Prism adaptation alters spatial remapping in healthy individuals: evidence from double-step saccades.

The visual system is able to represent and integrate large amounts of information as we move our gaze across a scene. This process, called spatial remapping, enables the construction of a stable representation of our visual environment despite constantly changing retinal images. Converging evidence implicates the parietal lobes in this process, with the right hemisphere having a dominant role. Indeed, lesions to the right parietal lobe (e.g., leading to hemispatial neglect) frequently result in deficits in spatial remapping. Research has demonstrated that recalibrating visual, proprioceptive a

Regulation of the Na,K-ATPase gamma-subunit FXYD2 by Runx1 and Ret signaling in normal and injured non-peptidergic nociceptive sensory neurons.

Dorsal root ganglia (DRGs) contain the cell bodies of sensory neurons which relay nociceptive, thermoceptive, mechanoceptive and proprioceptive information from peripheral tissues toward the central nervous system. These neurons establish constant communication with their targets which insures correct maturation and functioning of the somato-sensory nervous system. Interfering with this two-way communication leads to cellular, electrophysiological and molecular modifications that can eventually cause neuropathic conditions. In this study we reveal that FXYD2, which encodes the gamma-subunit of

Mitochondrial involvement in sensory neuronal cell death and survival.

Peripheral nerve injuries (PNI) are continuing to be an ever-growing socio-economic burden affecting mainly the young working population and the current clinical treatments to PNI provide a poor clinical outcome involving significant loss of sensation. Thus, our understanding of the underlying factors responsible for the extensive loss of the sensory cutaneous subpopulation in the dorsal root ganglia (DRG) that occurs following injury needs to be improved. The current investigations focus in identifying visual cues of mitochondria-related apoptotic events in the various subpopulations of senso

Orthodontic considerations in restorative management of hypodontia patients with endosseous implants.

The use of implant-supported restorations in patients with hypodontia remains challenging and requires a multistage treatment that begins in late mixed dentition and continues into late adolescence. The aim of this article is to review the role of orthodontics in endosseous implant rehabilitation of patients with hypodontia. The MEDLINE, Web of Science, Scopus, Cochrane databases, and necessary scientific textbooks were searched for relevant studies and reviews, and as far as possible, they were only included if they had been cited at least once in the literature. Dental implants are susceptib

Incorporating feedback from multiple sensory modalities enhances brain-machine interface control.

The brain typically uses a rich supply of feedback from multiple sensory modalities to control movement in healthy individuals. In many individuals, these afferent pathways, as well as their efferent counterparts, are compromised by disease or injury resulting in significant impairments and reduced quality of life. Brain-machine interfaces (BMIs) offer the promise of recovered functionality to these individuals by allowing them to control a device using their thoughts. Most current BMI implementations use visual feedback for closed-loop control; however, it has been suggested that the inclusio

Dissociation apraxia secondary to right premotor stroke.

Apraxia is usually associated with damage to the dominant parietal cortex, but several other areas, including the left dorsolateral frontal cortex and subcortical structures, have also been implicated, suggesting that the praxis system is mediated by an anatomically distributed network. The right premotor frontal cortex is commonly assumed to play a secondary role in this system. We describe a patient who developed left upper limb apraxia after a right premotor ischaemic stroke. The neuropsychological examination ruled out impairment of other cognitive functions, including those subserved by t

Supernumerary phantom limbs in spinal cord injury.

STUDY DESIGN AND OBJECTIVES: Case report and review of supernumerary phantom limbs in patients suffering from spinal cord injury (SCI). SETTING: SCI rehabilitation centre. CASE REPORT: After a ski accident, a 71-year-old man suffered an incomplete SCI (level C3; AIS C, central cord syndrome), with a C3/C4 dislocation fracture. From the first week after injury, he experienced a phantom duplication of both upper limbs that lasted for 7 months. The supernumerary limbs were only occasionally related to painful sensation, specifically when they were perceived as crossed on his trunk. Although the

West Nile virus infection in a green-winged macaw (Ara chloropterus).

A 29-year-old female green-winged macaw (Ara chloropterus) was presented for acute pelvic limb lameness. On physical examination the bird was found to be mildly dehydrated with paraparesis and proprioceptive deficits of the pelvic limbs. Antemortem diagnostics included a complete blood count and plasma biochemistry panel, fecal Gram and Ziehl-Neelsen acid fast stains, plasma heavy metal concentrations, reverse transcriptase (RT)-PCR testing for West Nile virus (WNV) RNA and PCR testing for Chlamydophila psittaci DNA from choanal/cloacal swabs, and survey radiography. Non-motor symptoms may herald Parkinson's disease.

Parkinson's disease is the second most common neurodegenerative disorder, after Alzheimer's disease. It predominantly affects the elderly. Age is the most clearly established risk factor and there is a male:female ratio of 1.5:1. Current incidence in the general population is 8.4-19 per 100,000 population per year with an approximate prevalence of 120 per 100,000 population. NICE recommends that patients with suspected Parkinson's disease should be referred untreated to a specialist with expertise in parkinsonian disorders. The diagnosis is primarily clinical. Parkinson's disease should be sus

Using rolling to develop neuromuscular control and coordination of the core and extremities of athletes.

Rolling is a movement pattern seldom used by physical therapists for assessment and intervention with adult clientele with normal neurologic function. Rolling, as an adult motor skill, combines the use of the upper extremities, core, and lower extremities in a coordinated manner to move from one posture to another. Rolling is accomplished from prone to supine and supine to prone, although the method by which it is performed varies among adults. Assessment of rolling for both the ability to complete the task and bilateral symmetry may be beneficial for use with athletes who perform rotationally

The effects of PNF training on the facial profile.

