Consistencies between recalled panic and lactate-induced panic.

This study investigated the frequency and severity of symptoms during naturally occurring panic attacks recollected as "usual" and during sodium lactate-induced attacks. Seventy-six male and 126 female patients with panic disorder or agoraphobics with panic attack (DSM-III criteria) underwent lactate infusion studies and were serially administered the Acute Panic Inventory (API). Fifty-nine percent of the subjects were rated by an attending psychiatrist to have experienced lactate-induced panic attacks. Patients were also asked to API symptom rate their "usual" panic attacks. For panic recolle

Effectiveness of an intensive partial hospitalization program for treatment of anxiety disorders.

This study evaluates the effectiveness of a partial hospitalization program for the treatment of anxiety disorders. The 5-week program was based on an integration of biological, cognitive, and behavioral theories. Neuman's systems theory provided the framework for the study. The 32 participants were tested pre- and posttreatment. All three hypotheses were supported; "fear of fear," severity of impairment, and general emotional distress were all significantly lower posttreatment than pretreatment. The limitations, including the lack of a comparison group, possible sample bias, and the lack of l

Effect of fluvoxamine on panic disorder.

Several reports suggest that selective serotonin reuptake blockers are helpful in the treatment of panic disorder. The aim of the study was to compare fluvoxamine with placebo in 50 panic disorder patients by using an 8-week, double-blind, parallel-groups design. Weekly assessment included a panic attack diary (frequency and severity), the Montgomery-Asberg Depression Scale, the Clinical Anxiety Scale, and the Sheehan Disability Scale. Although both groups improved on all measures, the fluvoxamine group experienced significantly less frequent major panic attacks from the third week on and sign

Response to treatment in panic disorder with associated depression.

Thirty outpatients with panic disorder (PD) were treated at an anxiety clinic using a symptom-focused approach. Thirteen (43%) had no history of major depressive disorder (MDD), 12 (40%) had a history of secondary MDD, and 5 (17%) had history of primary MDD. The outcome was equally good in patients with no history of MDD (77% had good or excellent outcomes) and patients with a history of secondary MDD (83% had good or excellent outcomes). Patients with a history of primary MDD had the worst outcomes. These findings agree with earlier studies showing that secondary MDD does not adversely affect

Cognitive therapy of panic disorder. A nonpharmacological alternative.

A naturalistic study with no predetermined duration of treatment was undertaken in order to examine the effectiveness of cognitive therapy in the treatment of panic disorder. Seventeen patients diagnosed as having panic disorder according to the Structured Clinical Interview for DSM-III Personality Disorders received a mean of 18 individual cognitive therapy sessions. Patients with personality disorder or depression required a longer duration of treatment to become symptom-free. As measured by a self-report weekly panic log, the mean number of panic attacks was reduced significantly to zero at

Effects of three non-insight-oriented treatment methods on agoraphobic women suitable for insight-oriented psychotherapy.

Twenty-three agoraphobic women rated suitable for insight-oriented psychotherapy received one of three randomly assigned non-insight-oriented types of treatment for 3 months and the effect was followed up after 9 months. All patients received basal therapy (B) in the form of standardized information, self-exposure instructions and anxiolytic medication. One group received this treatment only with monthly appointments. The others, in addition, received either therapist-directed prolonged exposure in vivo (PE) or relaxation therapy (R). The effects of insight-oriented psychotherapy could not be

Factors associated with incomplete recovery in primary depressive illness.

Factors associated with chronicity in primary depression were investigated. Chronic secondary and episodic primary depressives served as control groups. Shortened REM latency and relative absence of pronounced characterologic pathology in the chronic primary depressive group were similar to data for episodic primary depressive controls. Chronicity in primary depression was significantly correlated with a positive family history of affective disorder, multiple losses through death of family members, disabled spouses, concurrent incapacitating medical diseases (especially their treatment with de

Psychotherapy with phobic patients: the marriage relationship as the source of symptoms and focus of treatment.

Four patients demonstrating phobic symptoms occurring in the context of maladaptive marriages are presented. Thorough historical data collection is stressed. Similarities in family backgrounds and personality styles emerge in these cases and are discussed in relation to genesis of symptoms within the marital relationships. A psychotherapeutic approach is described and results of treatment examined.

Transdiagnostic dimensions of psychopathology in chronic tinnitus patients with and without hearing loss.

Chronic tinnitus is frequently described as occurring with psychiatric comorbidities. However, this diagnosis-based framing may obscure unde … tance-related symptoms. TRD was significantly predicted by diagnosis pointers for major depression, agoraphobia, health anxiety, anorexia nervosa, and psychosis. Factor analysis identified three dimensions: Internalizing psychopathology, harmful substance use, and fear-related (social) perceptions. Internalizing psychopathology predicted TRD. HL status was as …

Disorder-specific internet-based cognitive-behavioral therapy in treating panic disorder, comorbid symptoms and improving quality of life: A meta-analytic evaluation of randomized controlled trials.

Compared to conventional face-to-face psychological treatments, internet-based cognitive-behavioral therapy (iCBT) presents an innovative alternative that has been found to be effective in the treatment of anxiety disorders. The current study provides a meta-analysis investigating the efficacy of disorder-specific guided self-help (Gsh) iCBT compared to various active and inactive control conditions, with focus on adult panic disorder sufferers with or without agoraphobia (PD/A). Systematic literature search yielded 13 randomized controlled trials (RCTs) (N = 1214) that met the eligibility cri…
