Noisy galvanic vestibular stimulation promotes GABA release in the substantia nigra and improves locomotion in hemiparkinsonian rats.

BACKGROUND: The vestibular system is connected to spinal, cerebellar and cerebral motor control structures and can be selectively activated with external electrodes. The resulting sensation of disturbed balance can be avoided by using stochastic stimulation patterns. Adding noise to the nervous system sometimes improves function. Small clinical trials suggest that stochastic vestibular stimulation (SVS) may improve symptoms in Parkinson's disease. We have investigated this claim and possible mechanisms using the 6-hydroxydopamine (6-OHDA) hemilesion model of Parkinson's disease. METHODOLOGY/P

Virtual Neuromuscular Training: Preliminary Findings Using an Injury Prevention Program for Physically Active Adults.

CONTEXT: Neuromuscular training (NMT) is an effective injury prevention strategy but may have barriers to in-person delivery. Our objective was to examine injury incidence in the year following NMT intervention delivered virtually and to identify functional test performance associated with subsequent injuries. DESIGN: Secondary analysis of randomized controlled trial. METHODS: We recruited 40 physically active adults aged 18-40 years. Functional testing included simple (stimulus-response) and clinical (drop stick) reaction time (RT), single- and dual-task tandem gait, vestibular/oculomotor f

Transient evoked otoacoustic emissions in vestibular neuritis.

OBJECTIVES: Partial or total degeneration of the vestibulocochlear anastomosis at its takeoff from the saccular ganglion and regenerating efferent neural buds under the cochlear outer hair cells (OHCs) have been found in the temporal bones of human patients with a history of vestibular neuritis (VN). We sought to test whether VN has any functional impact on the ipsilateral OHCs by means of transient evoked otoacoustic emission (TEOAE) testing. METHODS: We retrospectively analyzed prospectively collected TEOAE data of 28 patients (19 female, 9 male; age range, 21 to 60 years; median age, 42.5

Automatic inverse treatment planning of Gamma Knife radiosurgery via deep reinforcement learning.

PURPOSE: Several inverse planning algorithms have been developed for Gamma Knife (GK) radiosurgery to determine a large number of plan parameters by solving an optimization problem, which typically consists of multiple objectives. The priorities among these objectives need to be repetitively adjusted to achieve a clinically good plan for each patient. This study aimed to achieve automatic and intelligent priority tuning by developing a deep reinforcement learning (DRL)-based method to model the tuning behaviors of human planners. METHODS: We built a priority-tuning policy network using deep c

Visual information potentiates incremental VOR adaptation.

INTRODUCTION: Vestibulo-ocular reflex (VOR) response to a given head rotation can be modified through adaptation by inducing a visuo-vestibular mismatch. Incremental velocity error protocols allow for rapid unilateral VOR adaptation in the everyday-life range of head movement high velocities. However, these protocols are conducted in darkness, with the sole visual stimulation of the central retina being provided by the fixated target. This study aimed to investigate whether enhanced visual information can improve incremental VOR adaptation, starting with an assessment of the feasibility to imp

Observation of positional nystagmus with infrared CCD camera.

Using infrared CCD camera and electronystagmography we analyzed positional nystagmus in 33 patients with peripheral positional vertigo. Patients were classified into three types; (a) lateral type: nystagmus occurs in lateral position, (b) sagittal type: nystagmus occurs in head-hanging position (so-called benign paroxysmal positional vertigo, BPPV), and (c) mixed type. Nystagmus in the lateral type was horizontal and that in the sagittal type was rotatory. In all types, the vertigo and nystagmus began with a few seconds' latency after head tilt and briefly increased to a maximum, and then grad

Statistical Considerations for Subjective Visual Vertical and Subjective Visual Horizontal Assessment in Normal Subjects.

OBJECTIVES: Judgments of the subjective visual vertical (SVV) and subjective visual horizontal (SVH) while seated upright are commonly included in standard clinical test batteries for vestibular function. We examined SVV and SVH data from retrospective control to assess their statistical distributions and normative values for magnitudes of the preset effect, sex differences, and fixed-head versus head-free device platforms for assessment. METHODS: Retrospective clinical SVV and SVH data from 2 test platforms, Neuro-otologic Test Center (NOTC) and the Neurolign Dx 100 (I-Portal Portable Assess

[Clearance rate of gingiva and content of collagen of inflammated and clinically non-inflammated papilla interdentalis].