Although orthodontic treatment improves dentoalveolar problems, the facial profile seldom changes because the perioral muscles do not easily adapt to the new morphological circumstances. We employed proprioceptive neuromuscular facilitation (PNF), which is training with added resisted movement to motions such as lifting the upper lip, lowering the lower lip, and sticking out the tongue, to adapt the perioral muscles to the new morphological circumstances. The subjects were 40 adults with an average age of 29.6 years. A series of PNF exercises was performed three times per day for 1 month. Late

Frontalis suspension with fascia lata for severe congenital blepharoptosis using enhanced involuntary reflex contraction of the frontalis muscle.

Increased stretching of the mechanoreceptors in Mueller's muscle induces involuntary reflex contraction of the ipsilateral frontalis muscle. We used this neurophysiological mechanism during surgery on 50 patients with severe unilateral or bilateral congenital blepharoptosis that does not have the levator muscle but has Mueller's muscle. To facilitate contraction of the superior rectus muscle to sufficiently stretch the mechanoreceptors in Mueller's muscle, transverse fibrous tissues that restrict the stretching are medially and laterally incised. Without fascial or muscular advancement, the fa

Stance ataxia and delayed leg muscle responses to postural perturbations in cervical spondylotic myelopathy.

OBJECTIVE: We studied balance control in patients with cervical spondylosis, on the hypothesis that ataxia and changes in postural responses occur concurrently as a consequence of structural problems in the cervical cord. SUBJECTS AND PATIENTS: Seventeen patients and 17 healthy subjects were recruited. Based on magnetic resonance imaging, the patients were divided into 2 groups, with (n=9) and without (n=8) signs of myelopathy. METHODS: Body sway was recorded under quiet stance on a force platform. Postural perturbations evoked early and late responses in soleus and tibialis anterior. RESUL

Abnormal spinal interactions from hand afferents to forearm muscles in writer's cramp.

OBJECTIVE: Spinal reflexes from hand to wrist muscles were investigated in writer's cramp. METHODS: Stimulus-triggered rectified EMG averages after ulnar nerve and cutaneous stimulation, in wrist flexors and extensors during tonic contraction, were compared in 18 controls and 19 patients. RESULTS: On the patient dystonic side, ulnar-induced EMG suppression was decreased in wrist extensors, and facilitation in wrist flexors modified dependent on the dystonic wrist posture during writing. No change was found on the patient non-dystonic side. Cutaneous stimulation increased wrist flexor EMG on

Acute ankle sprain: an update.

Acute ankle injury, a common musculoskeletal injury, can cause ankle sprains. Some evidence suggests that previous injuries or limited joint flexibility may contribute to ankle sprains. The initial assessment of an acute ankle injury should include questions about the timing and mechanism of the injury. The Ottawa Ankle and Foot Rules provide clinical guidelines for excluding a fracture in adults and children and determining if radiography is indicated at the time of injury. Reexamination three to five days after injury, when pain and swelling have improved, may help with the diagnosis. Therap

Pressure between the humeral head and the subscapularis tendon after the modified Boytchev procedure.

The pressure between the humeral head and the subscapularis tendon was determined in 32 patients who had recurrent anterior shoulder dislocations. These patients' pressures were measured during a modified Boytchev procedure both before and after transposition of the conjoined tendon, and they were remeasured at the time of screw removal, performed at a mean of 13 months after the initial surgery. The mean clinical follow-up period was 31 months. A micro-tip catheter transducer was inserted into the glenohumeral joint between the humeral head and the subscapularis tendon. Pressures were measure

Impairment of context-adapted movement selection in a primate model of presymptomatic Parkinson's disease.

The MPTP model allows the presymptomatic stage of parkinsonism to be studied in primates and hence specific behavioural manifestations of moderate nigrostriatal denervation to be identified. On the basis of the physiological literature, we hypothesized that depletion of striatal dopamine could impair the selection of context-relevant habits. To examine this hypothesis, we trained three African green monkeys to perform a simple reach-and-grasp task, including three contexts differing only in terms of the presence and position of transparent obstacles. At the end of training, the analysis of rea

Polyradiculopathy. A rare complication of neurobrucellosis.

Neurobrucellosis is chronic brucellosis affecting the nervous system. It may mimic many neurological diseases but it rarely presents as polyradiculopathy. Brucellar radiculopathy was diagnosed in 6 patients who presented with weakness of the lower extremities. Five patients had lumbar puncture, 4 had magnetic resonance imaging of lumbar spine and 4 had nerve conduction studies. Five patients had areflexia and weakness; one had areflexia with proprioceptive ataxia. All patients had positive Brucella serology; cerebrospinal fluid showed lymphocytic pleocytosis, elevated protein, normal-low gluco

Gait disorders.

BACKGROUND: Disorders of gait and mobility are common neurologic abnormalities of aging and can have disastrous consequences. Etiology is multifactorial, and normal age-related changes are difficult to distinguish from those resulting from the disease. REVIEW SUMMARY: Gait evaluation with special techniques provides scientific understanding of physiologic mechanisms, but experienced clinicians can make diagnoses based on general medical and neurologic history and physical examination. Details to observe include posture, length of stride, width of base, speed and fluidity of motion, arm swing,
