Machine learning versus clinical scoring systems for predicting poor wound healing after posterior surgery for thoracolumbar tuberculosis.

OBJECTIVE: Postoperative poor wound healing (PWH) is a significant complication following posterior surgery for thoracolumbar tuberculosis, leading to prolonged recovery and increased healthcare costs. Traditional prognostic markers often lack precision. This study aimed to develop and validate machine learning (ML)-based models to predict PWH and compare their performance with conventional logistic regression and established clinical scoring systems. METHOD: A retrospective analysis of 188 patients undergoing posterior debridement and internal fixation for thoracolumbar tuberculosis was cond

Multifocal spinal and extra-spinal Mycobacterium chelonae osteomyelitis in a renal transplant recipient.

Only localized cases of Mycobacterium chelonae osteomyelitis have been reported. In this article, a 55-year-old immunosuppressed man with M. chelonae osteomyelitis and multiple spinal and extra-spinal involvement is presented. The patient had nodule-pustular skin lesions, spondylodiscitis at multiple levels, and osteolytic lesions at extra-spinal locations. Biopsy and cultures of the osseous lesions showed M. chelonae osteomyelitis. The patient started antimycobacterial chemotherapy with ciprofloxacin and clarithromycin. Progressive cervical kyphosis associated with anterior wedged deformity o

Uninstrumented ventral surgery for subaxial cervical spine tuberculosis: clinical and radiological outcome.

The clinical and radiological outcomes in 14 patients with subaxial cervical spine tuberculosis following uninstrumented anterior decompression surgery and medical treatment were retrospectively reviewed. All the patients underwent an anterior decompression with bone graft followed by immediate mobilization or a period of bed rest for 4 to 6 weeks. The clinical status and whole spine curvature of the cervical spine were assessed preoperatively and at follow up. There was an improvement in the Nurick's grade from a preoperative mean of 2.4 (range 0 - 5) to 1.2 (range 0 - 4) at follow up (p = 0.

Infective discitis as an uncommon but important cause of back pain in older people.

CASE REPORTS: two elderly patients (aged 70 and 80 years) presented with severe back pain and restriction of spinal movements. Inflammatory markers were raised and in each case computed tomography findings confirmed infective discitis. One patient improved with antibiotics but the second developed paraplegia, a recognized complication of discitis. CONCLUSION: the association of back pain, restricted spinal movements and raised inflammatory markers should act as 'red flags', alerting the clinician to the presence of serious, but potentially treatable pathology.

MRI of spondylitis.

MRI of proven spondylitis was performed in 39 patients. Acute spondylitis appeared hypointense on T1-weighted and hyperintense on T2-weighted images. Following intravenous administration of Gd-DTPA, enhancement of the marrow and disc was seen. With progressive healing an increasing signal was seen on T1-weighted images. This was caused by the presence of focal fat marrow representing the first sign of therapy response. As healing progressed, the T1-signal increased due to fatty marrow, while the T2-signal decreased as well as the enhancement following Gd-DTPA administration. Therefore, MR imag

[Pott's disease in African (author's transl)].

A study of anatomical clinical, and therapeutic features of Pott's disease in Africans from 131 cases treated by the same team and compiled from the principal series published during the last ten years in Black Africa. Anatomical features are related to the frequency of the disease (70 p. 100 of tuberculous osteo-arthritis), the elective lumbodorsal localization involving several vertebrae (gpenerally 3 or 4) and being sometimes multifocal. Clinical features are in part explained by the delay with which patients report to the hospital for the first time so that 80 to 90 p. 100 of the patients

Role of antibodies to galactocerebroside in experimental allergic encephalomyelitis.

The possible role of antibodies to galactocerebroside was investigated in AS rats with experimental allergic encephalomyelitis (EAE) induced by guinea-pig spinal cord. Passive immunization with rat or rabbit antiserum to galactocerebroside did not protect rats from EAE. Active immunization with galactocerebroside before the encephalitogenic challenge produced antibody to galactocerebroside but also failed to protect rats from EAE. These experiments did not support previous suggestions that antibodies to galactocerebroside are protective.

Less invasive lumbopelvic stabilization of posterior pelvic ring instability: technique and preliminary results.

