Intraductal papillary neoplasm of the bile duct (IPNB) is an uncommon biliary tumor with recognized malignant potential and a broad clinicopathological spectrum. Its diagnosis remains challenging, particularly in extrahepatic disease, due to limitations of cross-sectional imaging. We report a consecutive series of five patients diagnosed with IPNB in a tertiary hepatobiliary center, highlighting their clinical, radiologic, endoscopic, and pathological features. Most patients presented with obstructive jaundice or cholangitis, and lesions were predominantly located in the distal bile duct. Cros

Intraoperative frozen-section (FS) margin assessment is essential for guiding precise resection in perihilar cholangiocarcinoma (pCCA). However, freezing artifacts, superficial sampling, drainage-related reactive epithelial changes, and subtle cytologic atypia in well-differentiated adenocarcinoma can make intraoperative interpretation difficult. We report a 70-year-old woman with Bismuth-Corlette type II pCCA who underwent radical extrahepatic bile duct resection after PTBD-mediated biliary decompression. Intraoperative FS of the distal bile duct margin showed atypical glands of undetermined

INTRODUCTION: Pancreatic ductal adenocarcinoma (PDAC) is one of the most aggressive malignancies, typically associated with late-stage diagnosis and limited survival despite advances in systemic therapy. Long-term survival in metastatic PDAC is exceedingly rare, and treatment strategies are limited by the intrinsic resistance of the tumor to standard therapies. However, emerging evidence suggests that molecular subtypes may exhibit distinct biological behaviors and may be amenable to new therapeutic options.
CASE PRESENTATION: We report the remarkable long-term survival of a 43-year-old male

Ultrasonography is often used to diagnose biliary diseases in dogs; however, it is difficult to delineate the entire bile and pancreatic ducts. Various imaging techniques for bile and pancreatic ducts have been attempted to overcome this problem. Magnetic resonance cholangiopancreatography (MRCP) is often used to evaluate the bile and pancreatic ducts in humans with obstructive jaundice, but very few reports exist on its usage in dogs. This study was designed as a prospective observational study to assess the feasibility and effectiveness of MRCP for visualizing the bile and pancreatic ducts i

BACKGROUND: Malignant lesions of the pancreatic head can cause obstructive jaundice requiring biliary drainage. However, the effect of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis and metal stents on surgical resection remains controversial. This study aimed to investigate the efficacy of primary endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) performed prior to pancreatoduodenectomy, excluding patients with biliary duct cancer.
METHODS: We retrospectively analyzed consecutive patients who underwent pancreatoduodenectomy at our institution between January

Pancreatic hydatid disease is an exceptionally rare manifestation of Echinococcus granulosus infection. Its clinical and radiological resemblance to other cystic pancreatic lesions often delays diagnosis and treatment. Here, we describe a case of a 71-year-old woman from Lebanon, who presented with progressive epigastric and right upper quadrant pain. Initial endoscopic examination revealed a benign gastric ulcer. Six months later, she developed obstructive jaundice and cholangitis. Examination showed scleral icterus and mild right upper quadrant tenderness. Laboratory tests revealed leukocyto

Iatrogenic trauma and perforation are among the most concerning complications of endoscopic retrograde cholangiopancreatography (ERCP). A 76-year-old man presented for management of obstructive jaundice caused by pancreatic cancer. The ERCP was planned for further evaluation of pancreatic cancer and endoscopic biliary drainage. The ERCP scope could not pass because of resistance during the initial attempt to insert it through the pyriform sinus. After two attempts, mild bleeding occurred in the oral cavity, and the ERCP scope was successfully inserted in the esophagus. Tissue debris was observ

Can combined spinal and epidural anesthesia be the gold standard for laparoscopic surgery for pregnant patients? This case report presents a first trimester pregnant patient who was admitted for obstructive jaundice syndrome (pain in the right hypochondrium, nausea, and vomiting). Initially, because of the risk/benefit ratio of pregnancy, the treatment was medical and the patient was immediately discharged because her clinical condition improved, but she was rapidly readmitted to the surgery department because of worsening symptoms. Emergency surgical intervention (laparoscopic cholecystectomy

BACKGROUND: Surgical clip migration is a rare complication that can happen many years following a successful cholecystectomy. It has a similar clinical presentation as choledocholithiasis. The diagnosis is usually made using imaging modalities such as ultrasounds, computed tomography (CT) scans, or magnetic resonance cholangiopancreatography (MRCP). If left untreated, complications such as acute cholangitis, acute pancreatitis, clip embolism, duodenal ulcer, and obstructive jaundice can develop. Endoscopic retrograde cholangiopancreatography (ERCP) is the therapeutic modality of choice for ext

Background and study aims Endoscopic nasobiliary drainage is accompanied by a risk of accidental removal of the nasobiliary drainage (NBD) tube, especially through self-removal in elderly patients. We studied the usefulness of an integrated biliary stent and nasobiliary catheter system (UMIDAS NB stent) for biliary drainage in case of accidental NBD tube removal. Patients and methods From April to November 2022, we placed a UMIDAS NB stent in 30 patients with acute cholangitis or obstructive jaundice. We evaluated the plastic stent (PS) position at the time of accidental stent removal and befo

3q27 chromosomal translocation involving the BCL6 gene is one of the most frequent forms of cytogenetic abnormality observed in B-cell lymphoma. We report a case with diffuse large B-cell lymphoma (DLBCL) presenting dual 3q27 translocations. The patient was a 71-year-old man who presented with swelling of multiple abdominal lymph nodes (LNs) and obstructive jaundice. LN biopsy exhibited dense proliferation of atypical large cells expressing CD20, MUM1/IRF4, BCL2, BCL6, and MYC, but not CD10. He was diagnosed with non-GCB/ABC type DLBCL and showed an initially good response to R-CHOP chemothera

INTRODUCTION: The widespread development of minimally invasive treatment methods have expanded the choices and options available to surgeons to manage patients suffering from cholelithiasis and its complications, including choledocholithiasis. As new surgical strategies are developed and become widely available, there is an ongoing debate as to which surgical strategies are optimal for the management of acute cholecystitis and concomitant choledocholithiasis. The treatment of patients in this category should be carried out according to clear criteria, taking into consideration the patient's co
