PURPOSE OF THE STUDY: Recurring and multidirectional instability of the knee joint is a therapeutic problem mainly due to a limited possibility of harvesting grafts for its treatment. The authors present the methodology of the solution of recurring and multidirectional instabilities of the knee joint by means of allografts. They inform about the technique of harvesting and preservation of grafts and include the necessary examinations and early results.
MATERIAL: Due to the above mentioned reasons the authors concentrated on the possibility of harvesting grafts for ligamentoplasty from donors.

OBJECTIVE: To improve the rotational stability of the knee by anatomic reconstruction of the anterior cruciate ligament by socalled double-bundle technique using anteromedial and posterolateral grafts from native semitendinosus and gracilis. The grafts are fixed with bioabsorbable screws utilizing aperture fixation.
INDICATIONS: Complete tear of the anterior cruciate ligament with positive Lachman sign and pivot shift.
CONTRAINDICATIONS: Open growth plate. Osteoarthritis > grade 1 according to Jäger & Wirth. Age > or = 50 years with low sports activity (relative contraindication).

Two cases are reported in which, after ACL reconstruction with autologous hamstring grafts, tibial polylactide interference screws migrated into the knee joint. Clinically, both patients presented with recurrent locking of the joint. In one case, a broken 15 mm-long tip of the screw was found intra-articularly. In the other case, the whole screw had migrated into the joint cavity. The degradation process of polylactic acid, operative technique and bone quality are discussed as possible reasons for these complications.

Fifteen neurovascular pedicle transfers of the abductor digiti quinti to the base of the proximal phalanx of the thumb were performed: seven for laceration of the median nerve, seven for neurological disease, and one for congenital absence of the thenar muscles. Twelve patients who had surgery to repair a rotator cuff tear, which could not be repaired because the residual tendon tissue was of poor quality, insufficient, or both, were followed up for a mean of 33 months. At surgery, the deltoid was detached from the acromion for 4 to 6 cm and an acromioplasty was done. Preoperatively, all patients except one had mild to severe pain.
