Endoscopic-assisted percutaneous fixation for displaced anterior inferior iliac spine avulsion fractures: a prospective cohort study.

INTRODUCTION: Anterior inferior iliac spine (AIIS) avulsion fractures commonly occur in adolescent patients during sports activities. To systematically evaluate fracture severity and guide management, an adaptation of the Hetsroni classification system was used to categorize fractures on the basis of their displacement relative to the acetabular rim. Traditional open reduction and internal fixation reported satisfactory consolidation rates but complications such as lateral femoral cutaneous nerve (LFCN) neuropathies, heterotopic ossifications (HO), and subspine impingement. The objectives of t

A novel external fixation for treating tibial fractures: a finite element and biomechanical study.

OBJECTIVE: Design a new type of external fixation device that is small in size, high in strength, and capable of achieving the mechanical requirements for fracture healing. Verify the rationality and effectiveness of the device in treating tibial fractures through finite element analysis and biomechanical comparative tests. METHODS: Finite element simulation was performed on the new external fixation device to treat fractures, to verify whether the mechanical properties of the device meet the requirements of fracture healing. A fracture gap model was created using Sawbones to simulate midshaf

Functional Outcome of Subtrochanteric Femoral Fractures Fixation by Proximal Femoral Locking Compression Plate.

Subtrochanteric femoral fractures are one of the common fractures encountered in today's Orthopaedic practice. This area consists of mostly cortical bone with high stress generation thus heal slowly which leads implant failure. The inherent instability of this fracture and forces of the muscles with comminuted medial calcar is giving the fracture a tendency to varus collapse. Intramedullary nails are not suitable for short proximal segment and wide medullary canal while extramedullary implants are associated with higher rate of implant failure. Recently proximal femoral locking compression pla

Fixation in Intertrochanteric Fractures Using Short Proximal Femoral Nail Anti-Rotation-2: A Functional and Radiological Prospective Study.

INTRODUCTION: Femoral shaft fractures are highly prevalent. Surgical treatment is the optimal approach for managing them as it greatly decreases the rates of illness and death in injured people. The optimal therapy for these diseases entails the utilization of nail, plates, screws, and external fixation devices. At present, reamed intramedullary nailing is regarded as the optimal approach and universally recognized treatment indicated for fractures occurring in the shaft of the femur. EXPERIMENTAL PROCEDURES AND TECHNIQUES: The study done at Krishna Hospital in Karad between February 2022 and

Biomechanical study of two different fixation methods for the treatment of Neer III proximal humerus fractures.

BACKGROUND: The lateral locking plate for the proximal humerus is currently the most commonly used surgical procedure for the treatment of elderly proximal humeral comminuted fractures. Previous studies have found that the rate of postoperative complications in patients of proximal humerus fractures with medial column involvement is relatively high. Through biomechanical methods, this study aims to investigate the effectiveness of the conventional lateral locking plate fixation along with the addition of the metacarpal supporting plate on the medial column in the treatment for proximal humeral

Comparison of complications and reoperations in AO/OTA 43.C3 pilon fractures treated with conventional ORIF versus minimally invasive hexapod ring fixation.

INTRODUCTION: Controversy exists regarding the optimal management of AO/OTA 43. C3 pilon fractures. Open reduction and internal fixation (ORIF) is the gold standard treatment, but serious soft tissue and infectious complications have been previously reported. Minimally invasive strategies using hexapod ring fixation (HRF) with supplemental limited internal fixation have been used to reduce the incidence of complications. Previous studies have included heterogeneous types of pilon fractures, with non-comminuted injuries being more likely to be treated with ORIF and complex fractures receiving H

Epidemiology of isolated olecranon fractures: a detailed survey on a large sample of patients in a suburban area.

BACKGROUND: Literature lacks data concerning several epidemiologic aspects of isolated olecranon fractures (IOFs). The few studies that have analyzed this type of fracture show a low sample size and contradicting results. METHODS: This retrospective study included 165 consecutive patients (82 men and 83 women) who sustained an IOF in the past 10 years. Participants who were aged <16 years or had a previous elbow fracture or had a fracture that involved other bones of the elbow joint were excluded. Data regarding age, sex, season, date, and fracture side were collected. As per the mechanism of

Two-Year Functional Outcomes of Operative vs Nonoperative Treatment of Completely Displaced Midshaft Clavicle Fractures in Adolescents: Results From the Prospective Multicenter FACTS Study Group.

BACKGROUND: The optimal treatment of midshaft clavicle fractures is controversial. Few previous comparative functional outcome studies have investigated these fractures in adolescents, the most commonly affected epidemiologic subpopulation. PURPOSE/HYPOTHESIS: The purpose was to prospectively compare the outcomes of operative versus nonoperative treatment in adolescents with completely displaced midshaft clavicle fractures. The study hypothesis was that surgery would yield superior outcomes. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: Patients aged 10 to 18 years treated for

Excision Versus Fixation of the Radial Head: A Comparative Study of the Functional Outcomes of the Two Techniques.

INTRODUCTION: The management of displaced and comminuted radial head fractures has been a matter of debate amongst surgeons for many years. Radial head excision formed the mainstay of surgical management of these injuries. Over the years, there have been improvements in the surgical techniques and availability of better implants and instrumentation techniques, hence, open reduction and internal fixation of these fractures is gaining popularity. AIM: To compare the outcome of elbow function between radial head excision and open reduction and internal fixation of the radial head with mini screw

[Treatment of complex fractures of the ankle and their sequellae using trans-plantar intramedullary nailing].

