CONTEXT: Aphakic glaucoma is a serious, sight-threatening complication in children who remain aphakic following congenital cataract surgery. The reported incidence varies from 15% to 45% and it has a higher incidence in small eyes and in babies who undergo surgery before 4 weeks of age. Most cases take the form of open-angle glaucoma. Despite careful monitoring, diagnostic difficulties in children may lead to delayed treatment. Aphakic children require ongoing monitoring, including examinations carried out under general anaesthesia.

BACKGROUND: The risk of glaucoma among aphakic children is as high as 32%, based primarily on intraocular pressure (IOP) measurements. Although IOP may be falsely elevated by increased central corneal thickness, central corneal thickness (CCT) values have not been reported in this population.

PURPOSE: To describe four patients who developed cystoid macular edema shortly after onset of treatment with latanoprost.
METHOD: Retrospective review of medical records of patients with open-angle glaucoma who developed cystoid macular edema shortly after starting latanoprost.
RESULTS: The use of topical latanoprost was temporally related to the development of cystoid macular edema in four patients (six eyes; two aphakic eyes and four pseudophakic eyes). Cystoid macular edema resolved in all patients after latanoprost was discontinued.

BACKGROUND: There are several advantages to implanting a posterior chamber intraocular lens (IOL) in the ciliary sulcus when the lens capsule is absent. I have developed a simple method of fixating an IOL into the ciliary sulcus which does not require different needles, multiple needle passes, scleral dissection, or awkward surgical maneuvers.

Seventy-nine eyes (61 patients) with open-angle glaucoma and uncontrolled intraocular pressure (IOP) of 23 mmHg or more despite maximal tolerated medical therapy and prior argon laser trabeculoplasty (75 eyes) were treated with neodymium: YAG (Nd: YAG) laser angle surgery and followed for at least 1 year. Forty-four aphakic and 45 pseudophakic eyes with typical cystoid macular edema following cataract surgery were reevaluated one to four years following their initial fluorescein angiograms in an effort to compare the natural courses of the macular lesions in the two groups of eyes. In a large proportion of late operative complications after anterior segment surgery good results can be achieved only by the use of a suction cutter. A review of the results of 120 such corrections carried out over a period of 20 months at Groote Schuur Hospital, including the operative and postoperative complications and final visual acuities, showed that the use of the suction cutter was most beneficial, except in cases of aphakic glucoma with blocked drainage sites. A two year study of one hundred and forty five cataract extractions showed a 38% incidence of post operative macular oedema. The leak had resolved in most cases by six months and the only factor of significance in the production of the oedema was preoperative hypertension. The likelihood of obtaining 6/6 vision postoperatively was much lessened in the presence of macular oedema.