BACKGROUND: Lumbopelvic distraction stabilization with (triangular osteosynthesis) or without additional iliosacral screw allows anatomic reduction of the posterior pelvic ring after severely displaced sacral fractures, correction or resection osteotomies of malunions, respectively, septic sacroiliitis and permits early weight bearing. However, this technique is complicated by wound necrosis or infection in up to 20% to 30%. We describe our experience with a less invasive technique. METHODS: The presented technique was performed in a consecutive series (December 2006 to October 2009) of 10 pa

Biological therapies in the spondyloarthritides--the current state.

Therapeutic options for patients suffering from the more severe spondyloarthritides (SpA) have been rather limited in the last decades. Evidence is now accumulating that anti-tumour necrosis factor (TNF) therapy is highly effective in SpA, especially in ankylosing spondylitis (AS) and psoriatic arthritis (PsA). Based on the data recently published concerning more than 1000 patients with AS and PsA, this treatment seems to be even more effective than in rheumatoid arthritis (RA). The anti-TNFalpha agents currently available, infliximab (Remicade), etanercept (Enbrel) and adalimumab (Humira), ar

A rare case of glial fibrillary acidic protein astrocytopathy that resolved spontaneously within a self-limited course.

Glial fibrillary acidic protein astrocytopathy is a form of autoimmune meningoencephalomyelitis. The presence of antibodies in spinal fluid against glial fibrillary acidic protein is necessary to diagnose the disease. There is no standard treatment and few cases of glial fibrillary acidic protein astrocytopathy have been reported. A 31-year-old healthy Japanese man presented to our emergency department with a 7-day history of fever and headache. He was in good general condition, without abnormalities on physical examination, and a general hematological examination revealed hyponatremia (130 mE

Biomarkers of Cerebral Injury and Inflammation in Pediatric Tuberculous Meningitis.

BACKGROUND: Tuberculous meningitis (TBM) leads to death or disability in half the affected individuals. Tools to assess severity and predict outcome are lacking. Neurospecific biomarkers could serve as markers of the severity and evolution of brain injury, but have not been widely explored in TBM. We examined biomarkers of neurological injury (neuromarkers) and inflammation in pediatric TBM and their association with outcome. METHODS: Blood and cerebrospinal fluid (CSF) of children with TBM and hydrocephalus taken on admission and over 3 weeks were analyzed for the neuromarkers S100B, neuron-

P09 Uncommon cause of back pain

Abstract Case report - Introduction Back pain is one of the common musculoskeletal presentations to GPs and rheumatologists. While common causes are frequent, our patient required extensive investigation to reach a clinical diagnosis. This case draws attention to complexities involved and aims to refresh knowledge of less common causes. Case report - Case description A 40-year-old man presented to the GP with a few months history of gradual onset worsening low back and buttock pain, which was non-radiating resulting in reduced mobility. There was no preceding illness, systemic symptoms, gross

Decreased mortality seen in rifampicin/multidrug-resistant tuberculous meningitis treated with linezolid in Shenzhen, China.

BACKGROUND: The morbidity of rifampicin/multidrug-resistant tuberculous meningitis (RR/MDR-TBM) has shown an increasing trend globally. Its mortality rate is significantly higher than that of non-rifampicin/multidrug-resistant tuberculous meningitis (NRR/MDR-TBM). This article aimed to explore risk factors related to RR/MDR-TBM, and compare therapeutic effects of linezolid (LZD)- and non-linezolid-containing regimen for RR/MDR-TB patients in Shenzhen city. Furthermore, we aimed to find a better therapy for pathogen-negative TBM with RR/MDR-TBM related risk factors. METHODS: We conducted a ret

How is the microbial diagnosis of bacterial vertebral osteomyelitis performed? An 11-year retrospective study.

Vertebral osteomyelitis (VOM) is often diagnosed with delays, resulting in poorer outcomes. Microbial documentation is particularly challenging and obtained using blood cultures (BCs) and vertebral biopsies (VBs; CT-guided or surgical). We retrospectively analysed VOM cases in a tertiary reference centre between 2004 and 2015, focusing on how and how quickly microbiological diagnosis was performed. Among 220 VOM, 88.2% had documentation, including Gram-positive cocci (GPC) (70.6%), Gram-negative rods (GNR) (9.3%), anaerobes (3.6%), polybacterial infections (6.7%) and tuberculosis (9.8%). BCs w
