Comparison of Transverse Cancellous Lag Screw and Ordinary Cannulated Screw Fixations in Treatment of Vertical Femoral Neck Fractures.

OBJECTIVE: To compare the clinical therapeutic effect of transverse cancellous lag screw (TCLS) fixations and ordinary cannulated screw (OCS) fixations for vertical femoral neck fractures. METHODS: A total of 62 eligible patients with an average age of 56.2 years (range, 19-45 years; 40 male and 22 female) with Pauwels' type III femoral neck fractures were recruited in our study from January 2016 to December 2017. Among the patients, 30 underwent TCLS fixation (TCLS group), and the others were treated with OCS fixation (OCS group). The baseline data, perioperative outcomes (operative time, in…

Comprehensive analysis of variables affecting delayed gastric emptying following pancreaticoduodenectomy.

INTRODUCTION: Delayed gastric emptying (DGE) occurs commonly following pancreaticoduodenectomy (PD), and criteria for its clinical diagnosis have recently been standardized by an international consensus group. We evaluated 24 operative and peri-operative variables to assess which were independent risk factors for DGE development. Secondarily, we also examined DGE incidence over time and evaluated the consensus guidelines' ability to diagnose DGE in patients with complicated post-operative courses. METHODS: A prospective, single-surgeon database of 235 patients undergoing PD at an academic ter

Learning curve for endoscopic posterior cervical foraminotomy.

PURPOSE: While anterior cervical discectomy and fusion as well as cervical disk arthroplasty are gold standard treatments for the surgical treatment of cervical radiculopathy, posterior endoscopic cervical foraminotomy (PECF) as a substitute procedure is gaining popularity. To date, studies investigating the number of surgeries needed to achieve proficiency in this procedure are lacking. The purpose of the study is to examine the learning curve for PECF. METHODS: The learning curve in operative time for two fellowship-trained spine surgeons at independent institutions was retrospectively asse…

Association Between Facial Nerve Monitoring With Postoperative Facial Paralysis in Parotidectomy.

IMPORTANCE: Electrophysiologic facial nerve monitoring is becoming an established intraoperative aid to assist the surgeon in facial nerve t … partial parotidectomy (PP) (n = 222) and total parotidectomy (TP) (n = 45) groups. An evaluation of operative time was performed to test the hypothesis of shorter duration of surgery with facial nerve monitoring. Final follow-up was completed on December 31, 2008, and data were analyzed from June 1 to December 31, 2013. MAIN OUTCOMES AND MEASURES: Comparison of …

Access to the hypertrophic pylorus: does it make a difference to the patient?

BACKGROUND: The aim of the study was to evaluate the effects of different access methods for the treatment of pyloric stenosis (PS). METHODS: Since 2001, we have operated on children with PS using three different access methods: classic right upper quadrant transverse incision (TI), incision on the superior umbilical fold (UI) and laparoscopic (L). We reviewed the records of these children with special emphasis on the number and characteristics of complications, operative time, and length of stay (LOS). RESULTS: We identified 256 patients (212 M, 44 F) with a mean age of 36 days. 138 procedu…

MCL pie crusting for concomitant medial meniscal surgery does not appear to adversely influence primary ACL reconstruction functional outcomes.

OBJECTIVES: Medial collateral ligament "pie-crusting" (MCLPC), selective release of the superficial MCL, has been shown to improve the arthroscopic access to the posterior horn of the medial meniscus without adversely affecting the outcomes of isolated meniscal surgery. However, whether MCL PC, to address concomitant meniscal lesions during anterior cruciate ligament reconstruction (ACLR) surgery, adversely affects ACLR outcomes is unknown. The aim of this study was to assess whether patients who had undergone MCLPC at the time of ACLR had similar post outcomes to patients undergoing isolated

Low Mean Arterial Pressure During Cardiopulmonary Bypass and the Risk of Acute Kidney Injury: A Propensity Score Matched Observational Study.

INTRODUCTION: Low mean arterial pressure (MAP) periods occur frequently during cardiopulmonary bypass (CPB), and their management remains controversial. Our aim was to correlate MAP during CPB with the occurrence of post-operative acute kidney injury (AKI), considering two different parameters: consecutive and cumulative low MAP periods. METHODS: Single-centre observational retrospective study including 250 patients submitted to non-emergent aortic valve replacement, with tepid to mild hypothermia (not below 32°C). The primary outcome was the occurrence of AKI. A propensity scored matching of

Operative Outcomes of Grade 3 Turf Toe Injuries in Competitive Football Players.

BACKGROUND: Turf toe is a term used to describe a hyperextension injury to the first metatarsophalangeal joint. Although the vast majority of turf toe injuries can be treated successfully without operative intervention, there are instances where surgery is required to allow the athlete to return to play. Although there is a plethora of literature on turf toe injuries and nonoperative management, there are currently few reports on operative outcomes in athletes. METHODS: We obtained all cases of turf toe repair according to the ICD-10 procedural code. The inclusion criteria included: age great

Peri-operative myocardial injury in patients undergoing surgery for critical limb ischaemia.

INTRODUCTION: Although up to a half of patients undergoing abdominal aortic aneurysm (AAA) repair suffer myocardial injury, as indicated by a rise in cardiac troponin I (cTnI), this is infrequently accompanied by a rise in creatine kinase (CK)-MB fraction or electrocardiogram (ECG) changes. This study compares for the first time peri-operative cTnI, CK-MB and ECG changes in patients undergoing surgery for critical lower limb ischaemia (CLI). METHODS: Twenty-nine patients (20 men, median age 75 [range, 57-95] years) were studied prospectively. cTnI, CK/CK-MB ratio and ECG were performed pre-op

Intra-ureteric indocyanine green for ureteric visualisation in complex benign gynaecological surgery: a prospective series of 50 cases.

Ureteric injury is a serious complication of gynaecological surgery, particularly in cases involving deep endometriosis. This prospective cohort study assessed the feasibility and safety of intra-ureteric indocyanine green (ICG) for real-time ureteric visualisation in complex endometriosis surgery at a tertiary referral unit. Fifty consecutive patients undergoing laparoscopic procedures received cystoscopy-guided bilateral intra-ureteric ICG. Mean additional operative time was 160 seconds. Bilateral ureteric fluorescence was achieved in all cases and persisted throughout surgery. Guidewires an…

[Fractures of the hand skeleton].

The complex anatomical conditions of the hand pose special problems for fracture treatment. On the one hand length and axes as well as rotation of the fractured bones have to be reestablished in order to prevent functional impairment. On the other hand operative possibilities are limited by the overlying vessels, nerves and tendons. Certain types of fractures are successfully treated by conservative means. Disadvantages are the long time of immobilisation and sometimes clumsy bandages. Other types of fractures require operative treatment in order to accomplish correct reduction. In addition st

[Profound revascularization: a forgotten alternative for multiple vessel reconstruction. A case study].

The profundaplasty represents with the lengthy conical widening of the deep femoral artery in combination with a lumbar sympathectomy L3-L5 a time-saving and effective alternative to multiple and extensive arterial reconstructions in the above and below knee level. An important prerequisite for this method is the exclusion of arteriosclerotic lesions at the aorto-iliac level. Critical selection of the patients and optimal operative technique of the profundaplasty offers results better or at least similar to that of time consuming femoropopliteal resp. cruro-pedal reconstruction.
