Safety of a refrigerator-stable varicella vaccine (VARIVAX) in healthy 12- to 15-month-old children: A randomized, double-blind, cross-over study.

The safety of a single injection of the refrigerator-stable formulation of varicella vaccine VARIVAX was assessed in a blind, randomized, cross-over trial. Five hundred seven healthy children aged 12 to 15 months received subcutaneous injections of VARIVAX on day 0 and the measles, mumps and rubella vaccine (M-M-R II) on day 42 or M-M-R II on day 0 and VARIVAX on day 42. To maintain blinding, injections were given by a study nurse not involved in safety assessments. M-M-R II acted as a reference to validate the safety assessment, as its safety profile is well known in this age range. Parents o

[Routine immunizations as a tool to reduce inequalities in health].

The effectiveness and safety of immunization against the varicella-zoster virus (vzv) has been established after decades of use in the Far East and the U.S. It has been licensed in Israel since 1999. Nevertheless, it has not yet been included in the routine immunizations provided free through Maternal and Child Health ("Tipat Halav") clinics. This is one of the causes of increasing inequalities in health since immunization against vzv is only available via a large co-payment. The Ministry of Health has decided to include immunization against vzv in its routine immunization program for children

On the viral hypothesis of multiple sclerosis: participation of varicella-zoster virus.

Recent studies, including our own, have accumulated evidence suggesting the etiological participation of varicella-zoster virus in multiple sclerosis. If confirmed, complex issues of individual susceptibility and immunopathogenesis would have to be unveiled.

Loss of antibodies to measles and varicella following solid organ transplantation in children.

The objective of this study was to determine if loss of antibodies to measles and to varicella occurs in pediatric solid organ transplant recipients. Antibodies were measured in eight hepatic, four cardiac, and six renal transplant patients who had antibodies to measles prior to transplantation, and in six hepatic, five cardiac, and seven renal transplant patients who had antibodies to varicella prior to transplantation. Within 6 months of the transplant, loss of antibodies occurred in four of 18 patients who were seropositive for measles prior to transplantation (22.2%) and in two of 18 patie

Herpesviridae infections in newborns: varicella zoster virus, herpes simplex virus, and cytomegalovirus.

Varicella zoster virus (VZV), herpes simplex virus (HSV) and cytomegalovirus (CMV) are all members of the Herpesviridae family.Humans are the only source of infection for these double stranded DNA viruses. Infants may acquire these infections in utero, peripartum, or postnatally, resulting in a variety of clinical syndromes, ranging from asymptomatic infection to severe infection, with high mortality rates and significant long-term morbidity. This article presents the epidemiology, clinical characteristics, treatment, and prevention strategies for VZV, HSV, and CMV infections in infants.

Profiles in patient safety: authority gradients in medical error.

The term "authority gradient" was first defined in aviation when it was noted that pilots and copilots may not communicate effectively in stressful situations if there is a significant difference in their experience, perceived expertise, or authority. A number of unintentional aviation, aerospace, and industrial incidents have been attributed, in part, to authority gradients. The concept of authority gradient was introduced to medicine in the Institute of Medicine report To Err Is Human, yet little has been written or acknowledged in the medical literature regarding its role in medical error.

Varicella complicated by group A streptococcal facial cellulitis.

An increase has been recently noted in the incidence of life-threatening group A beta-hemolytic streptococcal (GABHS) infections in children recovering from varicella. We report our experience with a patient who required pediatric intensive care unit admission because of a serious GABHS infection 1 week after the onset of varicella. Emergency physicians must look for this complication in patients with varicella remaining abnormally febrile and presenting unusual manifestations.

Linear IgA bullous dermatosis associated with vancomycin and disseminated varicella-zoster infection.

Linear IgA bullous dermatosis (LABD) is characterized by linear deposits of IgA at the basement membrane zone. Most cases are idiopathic, but medications, infections, autoimmune disorders, and malignancies have been documented as potential inducers. We report a case where both vancomycin and varicella-zoster infection were present as triggers.

Cardiac complications in children following infection with varicella zoster virus.

