ETHNOPHARMACOLOGICAL RELEVANCE: Pulsatilla chinensis (Bunge) Regel is a traditional Chinese herbal medicine used to treat intestinal amebiasis, malaria, vaginal trichomoniasis, and bacterial infections. Anemoside B4 (AB4), a pentacyclic triterpenoid saponin, is one of the primary bioactive substances in Pulsatilla chinensis (Bunge) Regel, and gavage administration of AB4 to animals has been demonstrated to exhibit anticancer, anti-inflammatory, and antiviral actions. However, AB4 exposure in plasma is very low after oral administration, and the biotransformation of AB4 in vivo after oral admin…

Amebiasis is an endemic disease and a public health problem throughout Mexico, although the incidence rates of amebic liver abscess (ALA) vary among the geographic regions of the country. Notably, incidence rates are high in the northwestern states (especially Sonora with a rate of 12.57/100,000 inhabitants) compared with the central region (Mexico City with a rate of 0.69/100,000 inhabitants). These data may be related to host genetic factors that are partially responsible for resistance or susceptibility. Therefore, we studied the association of the HLA-DRB1 and HLA-DQB1 alleles with resista…

The pathomechanisms of Acanthamoeba spp. infection are still poorly understood. The aim of the study was to determine the expression of the NLR family pyrin domain-containing protein 3 (NLRP3), prostaglandin-endoperoxide synthase 2 (PTGS2, commonly known as cyclooxygenase-2, COX-2), and various cytokines in the kidneys of immunocompetent and immunosuppressed mice infected with Acanthamoeba sp. (T16 genotype). The proteins were analyzed by quantitative reverse transcription PCR. In immunocompetent mice, we observed increased mRNA levels of interferon gamma (IFNγ), tumor necrosis factor alpha (T

Balamuthia mandrillaris is a causative agent of granulomatous amebic encephalitis, a rare but often fatal condition. To investigate the role of T helper (Th) cell subsets in the immune response against B. mandrillaris, we examined 3 mouse strains with distinct immunological profiles: C57BL/6 (Th1-dominant), BALB/c (Th2-dominant), and ICR (balanced Th1/Th2). Mice were infected intranasally with 1×105 amoebae. Body weight and neurologic symtoms were monitored weekly, and cytokine expression was assessed biweekly over 6 weeks. Minimal weight loss and no mortality were observed in C57BL/6 mice, wh

Cutaneous amoebiasis is a severe opportunistic infection, particularly prevalent among immunocompromised individuals, with high mortality due to the insidiousness of the disease, its ability to evade early diagnosis, and lack of specific symptoms. Herein, we aimed to evaluate the therapeutic potential of the marine alga, Padina pavonica, extract as a topical treatment for Acanthamoeba polyphaga skin infections. Immunocompromised mice with induced cutaneous lesions were topical infected with A. polyphaga and examined 3, 9, and 15 days post infection through histological and molecular analysis.

BACKGROUND: Acinetobacter baumannii is ubiquitous, facultative intracellular, and opportunistic bacterial pathogen. Its unique abilities allow it to survive in a diverse range of environments, including health care settings, leading to nosocomial infections. And its exceptional ability to develop resistance to multiple antibiotics leaves few drug options for treatment. It has been recognized as a leading cause of nosocomial pneumonia and bacteremia over the world.
CASE PRESENTATION: In this case, a 73-year-old woman presented with a Neer Group VI proximal humeral fracture. Six hours after a s

Balamuthia mandrillaris is a recently described ameba known to cause a subacute to chronic central nervous system infection called granulomatous amebic encephalitis. Evidence suggests that apparently immunocompetent persons are at risk for disease and show a similar nonspecific presentation to that of immunodeficient persons. However, evidence of hematogenous dissemination, which has been found in immunodeficient patients, has been lacking in immunocompetent patients. Here, we describe a previously healthy patient with B mandrillaris-associated granulomatous amebic encephalitis in whom both ce

Soil and freshwater-dwelling amoebae may opportunistically infect the skin and evoke a granulomatous dermatitis that camouflages their underlying morphology. Amoebic infestations are incredibly rare in the U.S., predominantly occurring in the young, elderly, and immunocompromised. Sadly, because diagnosis is difficult and unsuspected, most cases are diagnosed at autopsy. The following case is of a healthy 84-year-old man with a non-healing nodulo-ulcerative cutaneous lesion on his left forearm that appeared following a gardening injury. Lesional punch biopsies repeatedly showed non-specific gr

OBJECTIVES: Free-living amoebas are exceedingly rare causes of cutaneous infections and present unique diagnostic and therapeutic challenges. We describe a case of disseminated acanthamoebiasis with cutaneous manifestations and summarize additional diagnostic, prognostic, and therapeutic highlights.
METHODS: A 58-year-old man with relapsed chronic lymphocytic leukemia had several weeks of progressive, painful ulcerations on the forehead, arms, abdomen, and thighs. A biopsy was performed for histopathologic evaluation.
RESULTS: The biopsy specimen showed inflammatory infiltrate with abscess f

