Bypass to the intracranial internal carotid artery.

Extracranial to intracranial internal carotid artery bypass surgery with vein is well described for a number of diverse conditions. They provide high blood flow with good initial patency. However, long term patencies for specific graft types remains unknown. This is an analysis of consecutive interposition saphenous vein bypass cases between the common carotid artery (CCA) and the intracranial internal carotid artery (ICA) where the distal anastomosis was placed end-to-end immediately proximal to the posterior communicating artery in 55 patients undergoing 57 bypass procedures (bilateral in 2

Cerebral hemodynamics and metabolism in moyamoya disease--a positron emission tomography study.

We studied the cerebral blood flow (CBF), cerebrovascular response to hypercapnia, oxygen extraction fraction (OEF), metabolic rate for oxygen (CMRO2), blood volume (CBV) and transit time (TT: CBV/CBF) in Moyamoya disease using positron emission tomography (PET). The subjects consisted of 23 patients with Moyamoya disease, including eight pediatric and 15 adult patients. Among them, ten patients were examined both before and after surgery. The pediatric patients showed a marked increase in the CBV and a prolonged TT, especially in the striatum. The adult patients also showed a prolonged TT, bu

Long-term results of aortoinnominate and aortocarotid polytetrafluoroethylene bypass grafting for atherosclerotic lesions.

Between January 1978 and December 1986, 69 polytetrafluoroethylene bypass grafts on the ascending aorta were placed in 53 patients with atherosclerotic occlusive disease of the innominate and left common carotid arteries. Thirty-six patients had symptoms predominantly of amaurosis fugax, verebrobasilar ischemia, and upper limb ischemia. The remaining 17 patients had no symptoms but had severe hemodynamically significant occlusive disease. All operations were accomplished via median sternotomy. One patient died after surgery (operative mortality rate 1.9%), and one patient had minor postoperati

Clopidogrel Versus Dual-Antiplatelet Therapy for Long-Term Maintenance After Coronary Stenting in Ischemic and Bleeding Birisk Patients With Acute Coronary Syndromes and Diabetes: A Prespecified Subgroup Analysis of the OPT-BIRISK Trial.

BACKGROUND: Among patients with acute coronary syndromes at both high bleeding and ischemic risk (birisk), extended clopidogrel monotherapy after 9 to 12 months of dual-antiplatelet therapy reduces bleeding without increasing ischemia. Whether this benefit extends to birisk patients with diabetes is unknown. METHODS: This prespecified subgroup analysis of the OPT-BIRISK (Optimal Antiplatelet Therapy for High Bleeding and Ischemic Risk Patients) trial included birisk patients with acute coronary syndrome who had completed 9 to 12 months of dual-antiplatelet therapy after percutaneous coronary

The Efficiency of FLAIR Images for Hemodynamic Change After STA-MCA Bypass with Moyamoya Disease and Symptomatic Steno-Occlusive Disorder.

Background: Hyperintense vessels (HVs) visualized on FLAIR MRI are believed to reflect sluggish antegrade or retrograde flow in leptomeningeal collaterals that develop in response to major intracranial artery stenosis or occlusion. HV is frequently observed in conditions such as Moyamoya disease and symptomatic ICA/MCA steno-occlusion. However, the relationship between HV and cerebral hemodynamics-and the effect of STA-MCA bypass on HV-remains inadequately characterized. This study aimed to investigate the relationship between HV on FLAIR and cerebral vascular hemodynamic status, as measured b

Clonal Hematopoiesis Is Associated With Long-Term Adverse Outcomes Following Cardiac Surgery.

BACKGROUND: Cardiac surgery triggers sterile innate immune responses leading to postoperative complications. Clonal hematopoiesis (CH) is associated with short-term inflammation-mediated outcomes after cardiac surgery. The impact of CH on long-term postoperative outcomes remains unknown. METHODS AND RESULTS: In this cohort study, patients undergoing elective cardiac surgery were included from January 2017 to September 2019. Patients were screened for CH using a predefined gene panel of 19 genes. Recorded clinical events were all-cause death, major adverse cardiac and cerebral events including

Seven-year follow-up after minimally invasive direct coronary artery bypass: experience with more than 1300 patients.

