Prediction of mitral regurgitation resolution after coronary bypass graft surgery and cardiac resynchronization therapy by late gadolinium enhancement magnetic resonance imaging: a case report.

BACKGROUND: Appropriate surgical management of moderate functional mitral regurgitation (MR) at the time of coronary artery bypass graft (CABG) surgery remains controversial. A recent study demonstrated no survival benefit from adding mitral repair to CABG for patients with moderate functional MR. Preoperative prediction of reverse remodelling is crucial in making the decision to add mitral valve repair to CABG. Late gadolinium-enhanced magnetic resonance imaging (LGE MRI) offers a reference method to assess myocardial viability. CASE SUMMARY: A 60-year-old man with ischaemic cardiomyopathy w

The Ferroptosis-Related Mechanisms of Pentoxifylline in Atrial Fibrillation Treatment.

BACKGROUND: Ferroptosis has been increasingly implicated in the pathophysiology of atrial fibrillation (AF). Pentoxifylline (PTX), a methylxanthine derivative, has shown potential therapeutic benefits in cardiovascular diseases; however, its unique role in ferroptosis-associated AF remains unclear. This study aimed to elucidate the molecular mechanisms through which PTX may exert therapeutic effects on ferroptosis-related AF, using a multifaceted approach integrating network pharmacology, bioinformatics, and experimental validation. METHODS: Two transcriptomic datasets, GSE41177 and GSE79768,

Dysfunction of Myosin Light-Chain 4 (MYL4) Leads to Heritable Atrial Cardiomyopathy With Electrical, Contractile, and Structural Components: Evidence From Genetically-Engineered Rats.

BACKGROUND: There is increasing interest in the concept of atrial cardiomyopathy, but the underlying molecular and mechanistic determinants remain poorly defined. We identified a family with heritable atrial cardiomyopathy manifesting as progressive atrial-selective electromechanical dysfunction, tachyarrhythmias, and bradyarrhythmias requiring pacemaker implantation. Myosin light-chain 4 (MYL4), encoding the atrial-selective essential myosin light chain, was identified as a candidate gene. We used genetically modified rat models to investigate the role of MYL4 in atrial cardiomyopathy. METHO

HSP75 protects against cardiac hypertrophy and fibrosis.

Cardiac hypertrophy, a major determinant of heart failure, is associated with heat shock proteins (HSPs). HSP75 has been reported to protect against environmental stresses; however, its roles in cardiac hypertrophy remain unclear. Here, we generated cardiac-specific inducible HSP75 transgenic mice (TG) and cardiac hypertrophy was developed at 4 weeks after aortic banding in TG mice and wild-type littermates. The results revealed that overexpression of HSP75 prevented cardiac hypertrophy and fibrosis as assessed by heart weight/body weight ratio, heart weight/tibia length ratio, echocardiograph

Atrial fibrillation is linked to increased left ventricular fibrosis and inflammation measured by CMR with prognostic implications.

Approximately 30% of patients with atrial fibrillation (AF) develop heart failure (HF), but the underlying mechanisms and their impact on the left ventricle (LV) remain unclear. Fibrosis and inflammation are suspected contributors. This study aimed to investigate LV tissue characteristics in patients with and without AF using cardiac magnetic resonance imaging (CMR). Patients from a single-center CMR registry were categorized by AF status, adjudicated by two blinded investigators. LV fibrosis was assessed by native T1 and extracellular volume (ECV), while T2 indicated inflammation. Results wer

Association of Left Ventricular Hypertrophy and Atrial Fibrillation with Hemorrhagic Evolution of Small Vessel Disease.

OBJECTIVES: Cerebral small vessel disease (SVD) is often associated with hypertension and may evolve towards intracerebral hemorrhage (ICH) or lacunar ischemic stroke. However, the factors favoring the evolution towards ICH or lacunar stroke are not well understood. MATERIALS AND METHODS: This retrospective study included 326 consecutive patients (71.1±13.2 years, 38% women): 143 with deep ICH and 183 with lacunar lesions (LL) <2 cm, which were visible in a deep location on brain CT scan. Among LL patients, 143 had a small-artery occlusion (SAO) stroke according to the TOAST classification. C

Feedback mechanisms for cardiac-specific microRNAs and cAMP signaling in electrical remodeling.

BACKGROUND: Loss of transient outward K(+) current (Ito) is well documented in cardiac hypertrophy and failure both in animal models and in humans. Electrical remodeling contributes to prolonged action potential duration and increased incidence of arrhythmias. Furthermore, there is a growing body of evidence linking microRNA (miR) dysregulation to the progression of both conditions. In this study, we examined the mechanistic basis underlying miR dysregulation in electrical remodeling and revealed a novel interaction with the adrenergic signaling pathway. METHODS AND RESULTS: We first used a t

The impact of sacubitril/valsartan on outcome in patients suffering from heart failure with a concomitant diabetes mellitus.

