Clinical characteristics associated with isolated unilateral utricular dysfunction.

PURPOSE: The ocular vestibular evoked myogenic potential (oVEMP) represents the sound-induced activation of extraocular muscles and is believed to originate from the utricle and superior vestibular nerve. Isolated unilateral oVEMP abnormalities with otherwise normal balance function test (BFT) results have not yet been characterized in a large patient series, and their clinical significance remains unclear. MATERIALS AND METHODS: Retrospective review of adult patients with vestibular complaints at a tertiary academic neurotologic referral center was performed. Patients with isolated unilatera

Early computed tomography findings of the inner ear after stapes surgery and its clinical correlations.

OBJECTIVE: Pneumolabyrinth resulting from temporal bone trauma and stapes luxation has been associated with sensorineural hearing loss (SNHL). The principal purpose of this study was to determine the incidence and volume of pneumolabyrinth after stapedotomy in which iatrogenic perilymphatic fistula is created and to also correlate this with possible hearing loss and vertigo. STUDY DESIGN: Prospective study. SETTING: Tertiary referral center. PATIENTS AND METHODS: Fifty stapedotomy patients were operated on for otosclerosis, and of those 50, 20 underwent high-resolution computed tomography (

Postural stability in vestibular neuritis: age, disease duration, and residual vestibular function.

OBJECTIVES/HYPOTHESIS: To assess the influence of factors that can affect postural instability in vestibular neuritis (VN). STUDY DESIGN: Retrospective data collection study. METHODS: Foam posturography was performed in 58 VN patients. We examined six variables: the velocity of movement of the center of pressure and the envelopment area in eyes closed/foam rubber condition, Romberg's ratios of velocity and area with foam rubber, and the foam ratios of velocity and area with eyes closed. Multiple regression analyses were performed to explore the relationship between these variables and the fo

[How to analyse a vestibular evoked myogenic potential? Applying a non-linear [corrected] method].

INTRODUCTION: This article presents the investigation of the clinical utility of artificial neural networks as an alternative in interpreting vestibular evoked myogenic potential (VEMP). METHODS: We developed an application that incorporates a neural network. This software reproduces and analyses the vestibular potential under study. The software also gives an estimated error for this process. RESULTS: The correlation obtained in this study between the neural network and an experienced human observer was always above 95%. CONCLUSION: The neural network created offers useful, reliable interp

Otoconin-90 deletion leads to imbalance but normal hearing: a comparison with other otoconia mutants.

Our sense of gravitation and linear acceleration is mediated by stimulation of vestibular hair cells through displacement of otoconia in the utricle and saccule (the gravity receptor organ). We recently showed that otoconin-90 (Oc90) deletion led to formation of giant otoconia. In the present study, we determined the extent to which the giant otoconia affected balance and gravity receptor sensory input and compared the findings with other otoconia mutants. We employed a wide spectrum of balance behavioral tests, including reaching and air-righting reflexes, gait, swimming, beam-crossing, rotor

Diagnostic value of vestibular evoked myogenic potentials in cerebellar and lower-brainstem strokes.

BACKGROUND: Vestibular evoked myogenic potentials (VEMPs) provide assessment of vestibular function. They consist in picking up compound muscle action potentials in the sternocleidomastoid (SCM) muscles in response to auditory stimulation of the vestibulum. VEMP testing has found application mainly in peripheral vestibular disorders, whereas reports about VEMPs in central vestibular lesions are rather scarce. AIMS OF THE STUDY: Based on the physiological connections between the cerebellum and the vestibular nuclei, we investigated the influence on VEMPs of cerebellar and lower-brainstem strok

Differential effects of labyrinthine dysfunction on distance and direction during blindfolded walking of a triangular path.

While we walk through the environment, we constantly receive inputs from different sensory systems. For us to accomplish a given task, for example to reach a target location, the sensory information has to be integrated to update our knowledge of self-position and self-orientation with respect to the target so that we can correctly plan and perform the remaining trajectory. As has been shown previously, vestibular information plays a minor role in the performance of linear goal-directed locomotion when walking blindfolded toward a previously seen target within a few meters. The present study e

[Follow-up of caloric test response after acute peripheral vestibular dysfunction].