Infection with varicella zoster virus, leading to chicken pox in susceptible hosts, is usually a benign self-limiting disease conferring immunity in those affected. Cardiac complications are rare, but when present may lead to severe morbidity or mortality. We have recently encountered three children, all of whom developed significant cardiac complications secondary to infection with varicella. Myocarditis has long been associated with such infection. The pathological mechanism is presumed similar to other cardiotropic viruses, where both direct cytopathic and secondary auto-immune effects cont

Live varicella vaccine in both immunocompromised and healthy children.

BACKGROUND: There is no information on the use of live varicella vaccine in Mexican children. Our objective was to evaluate antibody response and safety of the live varicella vaccine in both healthy and immunocompromised Mexican children. METHODS: One hundred children with no history of varicella/zoster were vaccinated with a live attenuated varicella vaccine. According to their immune status, patients were divided into either a compromised (leukemia, solid tumors, chronic renal failure, and cirrhosis) or a healthy children group. Serum IgG antibodies against VZV were measured by ELISA at bas

Massive subperiosteal hemorrhage and femoral shaft osteonecrosis: a complication of tissue plasminogen activator therapy for purpura fulminans.

We present the case of a child who developed a massive subperiosteal hemorrhage and subsequent osteonecrosis of her right femur after treatment with tissue plasminogen activator for post-varicella streptococcal purpura fulminans. Radiographs showed posteromedial translation of the capital femoral epiphysis on the necrotic shaft, and the hip was immobilized. The femur slowly remodeled and has continued to grow. The child is independently ambulatory with a 2.1-cm leg length discrepancy, a varus deformity of the hip, and a valgus distal femur.

Varicella infection and toxoplasmosis in pregnancy.

Varicella occurring in pregnancy can be dangerous for the fetus, the mother, and the newborn. The fetus may experience multiple system damage. The mother and newborn are at increased risk for varicella pneumonia with a 9% and 20% fatality rate, respectively. The recent introduction of the varicella vaccine will affect the occurrence of gestational infection. Toxoplasmosis is rarely dangerous for the pregnant woman, yet the fetus and newborn may be at risk for chorioretinitis, hydrocephalus, intracranial calcifications, and convulsions. The greatest challenge in the management of toxoplasmosis

Serratia marcescens bacteremia associated with schistosomiasis mansoni.

The case of a 21-year-old man coming from rural Paraíba, northwestern Brazil, with schistosomiasis mansoni associated with Serratia marcescens bacteremia, is reported. His main complaints on admission were fever, diaphoresis and chills for ten days, and diarrhoea that lasted for four days. On physical examination he had jaundice and hepatosplenomegaly. Diagnosis of S. marcescens bacteremia was made by isolation of the bacterium in blood culture, and schistosomiasis was diagnosed by rectal and liver biopsies. This is the first time that the association of S. marcescens bacteremia and schistosom

[9 cases of encephalitis in an epidemic outbreak of exanthematous diseases].

We report here nine cases of encephalitis secondary to exanthematous diseases diagnosed from March to June 1990 at the University Hospital in Monterrey, Mexico. Two of the cases were secondary to measles, three to rubella, and four to varicella. One patient with varicella died, and the histopathological study showed findings compatible with viral encephalitis. The sex distribution was four males and five females with ages ranging from two to fourteen years. The most common clinical manifestations were hemiparesia, intracranial hypertension, meningism and altered consciousness. The CSF examinat

Varicella-zoster antibody titers in children with leukemia and lymphoma. Relationship of titer to varicella-zoster infection.

Serum varicella-zoster (VZ) antibody titers were determined by a simple, indirect immunofluorescent antibody technique in 50 children with leukemia or non-Hodgkin's lymphoma. Sixteen children had initial antibody titers that were not detectable at a serum dilution of 1:8 and were considered to be susceptible to varicella. Thirty-one patients had initial serum VZ antibody titers of 1:16 or greater, while three had levels of 1:8. The serum antibody titer was 1:8 or greater in 16 children with a history of varicella. Seven episodes of localized herpes zoster were observed in children whose VZ tit
