With a little help from a computer: discriminating between bacterial and viral meningitis based on dominance-based rough set approach analysis.

Differential Diagnosis of bacterial and viral meningitis remains an important clinical problem. A number of methods to assist in the diagnoses of meningitis have been developed, but none of them have been found to have high specificity with 100% sensitivity. We conducted a retrospective analysis of the medical records of 148 children hospitalized in St. Joseph Children's Hospital in Poznań. In this study, we applied for the first time the original methodology of dominance-based rough set approach (DRSA) to diagnostic patterns of meningitis data and represented them by decision rules useful in d

Developmental exposure to paraquat and maneb can impair cognition, learning and memory in Sprague-Dawley rats.

Paraquat and maneb are identified environmental pollutants. Combined exposure to paraquat and maneb is a latent risk factor for many diseases, particularly those of the central nervous system, including Parkinson's disease and Alzheimer's disease. Hippocampus is the key structure in memory formation and babies are more sensitive to environmental stimuli than adults, so we investigated the neurotoxicity of paraquat and maneb on the hippocampi of rat pups. Female and male Sprague-Dawley rats were mated (female : male = 2 : 1) every night for a week. The gravid rats were randomly divided into thr

Daily electronic self-monitoring in bipolar disorder using smartphones - the MONARCA I trial: a randomized, placebo-controlled, single-blind, parallel group trial.

BACKGROUND: The number of studies on electronic self-monitoring in affective disorder and other psychiatric disorders is increasing and indicates high patient acceptance and adherence. Nevertheless, the effect of electronic self-monitoring in patients with bipolar disorder has never been investigated in a randomized controlled trial (RCT). The objective of this trial was to investigate in a RCT whether the use of daily electronic self-monitoring using smartphones reduces depressive and manic symptoms in patients with bipolar disorder. METHOD: A total of 78 patients with bipolar disorder accor

DNA methyltransferase 1 mutations and mitochondrial pathology: is mtDNA methylated?

Autosomal dominant cerebellar ataxia-deafness and narcolepsy (ADCA-DN) and Hereditary sensory neuropathy with dementia and hearing loss (HSN1E) are two rare, overlapping neurodegenerative syndromes that have been recently linked to allelic dominant pathogenic mutations in the DNMT1 gene, coding for DNA (cytosine-5)-methyltransferase 1 (DNMT1). DNMT1 is the enzyme responsible for maintaining the nuclear genome methylation patterns during the DNA replication and repair, thus regulating gene expression. The mutations responsible for ADCA-DN and HSN1E affect the replication foci targeting sequence

How much is a word? Predicting ease of articulation planning from apraxic speech error patterns.

BACKGROUND: According to intuitive concepts, 'ease of articulation' is influenced by factors like word length or the presence of consonant clusters in an utterance. Imaging studies of speech motor control use these factors to systematically tax the speech motor system. Evidence from apraxia of speech, a disorder supposed to result from speech motor planning impairment after lesions to speech motor centers in the left hemisphere, supports the relevance of these and other factors in disordered speech planning and the genesis of apraxic speech errors. Yet, there is no unified account of the struc

Haptic computer-assisted patient-specific preoperative planning for orthopedic fractures surgery.

PURPOSE: The aim of orthopedic trauma surgery is to restore the anatomy and function of displaced bone fragments to support osteosynthesis. For complex cases, including pelvic bone and multi-fragment femoral neck and distal radius fractures, preoperative planning with a CT scan is indicated. The planning consists of (1) fracture reduction-determining the locations and anatomical sites of origin of the fractured bone fragments and (2) fracture fixation-selecting and placing fixation screws and plates. The current bone fragment manipulation, hardware selection, and positioning processes based on

Towards limb position invariant myoelectric pattern recognition using time-dependent spectral features.

