Functional gracilis flap in thenar reconstruction.

UNLABELLED: Restoration of lost opposition in the context of significant thenar soft tissue defects represents a tremendous reconstructive challenge. Free functioning muscle transfer has been described in this context and has the advantage of providing both a functioning muscle unit as well as soft tissue coverage in a single reconstructive procedure. It adds to the injured limb, and by sparing donor tendons avoids the need for re-education of motor function. We describe the use of a free innervated gracilis muscle flap for functional thenar reconstruction in two unique cases following extensi

Concomitant Acromioclavicular and Coracoclavicular Ligament Reconstruction with a Duo-Figure-8 Autogenic Graft Wrapping Technique for Treating Chronic Acromioclavicular Separation.

BACKGROUD: Coracoacromial ligament transfer is the traditional procedure for treating chronic acromioclavicular separation, but it is significantly inferior to ligament reconstruction according to biomechanical and clinical studies. However, ligament reconstruction carries the risk of complications of graft loosening and peri-tunnel fractures. Currently, there is no ligament reconstruction procedure optimal for preventing such complications. The purpose of this study was to describe and retrospectively analyze the clinical and radiological outcomes of a "duo-figure-8" autogenic graft wrapping

Editorial Commentary: The Pelvis is the Lowest Vertebral Level: Diagnostic Approach to Hip-Spine Syndrome.

The human pelvis represents a wonderful example of apparent idealistic simplicity overwhelmed by realistic complexity. Traditionally, the pelvis has been termed a "ring" linking the lower extremity to the spine via the sacroiliac joint. In essence, the pelvis is the lowest vertebral level-"the hip bone's connected to the spine bone." Thus, the law of parsimony seemingly applies in the diagnosis and management of both arthritic and nonarthritic hip and spine disorders in isolation or combination. However, an inverse Occam's razor is much more likely. The layered theory of hip disorders illustra

A 2-Stage Approach in Managing Diabetic Forefoot Ulcers.

BACKGROUND: Forefoot plantar ulcers in patients with diabetic neuropathy are considered to be primarily the result of increased shear forces applied over prominent plantar bony prominences. The purpose of this article is to describe a 2-stage treatment pathway utilizing an outpatient percutaneous tendon-Achilles lengthening (TAL) as the first stage procedure and subsequent proximal metatarsal osteotomy (MTO) as a second stage procedure for a persistent or recurrent ulcer. METHODS: A consecutive 112 patients (146 feet), who presented to our Multidisciplinary Diabetic Foot Team clinics since Fe

BACKGROUND: Pirogoff amputation is a calcaneal amputation invented by Nicolás Pirogoff that involves partial preservation of the calcaneus. CASE: A 59-year-old woman was diagnosed with left Lisfranc and Chopart joint fracture-dislocation 9 months after a fall. The patient underwent debridement together with Pirogoff amputation and surgery to place an Ilizarov external fixator. Five months later, the patient was transferred to a rehabilitation hospital. Because of inadequate bone fusion, for 3 months after the amputation the patient underwent gait training with a patellar tendon weight-bearing

Biomechanics of Upper Extremity Double Plating.

Double plating (DP) is an established treatment modality in traumatology. For certain indications, such as open reduction and internal fixation of bicondylar tibial plateau fractures and distal humeral fractures, DP is standard of care in the clinical routine. However, the principles of DP can be transferred to other anatomic regions and indications. In the past years, DP has been brought more and more into focus. For revision cases, such as the treatment of fracture non-unions, DP seems generally useful. In anatomical locations with high torsional stress and bending forces, but also in region

[Functional reconstruction of the forearm by composite free flap of fibula and soleus reinnervated. About a clinical case].

SUBJECT: We present the case of a forearm functional reconstruction of composite fibula free flap. This is a complex forearm trauma by crushing in a 24-year-old patient. The assessment of the lesions showed a bone defect of 15cm radial and ulnar diaphysis, a muscular defect of the posterior compartment of the forearm and a large skin defect next. The radial pedicle was also injured. METHODS: At 48hours, we conducted a reconstruction by free fibular flap and soleus muscle. The fibula osteosynthesys was performed between the proximal ulna and radius distally. The nerve of the soleus muscle was

The treatment of flexion contracture of the knee in myelomeningocele.

A follow-up study, on average four years after operation, of seventy-six cildren with myelomeningocele who had surgical treatment for flexion contracture of the knee, is reported. After a single operation sixty-eight knees (55 per cent) had a contracture of less than 20 degrees, which was the main criterion of success. About half of the cases of transplantation of the hamstrings to the femur, division or elongation of the hamstrings, or supracondylar osteotomy had successful results. Transfer of the hamstring tendons to the patella was successful in nine of twelve instances, so giving the lowe

Neuro-Orthopedic Surgery for Equinovarus Foot Deformity in Adults: A Narrative Review.

Neuro-orthopedic surgery is an alternative to the conservative treatment of spastic equinovarus foot (SEF) in adults. The objective of the present narrative review was to summarize current practice with regard to patient assessment, the choice of treatment, the various neuro-orthopedic procedures, and the latter's outcomes. We searched literature databases (MEDLINE, EMBASE, Cochrane) for original articles or opinion papers on surgical treatment of spastic equinovarus foot in adults. Neuro-orthopedic approaches require a careful analysis of the patient's and/or his/her caregiver needs and thus

The prevalence of the extensor digiti minimi tendon of the hand and its variants in humans: a systematic review and meta-analysis.

The extensor digiti minimi (EDM) is frequently used in the case of an abduction deformity of the little finger. It is also considered as a main resource for tendon transfer. However, it shows many variations in the human hand, which include splitting into two or more slips and sending a slip to the fourth finger, named the extensor digiti minimi et quarti (EDMQ). The aim of this systematic review is to perform an evidence synthesis on the prevalence of the EDM and its variants. Twenty-six cadaveric studies met the inclusion criteria with a total of 2247 hands. Meta-analysis results yielded an …

Ankle joint pressure changes in a pes cavovarus model after lateralizing calcaneal osteotomies.

BACKGROUND: Tendon transfers and calcaneal osteotomies are commonly used to treat symptoms related to medial ankle arthrosis in fixed pes cavovarus. However, the relative effect of these osteotomies in terms of lateralizing the ground contact point of the hindfoot and redistributing ankle joint contact stresses are unknown. MATERIALS AND METHODS: Pes cavovarus with fixed hindfoot varus was simulated in eight cadaver specimens. The effect of three types of calcaneal osteotomies on the migration of the center of force and tibiotalar peak pressure at 300 N axial static load (half-body weight) we…

Management of the acutely burned upper extremity.

In summary, thermal injury is a frequent from of upper extremity trauma that often affects young working-class males. Its management must be designed with preserving life as a priority, recognizing the additional risk factor of inhalation injury as well as the traditional predictors of mortality--age and proportion of the TBSA burned. The aim of care in dorsal hand burns is to restore wound coverage and movement within 14 days of injury. Determining the depth of thermal injury to the upper extremity is notoriously difficult and requires serial assessment over time to most accurately predict th

Osteotomies in the midfoot and the calcaneal part of the foot in relapsed clubfoot.

There are no clear guidelines on the treatment of relapsed clubfoot, which is a relatively frequent and difficult problem in paediatric orthopaedics. Numerous operative interventions are mentioned in the literature as suitable for correction of a residual deformity of the food. There are numerous soft tissue procedures (release operations, tendon extensions, tendon transfers and redressement by means of a fixateur externe) and osseous interventions (osteotomies, arthrodeses) that can be carried out in isolation or in combination. In the present article two types of osteotomy are described that…