BACKGROUND: Guidelines classify sacubitril/valsartan as a significant part of medical treatment of heart failure with reduced ejection fraction (HFrEF). Data have shown that the HbA1c levels in patients with diabetes mellitus could be impacted by sacubitril/valsartan. A possible positive effect in diabetes patients treated with sacubitril/valsartan on outcome and echocardiography parameters is not well studied yet. AIMS: The aim of the present study was to compare the impact of sacubitril/valsartan on life-threatening arrhythmias, atrial fibrillation, different echocardiography parameters and

Understanding subclinical heart disease and cytokine profile in cardiorheumatic care of systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) affects several organs and systems. The involvement of the cardiovascular system confers high morbidity and mortality in patients, so the detection of possible subclinical or advanced cardiac involvement at the time of diagnosis will lead to preventive solutions according to the degree and type of cardiovascular involvement. New echocardiographic techniques such as left ventricular global longitudinal strain (LV-GLS) allow the detection systolic dysfunction of the LV before the onset of symptoms secondary to heart failure. The objective of the study was to co

Multi-omics Mendelian Randomization Prioritizes Neutrophil Extracellular Trap-related Genes Associated with Atrial Fibrillation Risk.

INTRODUCTION: Neutrophil extracellular traps (NETs) participate in thrombosis, inflammation, and cardiovascular remodeling, yet whether NET-related genes (NRGs) are associated with atrial fibrillation (AF) risk across multiple molecular layers remains unclear. This study used a multiomics Mendelian randomization framework to prioritize NRGs supported by methylation, expression, and protein quantitative trait loci (QTL) data. METHODS: Genome-wide significant cis instruments (P < 5 × 10-8) were obtained for 90 methylation QTLs (mQTLs), 100 expression QTLs (eQTLs), and 38 protein QTLs (pQTLs) ma

Electrocardiographic markers of right heart strain and their association with arrhythmias in patients with COPD.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is frequently associated with cardiovascular complications, including right heart dysfunction and cardiac arrhythmias. Chronic hypoxia and pulmonary vascular remodeling may lead to structural and electrical cardiac changes. Electrocardiography (ECG) may provide simple markers of right heart strain that could help identify COPD patients at increased arrhythmic risk. OBJECTIVE: To evaluate the association between ECG markers of right heart strain and arrhythmias and to identify independent predictors using multivariable analysis. METHODS

Left Heart Abnormalities in Patients With Lung Disease, OSA, and Chronic Thromboemboli at Risk for or With Known Pulmonary Hypertension.

BACKGROUND: Patients with lung disease, sleep apnea, and chronic thromboemboli can develop pulmonary hypertension, currently classified as group 3 or 4. Many of these patients also have risk factors for heart failure with preserved ejection fraction (HFpEF), but the optimal approach to identify the disease overlap remains unclear. METHODS: Pretest probability for HFpEF was determined using the HFpEF-ABA (age, body mass index, atrial fibrillation) algorithm among adjudicated group 3 or 4 patients at risk for pulmonary hypertension in the PVDOMICS study (Redefining Pulmonary Hypertension Throug

Neurohumoral Dysregulation in Acute Myocardial Infarction With Chronic Kidney Disease: Implications for Prognosis and Management.

Background Acute myocardial infarction (AMI) and chronic kidney disease (CKD) often coexist, and both are associated with neurohumoral imbalances that may worsen cardiovascular outcomes. Reduced kidney function can contribute to arterial stiffness and activation of the renin-angiotensin-aldosterone system (RAAS), which promotes vasoconstriction, sodium and water retention, myocardial remodeling, and fibrosis, mechanisms that exacerbate both cardiac and renal dysfunction. Endothelial injury is also common in these patients, and von Willebrand factor (vWF) serves as a recognized marker of endoth

Dual-chamber ICD for left bundle branch area pacing: the cardiac resynchronization and arrhythmia sensing via the left bundle (cross-left) pilot study.

BACKGROUND: Left bundle branch area pacing (LBBAP) has emerged as a promising technique to deliver cardiac resynchronization therapy (CRT). However, safety and efficacy of ventricular arrhythmia sensing via the left bundle in implantable cardioverter-defibrillator (ICD) recipients remain unclear. We sought to evaluate the feasibility of a single LBBAP lead connected to a dual-chamber ICD in patients indicated with a CRT-D implantation. METHODS: The CROSS-LEFT pilot study prospectively included 10 consecutive patients with a reduced ejection fraction and a complete left bundle branch block, in

Spontaneous tricuspid valve chordal rupture in a dog with severe, irreversible pulmonary hypertension caused by Angiostrongylus vasorum infection.

BACKGROUND: The adult worms of Angiostrongylus vasorum reside in the pulmonary artery of dogs and can lead to cardiac, respiratory, and central neurologic signs. Due to luminal obstruction and perivascular inflammation of the pulmonary artery branches, pulmonary hypertension can arise. Pulmonary hypertension, in turn, can lead to severe damage of the right-sided cardiac structures, leading to right ventricular remodeling and tricuspid valve regurgitation. CASE PRESENTATION: An 8-year-old neutered female English Cocker Spaniel was presented to the author's institution because of abdominal dist

Influence of apocynin on cardiac remodeling in rats with streptozotocin-induced diabetes mellitus.

BACKGROUND: Increased reactive oxygen species (ROS) generation in diabetes mellitus (DM) is an important mechanism leading to diabetic cardiomyopathy. Apocynin, a drug isolated from the herb Picrorhiza kurroa, is considered an antioxidant agent by inhibiting NADPH oxidase activity and improving ROS scavenging. This study analyzed the influence of apocynin on cardiac remodeling in diabetic rats. METHODS: Six-month-old male Wistar rats were assigned into 4 groups: control (CTL, n = 15), control + apocynin (CTL + APO, n = 20), diabetes (DM, n = 20), and diabetes + apocynin (DM + APO, n = 20).
