Group B Streptococcal septicaemia/meningitis in neonates in a Singapore teaching hospital.

Our aims were to establish the incidence and clinical characteristics of early and late onset Group B Streptococcal (GBS) septicaemia in neonates in our hospital over a period of 1 year. Routine screening for maternal GBS was not standard practice in the hospital. GBS was isolated from high vaginal swabs (HVS) obtained antenatally or postnatally for risk factors as determined by the obstetrician or neonatologist in charge. Data obtained were analyzed separately and these did not form part of the study. By a system of clinical case review and follow-up, mail, telephone and home visits, the outc

Vaginal colonization by Escherichia coli as a risk factor for very low birth weight delivery and other perinatal complications.

This study evaluated the relationship of vaginal Escherichia coli colonization to birth weight <1500 g and other perinatal complications in a cross-sectional study of 2646 women at the University of Washington Medical Center, Seattle, between October 1992 and January 1995. Vaginal E. coli colonization was more strongly associated with delivery at <34 weeks (relative risk [RR], 1.7; 95% confidence interval [CI], 1.3-2.3) and very low birth weight (RR, 1.9; 95% CI, 1.3-2.7) than with prematurity between 34 and 36 weeks or low birth weight. Heavy growth of E. coli had a higher risk of very low bi

Risks for bacteremia and urinary tract infections in young febrile children with bronchiolitis.

OBJECTIVE: To compare the risks for bacteremia and urinary tract injections (UTI) in young febrile children with and without bronchiolitis. DESIGN: A prospective cohort study. SETTING: The emergency departments of 3 pediatric referral hospitals. PATIENTS: A convenience sample of 432 previously healthy febrile patients aged 24 months or younger. Patients were divided into groups, based on the presence (n = 163, bronchiolitis group) or absence (n = 269, control group) of wheezing and/or retractions on examination. Blood cultures were obtained from all patients, and urine cultures were obtaine

Colonization in the rectum and uterine cervix with group B streptococci may induce specific antibody responses in cervical secretions of pregnant women.

We have studied the relationships between genital or rectal carriage of group B streptococci (GBS) with the levels of systemic and mucosal antibodies to GBS in 200 women at about week 17 of pregnancy. Secretions from the uterine cervix were collected with absorbent cylindrical wicks for quantification of antibody levels with whole cell enzyme-linked immunosorbent assay. GBS were cultured from the cervix (with or without concomitant rectal colonization) of 13.5%, from the rectum (with or without concomitant cervical colonization) of 12%, and from both culture sites of 8.5% of the women. Serotyp

Mitral valve and left ventricular thrombi in an infant with acquired protein C deficiency. A case report.

The authors present an infant with left ventricular and mitral valve thrombi diagnosed by cross-sectional echocardiography. Thrombosis was due to acquired transient protein C deficiency, which was caused by impaired liver function due to sepsis. Because the thombi were very large and mobile, urgent surgery was performed. Eight weeks later, the patient's protein C level returned to normal ranges. The authors suggest that in all cases with intracardiac thrombosis, protein C deficiency should be investigated.

Early-onset neonatal group B streptococcal sepsis: economics of various prevention strategies.

OBJECTIVES: To evaluate three strategies for preventing group B streptococcal neonatal sepsis in large teaching hospitals and to examine their cost effectiveness and cost benefit. METHODS: A decision tree was constructed to determine the outcome of three prevention strategies: (1) Screen all pregnant women for group B streptococcus (GBS) at 28 weeks' gestation and give intrapartum chemoprophylaxis to all GBS carriers; (2) Screen all pregnant women, but give chemoprophylaxis selectively to those carriers with obstetric risk factors (i.e., premature labour, prolonged rupture of membranes and ma

The limitations and usefulness of C-reactive protein and elastase-alpha 1-proteinase inhibitor complexes as analytes in the diagnosis and follow-up of sepsis in newborns and adults.

C-reactive protein and elastase-alpha 1-proteinase inhibitor complexes were compared in the diagnosis of neonatal sepsis and bacterial infections in adults on the intensive care unit. Both analytes were measured in the same sample immediately after receipt. EDTA-plasma samples (n = 115) from 28 neonates (gestational age 29-42 weeks) within the first 72 hours of life with suspected neonatal sepsis, 2 babies between 14 and 28 days old with B-streptococcus infections and 28 adults on the intensive care unit with positive bacterial cultures were analysed for both analytes. Two adults with long-ter

Neonatal group B streptococcal sepsis during 2 years of a universal screening program.

OBJECTIVE: To assess the feasibility and efficacy of a protocol for universal screening for group B streptococci combined with selective intrapartum prophylaxis at a teaching hospital. METHODS: This is a descriptive study of experience with a standardized protocol in which patients were screened at 26-28 weeks with a rectal and genital culture placed directly in selective media. As risk factors, we used clinical chorioamnionitis, preterm birth, and rupture of the membranes greater than 12 hours. Participants were all women receiving prenatal care at our hospital. Major outcomes were complianc

Mobile extracorporeal membrane oxygenation.

Transport of critically ill neonates in need of extracorporeal membrane oxygenation can be risky. Their extreme cardiorespiratory instability may delay or even preclude conventional transport to an extracorporeal membrane oxygenation center. We report the use of a UH-1 helicopter specially adapted for mobile extracorporeal membrane oxygenation support to transport a critically ill neonate.