This study examined retrospectively the spontaneous recovery of patients with an acute peripheral vestibular deficit in order to determine whether the caloric test response and with it vestibular function improves over time. The caloric bithermal was tested three times on 79 patients who were hospitalised with an acute deficit. The first test was recorded on emergency admission by observing nystagmus beats under the Frenzel glasses. Two to five days later a complete electronystagmus (ENG) examination was performed. A second ENG was performed, on average, 4 months later. 46% of the patients rec

Evaluation of the human vestibulo-ocular reflex at high frequencies with a helmet, driven by reactive torque.

A new technique was developed to test the VOR in humans. A tightly but comfortably fitting helmet was provided with an electronically controlled torque motor that rotated a mass around a vertical axis. Acceleration of the motor caused reactive torque of the helmet in the opposite direction which was transmitted to the (freely movable) head. Sinusoidal head oscillations in the frequency range 2-20 Hz (peak-to-peak amplitudes about 10 degrees and 0.1 degrees respectively) were easily achieved, as well as step displacements of the head with accelerations on the order of 1000 degrees/s2. Syphilitic labyrinthitis--an update.

Forty-eight patients with syphilitic labyrinthitis have been treated and followed up for periods of six months to 25 years; interpretation of the hearing results must be in the light of the natural history of this condition which is perceived to be progression of hearing loss to profound deafness. At the beginning of the series treatment was with ampicillin alone and on this treatment 11 ears with useful hearing were maintained at this level. The group with combined ampicillin and prednisone treatment, which included some failures from ampicillin alone, maintained useful hearing in 28 out of 2

Management of the open labyrinth.

Labyrinthine fistulas occur in approximately 5% of cholesteatoma cases, but the management of this difficult problem remains controversial. This study assessed the preoperative presentation and outcome in 37 patients operated on for cholestatoma complicated by labyrinthine fistula. Therapy involved removing the matrix from each fistula and reconstructing the bony wall of the labyrinth with bone dust, fibrin glue, and perichondrium. Corticosteroids were added to the management protocol in more recent cases. A fistula classification scheme was introduced to standardize the reporting of the exten

Vestibular compensation in vestibular neuronitis. Long-term follow-up evaluation.

We evaluated vestibular function, especially for positional, head shaking and caloric nystagmus, in 43 patients with vestibular neuronitis, for long periods from the onset. The results suggested that in these cases of vestibular neuronitis, which were studied for more than 10 years after onset of the disease, the disease may still be in the uncompensated stage or may have worsened in the partially compensated stage.

Oculomotor and vestibular anomalies in Pelizaeus-Merzbacher disease: a study on a kindred with 2 affected and 3 normal males, 3 obligate and 8 possible carriers.

Two males suffering from Pelizaeus-Merzbacher disease were examined, one at the age of 1 year 4 months and at the age of 7 years, and the other at the age of 7 years 8 months. The former had spontaneous vertical pendular nystagmus. He also showed horizontal "micronystagmus", present only at the age of 1 year, which might be similar to "voluntary nystagmus". Both males had jerky bilateral gaze-evoked nystagmus, defective smooth pursuit and optokinetic responses and a hyporeactive vestibulo-ocular reflex (VOR). All three obligate carriers exhibited typical VOR disinhibition in the two horizontal

Bilateral loss of caloric response in conscious patients (areflexia vestibularis).

Twenty-six of 11 793 consecutive vestibular examinations showed no vestibular response to bilateral irrigation with 20 degrees C water (areflexia vestibularis). CNS malignancies, infections, congenital disorders, drugs and various central nervous system disorders were found to be the most frequent aetiological factors. Associated sensorineural hearing loss--infection expected--was an uncommon finding as 17 of 26 examinations showed no auditory impairment.
