The delivery modes and morbidity/mortality in very preterm infants with birth weight <1,500 g: a retrospective cohort study.

INTRODUCTION: Compare outcomes of very preterm infants (VPIs) with a birth weight<1,500 g based on delivery mode and propensity score matching (PSM). METHODS: This was a retrospective study (2016-2021) of 1,375 VPIs (692 vaginal and 683 cesarean deliveries). PSM created 390 matched pairs. Outcomes included respiratory and neurological morbidity and mortality. RESULTS: PSM revealed no significant difference between the two groups at baseline or after antenatal corticosteroids administration. The incidence rates of birth asphyxia, delivery room resuscitation, neonatal respiratory distress synd

Neonatal Brucellosis in an Extremely Preterm Infant: A Fatal Case Report.

BACKGROUND: Brucellosis is a globally significant zoonotic disease, but congenital brucellosis remains exceedingly rare. It is primarily transmitted transplacentally from an infected mother to the fetus, often presenting with nonspecific signs that mimic neonatal sepsis. The condition poses diagnostic and therapeutic challenges, particularly in endemic regions where early identification is crucial to improving neonatal outcomes. CASE PRESENTATION: We report the case of a 19-year-old gravida 1, para 0 female with a history of treated Brucella infection at 12 weeks of gestation. She experienced

Cervical Pessary Versus Vaginal Progesterone to Prevent Preterm Birth in Women with a Singleton Pregnancy and a Short Cervix: A Systematic Review and Meta-Analysis.

Vaginal progesterone (VP) is the recommended intervention for preventing preterm birth (PTB) in women with short cervix and singleton pregnancy. However, cervical pessary (CP) has been proposed as a simpler alternative. This study compares the efficacy and safety of both interventions. A systematic review and meta-analysis included randomized controlled trials (RCTs) from PubMed, CENTRAL, Scopus, Google Scholar, and Web of Science (up to November 2024). Based on heterogeneity, fixed- or random-effects models were used to analyze dichotomous outcomes as risk ratios (RR) and continuous outcomes

Breast milk pasteurization for preventing cytomegalovirus infection: A preliminary study in a Japanese perinatal center.

BACKGROUND: Postnatal cytomegalovirus (CMV) infection, primarily transmitted through breast milk, poses significant risks for very preterm infants (VPIs), potentially leading to sepsis-like syndromes, necrotizing enterocolitis, and pneumonia. Although pasteurization of breast milk has been shown to be effective in preventing postnatal CMV infection, no studies in Japan have examined its clinical efficacy in practice. This preliminary study investigated the incidence of postnatal CMV infection in VPIs born to CMV-seropositive mothers who received pasteurized breast milk at our Japanese perinata

Association between two different lipid injectable emulsions and parenteral nutrition-associated cholestasis in very low birth weight infants: A retrospective cohort study.

BACKGROUND: Using soybean oil-based lipid emulsions (Intralipid), which contain higher amounts of ω-6 fatty acids and phytosterols in parenteral nutrition, is a risk factor for cholestasis (parenteral nutrition-associated cholestasis [PNAC]). An alternative form of a mixed lipid emulsion (SMOFlipid) has been developed to reduce the risk of PNAC, but significant benefits over Intralipid in very low birth weight (VLBW) infants have yet to be demonstrated. The aim of this study was to compare the differences in PNAC incidence in VLBW infants receiving SMOFlipid vs Intralipid. METHODS: The study

Multiplex Determination of K-Antigen and Colanic Acid Capsule Variants of Cronobacter sakazakii.

Cronobacter sakazakii is associated with the ingestion of contaminated reconstituted powdered infant formula (PIF), resulting in necrotizing enterocolitis, sepsis and meningitis in neonatal infants. Potential virulence determinants include the variable capsular polysaccharides; K-antigen and colanic acid (CA). Strains encoding for the capsule variant K2:CA2 have been strongly associated with neonatal meningitis cases. This study aimed to develop and apply a multiplex PCR assay to determine C. sakazakii K-antigen and colanic acid types. Twenty-six strains of C. sakazakii which had previously be

The proteomic fingerprint in infants with single ventricle heart disease in the interstage period: evidence of chronic inflammation and widespread activation of biological networks.

INTRODUCTION: Children with single ventricle heart disease (SVHD) experience significant morbidity across systems and time, with 70% of patients experiencing acute kidney injury, 33% neurodevelopmental impairment, 14% growth failure, and 5.5% of patients suffering necrotizing enterocolitis. Proteomics is a method to identify new biomarkers and mechanisms of injury in complex physiologic states. METHODS: Infants with SVHD in the interstage period were compared to similar-age healthy controls. Serum samples were collected, stored at -80°C, and run on a panel of 1,500 proteins in single batch an

Location of Treatment Among Infants Requiring Complex Surgical Care.

INTRODUCTION: Rural children have worse health outcomes compared to urban children. One mechanism for this finding may be decreased access to specialized care at children's hospitals. The objective of this study was to evaluate the hospital types where complex surgical care in infants is performed nationally. METHODS: This study examined infants (<1 y old) in the Kids' Inpatient Database from 2009 to 2019 who underwent surgery for one of the following conditions: esophageal atresia, gastroschisis, omphalocele, Hirschsprung disease, anorectal malformation, pyloric stenosis, small bowel atresia