Cervical amoebiasis is an extremely rare diagnosis with only a small number of published case reports. This disease may present as cervical growth mimicking cervical carcinoma. Owing to the similarity of the clinical presentation of bleeding per vagina and per speculum examination showing growth or ulcers, definitive diagnosis is made on microscopic examination only. We present a rare case of cervical amoebiasis in a 28-year-old, multiparous female who presented with a history of vaginal bleeding. The patient was treated with metronidazole and diloxanide furate, after which she recovered. Awar

We report two cases of fatal chronic meningoencephalitis caused by Balamuthia mandrillaris in immunocompetent men. Diagnosis of amoebic meningoencephalitis was made ante-mortem in one case and postmortem in another by histopathological examination and confirmed by demonstration of B. mandrillaris deoxyribonucleic acid (DNA) by polymerase chain reaction (PCR).

OBJECTIVE: To our knowledge, there is no study dealing with the prevalence of free-living amoebas (FLA) in water sources in Turkey, previous studies were mostly case presentations. The aim of the present study was to investigate the prevalence of FLA from tap water and natural water sources in different parts of the city.
METHODS: In the study, 250 samples were collected from the city centre, districts and villages. Two litres of water was collected from each source and filtered through a vacuum filtration system. The filter papers were washed in "Page's Amoeba Saline (PAS)" solution and incu

A case of granulomatous amoebic meningoencephalitis in a previously healthy, mature, apparently immunocompetent dog, with a history of exposure to stagnant water, is reported. The case presented with ataxia and a tendency to fall to the left side. A computed tomography (CT) showed a ring-enhancing lesion within the cerebellum; an examination of cerebrospinal fluid (CSF) revealed nonspecific mixed-cell pleocytosis. Despite antibiotic and anti-inflammatory therapy, clinical signs progressed rapidly to decerebellate rigidity over 4 days, and the dog was euthanased. Significant post-mortem finding

BACKGROUND: Naegleria fowleri primary amebic meningoencephalitis (PAM) has a very high mortality rate, probably exceeding 95%. A few people have survived after getting intravenous and intrathecal amphotericin, variably coupled with other agents that include dexamethasone, diflucan, chloramphenicol and rifampin, but even with prompt initiation of therapy, it is still a very uphill battle.
PRESENTATION OF THE HYPOTHESIS: Survival could be improved by combined intrathecal, intranasal and intravenous amphotericin, diflucan and rifampin, with aduvant intravenous chloramphenicol, muramyl dipeptide,

We report a case of an amebic brain abscess in a 2-year-old girl, with symptoms mimicking bacterial meningitis with no evidence of disease elsewhere. Histological evaluation of the abscess revealed the organisms, and the abscess regressed in response to specific medical treatment. This article reviews the rarity of these abscesses and difficulty in the diagnosis.

PURPOSE OF REVIEW: Infectious keratitis is a medical emergency. Improper management can lead to marked loss of vision. This review identifies recent trends in the study of infectious keratitis.
RECENT FINDINGS: A multicountry outbreak of Fusarium keratitis emphasizes that contact lens wear is a major risk factor for infectious keratitis. Acanthamoeba and fungal keratitis are the most expensive forms of infectious keratitis to treat. Noninvasive methods and molecular techniques have improved diagnosis of infectious keratitis. Fortified topical antibiotics and fluoroquinolones are still the mai

Pulmonary amoebiasis without liver involvement occurs sporadically as a result of haematogenous spread from a primary site, the colon. The case history is presented of a patient who developed superior vena cava syndrome due to a pulmonary amoebic abscess without liver involvement. He was initially suspected of having a neoplasm but a combination of tests including histological examination of the H&E stained excised tissue, immunofluorescence using anti-Entamoeba histolytica antibodies, and serology confirmed the diagnosis of amoebiasis. To our knowledge this is the first description of pulmona

We studied the cyst production of 340 asymptomatic carriers with single or mixed infections of Entamoeba histolytica/Entamoeba dispar, Endolimax nana and Entamoeba coli. It was found that the main source of transmission was a small group of 37 (10.9%) carriers who produced 70.2% of all cysts. Gender was an important factor for cyst production of Ent. histolytica/Ent. dispar, since males produced 6-fold more cysts than females. Females produced 'large' (13-15 microns) Ent. histolytica/Ent. dispar cysts and males small (10-12 microns) cysts when concentration was > 2000 cysts/g. When concentrati

The protozoan parasite Entamoeba histolytica causes intestinal inflammation and ulceration. Amoebic trophozoites activate the transcription factor NF-kappa B in human intestinal epithelial cells, initiating an inflammatory response programme with resultant damage to the intestinal tissue. Amoebic cysteine proteinases have been proposed as important virulence factors for amoebiasis. To test the role of amoebic cysteine proteinases in the pathogenesis of amoebic colitis, human intestinal xenografts in SCID mice were infected with E. histolytica trophozoites expressing an antisense message to ehc