UNLABELLED: The gingiva vessel function has been defined by gingivitis (A), clinical sound gingiva without (B) and with loss in attachment (C) as well as the collagen fraction by biopsy material. The division of the probands at the age between 20 and 24 years into groups has been carried out after checking the SBI, PII (Silness/Loe) and the loss of attachment of the teeth 43-33. The vessel function has been found out through the impulse rate of the gingiva Xenon-133-clearance by calculating the clearance rates (CR) (groups: A1 n = 15; B1 n = 18; C1 n = 16). The vestibular interdental papillae

[Diagnosis and Management of Vestibular Schwannomas - An Interdisciplinary Challenge].

Vestibular schwannomas expand slowly in the internal auditory canal, in the cerebellopontine angle, inside the cochlear and the labyrinth. Larger tumors can displace and compress the brainstem. With an annual incidence of 1:100 000 vestibular schwannoma represent 6-7% of all intracranial tumors. In the cerebellopontine angle they are by far the most neoplasm with 90% of all lesions located in this region. Magnetic resonance imaging (MRI), audiometry, and vestibular diagnostics are the mainstays of the clinical workup for patients harboring tumors. The first part of this paper delivers an overv

Spontaneous inversion of nystagmus without a positional change in the horizontal canal variant of benign paroxysmal positional vertigo.

OBJECTIVE: We investigated the neuro-otological findings, including nystagmus, and the clinical course of patients with the horizontal canal variant of benign paroxysmal positional vertigo (HC-BPPV), who showed spontaneous inversion of nystagmus without a positional change. Furthermore, we speculated on the possible mechanism of spontaneous inversion of nystagmus without a positional change. PATIENTS AND METHODS: The characteristics of spontaneous inversion of positional nystagmus without a positional change were analyzed in 7 patients with HC-BPPV. RESULTS: All patients were diagnosed as ha

Changes in utricular function during artificial endolymph injections in guinea pigs.

Various theories suggest endolymphatic hydrops may cause a rupture of the membranous labyrinth or may force open the utriculo-saccular duct, resulting in a sudden change in inner ear function. Here, we have used slow injections of artificial endolymph into either scala media or the utricle of anaesthetised guinea pigs to investigate the effects of hydrops. Vestibular function was continuously monitored in addition to the measurements of cochlear function developed in our laboratory (Brown et al. Hear Res, 2013). Scala media injection induced consistent functional changes, which occurred in two

"Minimized rotational vestibular testing" as a screening procedure detecting vestibular areflexy in deaf children: screening cochlear implant candidates for Usher syndrome type I.

Morbus Usher (USH), a combination of sensorineural hearing loss and retinal visual impairment, is classified into group I-III. USH I patients are born deaf. Within the first 10 years of life, they develop a severe vision impairment due to progressive retinal dystrophy (retinitis pigmentosa). USH I patients show vestibular deficits. The incidence of USH I among congenitally deaf children is assumed to be as high as 10%. We intend to create a simple examination procedure for screening congenitally deaf children for vestibular deficiency and subsequently USH I. The examination procedure is named

The effect of alcohol ingestion on ocular counterrolling.

Nineteen normal subjects underwent two naso-occipital rotations to 90 degrees right and left ear down in our standard ocular counterrolling (OCR) protocol. Both eyes were videotaped. Following two rotations, subjects drank 90 ml vodka in 180 ml orange juice; and in about 20 min, when blood alcohol levels reached 0.04-0.09%, the protocol was repeated. An SMI videooculography system provided measurements. Results showed that amplitude of OCR was significantly decreased after alcohol ingestion; smoothness was significantly increased after alcohol, similar to alcohol's effect on essential tremor.

Predicting neuroradiologic outcome in patients referred for audiovestibular dysfunction.

PURPOSE: To relate clinical presentation and results of audiovestibular testing to neuroradiologic outcome in patients with audiovestibular dysfunction. METHODS: We retrospectively reviewed the neuroimaging studies, results of audiometric and vestibular testing, and medical records of 118 patients referred for imaging over a 2-year period for evaluation of sensorineural hearing loss, dizziness, and/or vertigo, and to rule out acoustic neuroma. Patients' presentation and results of audiometric and vestibular testing were associated with either a positive or negative neuroimaging outcome. Discr

Magnetic resonance imaging: a cost-effective first line investigation in the detection of vestibular schwannomas.

Magnetic resonance imaging (MRI) is currently the 'gold-standard' investigation in patients with a unilateral sensorineural hearing loss. The procedure, however, is expensive and of limited availability. Instead, such patients often undergo a series of audiovestibular tests and computed tomography in an attempt to exclude or diagnose a vestibular schwannoma. We describe seven cases of unilateral vestibular schwannoma in which conventional assessment was either equivocal or failed to demonstrate a tumour subsequently diagnosed by magnetic resonance imaging. Two patients with neurofibromatosis t
