intensity similar to that of a herniated disk with a rim enhancement in the posterior epidural space and a ruptured outermost annulus of the intervertebral disk at L2-3. Awareness of these MR imaging findings can help in the diagnosis of posterior epidural disk migration.

BACKGROUND: Compression of the left renal vein between the aorta and the superior mesenteric artery is a rare but possibly underestimated condition. Surgical correction (42 cases reported in the literature) can be performed by means of a variety of different techniques. Although endovascular stenting is well accepted for iliocaval occlusive disease, it has been poorly evaluated in this indication. PURPOSE: To study a new surgical option of primary placement of a hydroxyapatite-coated sleeve into the Bioceramic implant during enucleation or evisceration.
DESIGN: Retrospective, observational case series.
METHODS: A standard enucleation or evisceration was performed, followed by the preplacement of a hydroxyapatite-coated sleeve into the Bioceramic implant. Care must be taken to ensure the sleeve has been positioned centrally when the implant is put inside the orbital socket. Complications such as sleeve exposure, Bioceramic implant exposure, and infection were closely observed.

A 48-year-old patient with a scleral-fixated posterior chamber intraocular lens had dislocation of the intraocular lens into the anterior chamber and associated pseudophakic bullous keratopathy. The patient underwent intraocular lens extraction and partial penetrating keratoplasty. Because an anterior chamber intraocular lens is easier to implant and has fewer complications, surgeons may want to consider this as the first choice for treatment. If the scleral-fixated posterior chamber intraocular lens is used, it must be implanted properly and meticulously to avoid complications.

During our day-to-day practice, we, as clinicians, occasionally come across patients whose symptomatology is atypical. In major teaching hospitals, it is usually easy to consult with other specialists to optimize patient management and standard of care. Our study patients were treated by the authors between January 1998 and January 2003. In this article, the authors report on 6 different cases of unexpected causes of pelvic pain, all of which were managed in a general gynecological unit at a major tertiary referral institution.

We report on two cases of duodenocaval fistula. The first patient, a 73-year-old man, had sepsis and occult digestive bleeding. We diagnosed a fistula that resulted from a right nephrectomy and subsequent radiotherapy for a urothelial tumor 20 months earlier. The second patient, a 60-year-old woman, complained of right abdominal pain.