UNLABELLED: Nine patients were treated for complex tibial pilon and talus fractures or non unions from 1990 to 1997 using intramedullary nails introduced through the calcaneus. All patients were followed until healing. The average follow-up was 14 months. Failure of the procedure was defined by the occurrence of complications or reintervention. We used a modified Seidel nail in 5 cases, a tibial shaft nail in 3 cases, and a special nail in one case. All nails but two were locked. CASES: These arthrodeses were performed for treatment of sequels from open talus fractures. Transplantar nailing w

Open Lumbar Spine Fracture-Dislocation in a Rural Setting Associated With a Fall From Height: A Case Report.

Open spinal fractures are rare and highly complex injuries associated with a high risk of infection and neurological impairment. We report a case of a previously healthy 25-year-old male who sustained a fall from approximately four meters in a rural setting, with direct impact of the lumbar region against a toilet, resulting in an extensive wound contaminated with feces, urine, soil, and ceramic fragments. Upon admission, he presented with an approximately 15-cm lacerated and contused wound, disruption of the paravertebral fascia, and exposure of the posterior spinal elements. Computed tomogra

Combined Regional Analgesia for Complex Unilateral Clavicular and Thoracic Trauma Undergoing Clavicle Fixation: A Case Report.

Patients with simultaneous clavicular and ipsilateral thoracic trauma, including multiple rib fractures and traumatic pneumothorax, present a complex perioperative analgesic challenge. Effective pain control must be balanced against the preservation of respiratory function while minimizing opioid-related adverse effects. Individual regional anesthesia techniques for clavicle surgery and rib fracture analgesia are well established; however, selecting an appropriate multimodal analgesic strategy for patients with combined injuries requires careful clinical judgment. We report the case of a 51-ye

Results of Linked Convertible Total Elbow Arthroplasty for the Management of Distal Humeral Fractures in the Elderly.

PURPOSE: Total elbow arthroplasty (TEA) is increasingly used for the management of comminuted distal humeral fractures in elderly patients. There are limited data on the outcome of modern elbow arthroplasty designs in larger patient cohorts. The aim of the current study was to review the outcomes and complications using a cemented convertible TEA system in a linked configuration in patients with distal humeral fractures. METHODS: Patients with distal humeral fractures treated with TEA and a minimum of 2 years' follow-up were reviewed. Demographic information, patient-reported outcome, functio

Bone Allograft and Locking Plate for Severe Proximal Humeral Fractures: Early and Late Outcomes.

BACKGROUND Early failure of osteosyntheses is common even with use of locking plates. In patients with comminuted fractures and epiphyseal osseous defects, we performed a series of osteosyntheses by locking plate in combination with an allograft bone augmentation. Because of encouraging short-term results in the literature, we assumed that the method could be a potential alternative to a reverse shoulder prosthesis. MATERIAL AND METHODS Twenty-six patients with a dislocated proximal humeral fracture (Neer IV/V/VI) were studied. A lyophilized allogeneic bone graft was used to reinforce the hume

Olecranon Osteotomy Fixation Following Distal Humerus Open Reduction and Internal Fixation: Clinical Results of Plate and Screws Versus Tension Band Wiring.

Olecranon osteotomy allows for improved visualization of the distal humeral articular surface. This study compared the clinical outcomes of 2 methods of olecranon repair following olecranon osteotomy as part of distal humerus fracture repair. This was a retrospective review of distal humerus fractures treated via a transolecranon approach during a 9-year period. In each case, the olecranon osteotomy was fixed with either tension band wiring (TBW) or plate fixation (PF). Patient demographics, injury information, and surgical management were recorded. Measured outcomes included elbow motion, tim

Acute total hip arthroplasty combined with internal fixation for displaced acetabular fractures in the elderly: a short-term comparison with internal fixation alone after a minimum of two years.

AIMS: Displaced, comminuted acetabular fractures in the elderly are increasingly common, but there is no consensus on whether they should be treated non-surgically, surgically with open reduction and internal fixation (ORIF), or with acute total hip arthroplasty (THA). A combination of ORIF and acute THA, an approach called 'combined hip procedure' (CHP), has been advocated and our aim was to compare the outcome after CHP or ORIF alone. PATIENTS AND METHODS: A total of 27 patients with similar acetabular fractures (severe acetabular impaction with or without concomitant femoral head injury) w

Management of complex open fracture injuries of the midfoot with external fixation.

Ten patients (11 feet) with severe, high-velocity, open injuries to the midfoot were treated with uniplanar external fixation. The mean patient age was 38 years. Five wounds measured >10 cm, and 3 had extensive degloving of the foot extending into the lower leg. All had grossly comminuted fractures of the tarsal and metatarsal bones: 9 patients had a fractured cuboid; 6 had a fractured navicular; 7 had a fractured cuneiform; and all had metatarsal fractures. Lisfranc joint dislocations were present in 7 feet, and intertarsal dislocations were seen in 3 cases. Six patients underwent split-thick

Gunshot wound to the frontal sinus.

Immediate treatment of this patient, evaluation of the wound, the use of antibiotics, and eventual reconstruction are all concerns in this complicated gunshot wound to the frontal sinus. The consultants agree that the first priority in this patient is to assess and stabilize the airway, examine the wound, and obtain a CT scan to ascertain the extent of the defect. Dr. Key feels that these wounds are contaminated and would prophylactically order a broad-spectrum cephalosporin and/or chloramphenicol. Dr. Tami would use antibiotics aimed at Staphylococcus and oral flora; Dr. Donald prefers to tre