BACKGROUND: Minimally invasive direct coronary artery bypass (MIDCAB) for revascularization of the left anterior descending artery has become a routine operation. Here we present the experience after more than 1300 MIDCAB procedures with up to 7 years of follow-up. METHODS: All patients undergoing standard MIDCAB between 1996 and 2004 were included. Long-term follow-up information about health status, major cardiac and cerebral adverse events, and freedom of angina was collected annually by a questionnaire or personal contact. Preoperative, intraoperative, and postoperative data could be comp

Diving MCA and First-Pass Effect: A Subgroup Analysis From the ARISE II Study.

BACKGROUND: The impact of internal carotid artery tortuosity on achieving first-pass effect in mechanical thrombectomy has been described; however, middle cerebral artery (MCA) tortuosity remains understudied. We investigated the association of MCA tortuosity with radiographic and functional outcome using quantitative and qualitative measures. METHODS: The ARISE II study (Analysis of Revascularization in Ischemic Stroke With EmboTrap) was used to identify patients with anterior circulation stroke. MCA tortuosity was calculated using the Inflection Count Metric by 2 readers blinded to the angi

Minocycline prevents focal neurological deterioration due to cerebral hyperperfusion after extracranial-intracranial bypass for moyamoya disease.

BACKGROUND: Cerebral hyperperfusion (CHP) is a potential complication of superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis for moyamoya disease (MMD), and optimal postoperative management has not yet been established. Minocycline, a neuroprotective antibiotic agent, plays a role in blocking matrix metalloproteinase 9 (MMP-9), which contributes to edema formation and hemorrhagic conversion after cerebral ischemia-reperfusion. Patients with MMD have been shown to have increased serum MMP-9 levels. OBJECTIVE: To examine the effect of minocycline on the prevention of postop

Therapeutic strategies for patients with internal carotid or middle cerebral artery occlusion complicated by severe coronary artery disease.

BACKGROUND/OBJECTIVES: Ischemic stroke is one of major complications of cardiac surgery. Although a current American Heart Association (AHA) guideline states that carotid endarterectomy is probably recommended before or concomitant to coronary artery bypass grafting (CABG) for the carotid stenosis, there is no report that analyzed optimal strategies in cardiac surgery for patients with total occlusion of the internal carotid artery (ICA) or the middle cerebral artery (MCA). Therefore, this preliminary study was aimed to clarify whether preoperative blood flow measurements and prophylactic supe

Diagnosis and management of tandem occlusion in acute ischemic stroke.

Approximately 20-30% of patients with acute ischemic stroke, caused by large intracranial vessel occlusion, have a tandem lesion, defined as simultaneous presence of high-grade stenosis or occlusion of the cervical internal carotid artery and thromboembolic occlusion of the intracranial terminal internal carotid artery or its branches, usually the middle cerebral artery. Patients with tandem lesions have usually worse outcomes than patients with single intracranial occlusions, and intravenous thrombolysis is less effective in these patients. Although endovascular thrombectomy is currently a co

Clinical and vascular features of Takayasu arteritis at the time of ischemic stroke.

OBJECTIVES: Takayasus arteritis (TA) is a systemic vasculitis whose clinical presentation varies from asymptomatic to serious neurovascular events, including stroke. However, few studies are currently available assessing stroke in TA. Thus, we described the clinical and laboratory characteristics and vascular imaging features in patients with TA at the time of stroke. MATERIALS AND METHODS: This is a single center retrospective cohort study investigating the clinical and demographic data of 18 (15.0%) patients with a history of stroke confirmed by imaging methods, among 120 patients with TA,

Intermediate outcome after carotid stenting: what should we expect?

Atherosclerosis of the extracranial carotid artery is a major public health burden. Stroke is the third leading cause of death in Western countries, after heart disease and cancer, and the leading cause of long-term disability. In the United States, there are more than 500,000 strokes annually, accounting for approximately 3 million stroke survivors with varying degrees of disability. Data from stroke registries suggest that internal carotid artery atheroembolic disease accounts for approximately 35% of all ischemic cerebral infarctions; therefore, approximately 150,000 strokes in the United S

Age-dependent associations of peripheral immune-inflammatory indices with cerebrovascular remodeling after revascularization in ischemic moyamoya disease.

OBJECTIVE: Peripheral immune-inflammatory activity may influence postoperative vascular remodeling in Moyamoya disease (MMD), but the relevant pathways may differ by developmental stage. We investigated age-dependent associations of blood cell-derived inflammatory indices with two complementary imaging outcomes: collateralization angiogenesis ratio (CAR) and arterial residual volume (ARV). METHODS: This retrospective observational study included 108 patients with ischemic MMD who underwent revascularization (76 adults and 32 pediatric patients). Associations of neutrophil-to-lymphocyte ratio