Recent studies in Electromyogram (EMG) pattern recognition reveal a gap between research findings and a viable clinical implementation of myoelectric control strategies. One of the important factors contributing to the limited performance of such controllers in practice is the variation in the limb position associated with normal use as it results in different EMG patterns for the same movements when carried out at different positions. However, the end goal of the myoelectric control scheme is to allow amputees to control their prosthetics in an intuitive and accurate manner regardless of the

Weight-bearing MR imaging as an option in the study of gravitational effects on the vocal tract of untrained subjects in singing phonation.

Magnetic Resonance Imaging (MRI) of subjects in a supine position can be used to evaluate the configuration of the vocal tract during phonation. However, studies of speech phonation have shown that gravity can affect vocal tract shape and bias measurements. This is one of the reasons that MRI studies of singing phonation have used professionally trained singers as subjects, because they are generally considered to be less affected by the supine body position and environmental distractions. A study of untrained singers might not only contribute to the understanding of intuitive singing function

Investigation of time and weather effects on crash types using full Bayesian multivariate Poisson lognormal models.

Previous research shows that various weather elements have significant effects on crash occurrence and risk; however, little is known about how these elements affect different crash types. Consequently, this study investigates the impact of weather elements and sudden extreme snow or rain weather changes on crash type. Multivariate models were used for seven crash types using five years of daily weather and crash data collected for the entire City of Edmonton. In addition, the yearly trend and random variation of parameters across the years were analyzed by using four different modeling formul

Image-guided neurosurgery with 3-dimensional multimodal imaging data on a stereoscopic monitor.

BACKGROUND: In the past 2 decades, intraoperative navigation technology has changed preoperative and intraoperative strategies and methodology tremendously. OBJECTIVE: To report our first experiences with a stereoscopic navigation system based on multimodality-derived, patient-specific 3-dimensional (3-D) information displayed on a stereoscopic monitor and controlled by a virtual user interface. METHODS: For the planning of each case, a 3-D multimodality model was created on the Dextroscope. The 3-D model was transferred to a console in the operating room that was running Dextroscope-compati

Parameter trajectory analysis to identify treatment effects of pharmacological interventions.

The field of medical systems biology aims to advance understanding of molecular mechanisms that drive disease progression and to translate this knowledge into therapies to effectively treat diseases. A challenging task is the investigation of long-term effects of a (pharmacological) treatment, to establish its applicability and to identify potential side effects. We present a new modeling approach, called Analysis of Dynamic Adaptations in Parameter Trajectories (ADAPT), to analyze the long-term effects of a pharmacological intervention. A concept of time-dependent evolution of model parameter

Hand grip strength and 2-minute walk test in chronic graft-versus-host disease assessment: analysis from the Chronic GVHD Consortium.

Hand grip strength (HGS) and the 2-minute walk test (2MWT) have been proposed as elements of chronic graft-versus-host disease (GVHD) assessment in clinical trials. Using all available data (n = 584 enrollment visits, 1689 follow-up visits, total of 2273 visits) from a prospective observational cohort study, we explored the relationship between HGS and 2MWT and patient-reported measures (Lee symptom scale, MOS 36-Item Short-Form Health Survey [SF-36], and Functional Assessment of Cancer Therapy [FACT]-Bone Marrow Transplantation quality of life instruments and Human Activity Profile [HAP]), ch

Assessment of the serotonin pathway as a therapeutic target for pulmonary hypertension.

Blockade of the serotonin reuptake transporter (5-HTT), using fluoxetine, has been identified as a potential therapeutic target for preventing and, importantly, reversing pulmonary hypertension (PH). This study utilized synchrotron radiation microangiography to determine whether fluoxetine could prevent or reverse endothelial dysfunction and vessel rarefaction, which underpin PH. PH was induced by a single injection of monocrotaline (MCT; 60 mg kg(-1)). Following MCT administration, rats received daily injections of either saline or fluoxetine (MCT+Fluox; 10 mg kg(-1)) for three weeks. A third