Phenotypic diversity in the alpha C protein of group B streptococci.

Group B streptococci (GBS) is the leading cause of neonatal sepsis and meningitis. C proteins are an immunologically important group of surface-associated antigens in GBS that remain incompletely characterized. Two C proteins have been designated alpha and beta on the basis of protease susceptibility. We recently used a monoclonal antibody to describe a protective epitope of the GBS alpha (or trypsin-resistant) C protein in the prototype Ia/c GBS strain. In the present study, we examined 51 GBS isolates for expression of C-protein alpha and beta antigens. The alpha antigen, as detected with mo

Chronic carrier state in mothers of infants with group B streptococcal infections.

Seven of eight women who had given birth to infants with early onset or intrauterine infection caused by group B streptococci remained carriers of the same serotype of group B streptococci up to 38 months after their pregnancy. In contrast, only 34 of 88 group B streptococci carriers who had given birth to healthy infants harbored the same serotype at the 34 months' follow-up (P = .009). Among the control subjects, 29 of 71 showed increased serum levels at followup of antibodies against the serotype isolated at delivery, a significantly higher proportion compared with the mothers of infected i

Mechanisms of pulmonary gas exchange abnormalities during experimental group B streptococcal infusion.

Group B streptococcal sepsis in newborns produces pulmonary arterial hypertension and hypoxemia. The purpose of this study was to investigate the mechanisms by which hypoxemia occurs. Ten anesthetized, ventilated piglets were infused with 2 X 10(9) colony forming units/kg of Group B streptococci over a 30-min period. Pulmonary arterial pressure rose from 14 +/- 2.8 to 38 +/- 6.7 torr after 20 min of the bacterial infusion (p less than 0.01). During the same period, cardiac output fell from 295 to 184 ml/kg/min (p less than 0.02). Arterial PO2 declined from 97 +/- 7 to 56 +/- 11 torr (p less th

Meningitis and sensorineural hearing loss.

A 12 month retrospective study was conducted on 54 children discharged from the Children's Hospital, Birmingham, Alabama, with a diagnosis of generalized meningitis, a major cause of post-natal sensorineural hearing loss (SNHL). Of these high risk patients, 38 or 70% had Haemophilus influenza meningitis and fully 40% of those children tested audiometrically were determined to have SNHL. Because there would appear to be an increase in SNHL in the post meningeal population, all children with a diagnosis of Haemophilus influenza, pneumococcal, or meningococcal meningitis should have an audiologic

Colonization of newborns with group B streptococci: relation to maternal urogenital carriage.

Urethral and cervical specimens were obtained from 786 parturients during labour. 126 women (16%) were found to be urogenital carriers of group B streptococci (GBS). Bacteriological specimens were also obtained from the throat, umbilicus and external auditory canal of their 786 infants immediately after birth, and from 671 of the infants staying at the maternity unit 4 days later. 51% (64/126) of the infants born to GBS carriers were culture-positive for GBS immediately after birth. Only 27% (6/22) of the infants born to women who were GBS culture-positive in the urethra but not in the cervix

A prospective study of group B streptococcal bacteriuria in pregnancy.

Bacteriuria in pregnancy was prospectively studied in 569 women, with specific reference to group B streptococcal infection. Forty-six patients (8%) had bacteriuria, including 14 with group B streptococcal infection; group B streptococci (GBS) were exceeded in frequency only by Escherichia coli. Two thirds of the bacteriuric patients remained asymptomatic. The outcome of pregnancy was studied in 41/46 bacteriuric patients, including all those with group B streptococcal infection. Two pregnancies ended in intrauterine fetal death, and one neonate developed group B streptococcal sepsis; all thre

Nucleotide sequence analysis of genes essential for capsular polysaccharide biosynthesis in Streptococcus pneumoniae type 19F.

Previous studies have shown that the capsular polysaccharide synthesis (cps) locus of the type 19F Streptococcus pneumoniae strain SSZ was closely linked to a copy of the insertion sequence IS1202 (J.K. Morona, A. Guidolin, R. Morona, D. Hansman, and J.C. Paton, J. Bacteriol. 176:4437-4443, 1994). In the present study, we used plasmid insertion and rescue and inverse PCR to clone 6,322 bp of flanking DNA upstream of IS1202. Sequence analysis indicated that this region contains six complete open reading frames (ORFs) and one partial ORF that are arranged as a single transcriptional unit. Chromo

Temporal trends in Group B Streptococcus colonization, serotype distribution, and antimicrobial resistance among pregnant women in the Brazilian Amazon across the COVID-19 pandemic, 2018-2023.

PURPOSE: Group B Streptococcus (GBS) is a leading cause of preterm birth and neonatal infection. The COVID-19 pandemic substantially altered healthcare access, antibiotic use, and pathogen circulation. However, its influence on GBS remains poorly understood. This study evaluated temporal changes in GBS colonization, serotypes, and antimicrobial susceptibility among pregnant women in the Brazilian Amazon before, during, and after the COVID-19 pandemic. METHODS: A total of 1,449 pregnant women at 35–37 weeks of gestation attending public health units in Porto Velho, Brazil, between February 2018
