Squamous cell cancer: a practical approach.

Cutaneous squamous cell carcinoma (SCC) is the second most common form of skin cancer and is treated frequently by dermatologists. For many years, the level of knowledge regarding treatment of SCC has taken a backseat to the treatment of basal cell carcinoma and malignant melanoma. As dermatologists become more surgically proficient and assume a leading role in the surgical care for cutaneous carcinoma, a thorough knowledge of the appropriate management of SCC is of paramount importance. In particular, it is essential to recognize that, unlike basal cell carcinoma, certain SCC have a significa

Transscleral Nd: YAG laser cyclophotocoagulation versus cyclocryotherapy.

The post-operative course of intraocular pressure (IOP) after transscleral Nd: YAG laser cyclophotocoagulation (TSYLC) in refractory glaucoma was studied using the life-table method and retrospectively compared with that of cyclocryotherapy. In all, 36 eyes of 36 patients who underwent TSYLC for the first time and 80 eyes of 75 patients who underwent cyclocryotherapy for the first time were included in the present study. The follow-up averaged 9.2 months (range, 1-21 months) in patients receiving cyclocryotherapy. In patients with a preoperative visual acuity of greater than or equal to 0.01,

Retinal manifestations in fibromuscular dysplasia.

Fibromuscular dysplasia of the arteries (FMD) is a segmental angiopathy which may produce obstruction of the carotid, cerebral, renal, mesenteric, coronary or iliac arteries. Except for lesions related to arterial hypertension, retinal manifestations have not yet been reported. This paper describes the case of a 10-year-old boy with progressive deafness, a history of an unexplained stroke and progressive occlusions of the retinal arterioles in the fundus periphery. This resulted in retinal neovascularization and recurrent retinal and vitreous hemorrhages. Despite repeated photo- and cryocoagul

Surgical epicardial ablation of left ventricular pathway using sling exposure.

We report our experience with 43 consecutive patients with left free wall pathways operated on since December 1988 using a modified direct epicardial approach through a medial sternotomy, without the adjunct of normothermic cardiopulmonary bypass. The left atrioventricular sulcus is exposed by dislocating the heart cephalad and to the right using a sling made of a large sponge passed around the ventricle through the transverse sinus. While the arterial pressure is monitored, the heart is positioned to obtain adequate exposure without compromising the ventricular function. The left atrioventric

Pulse field ablation versus thermal ablation: A systematic review and meta analysis of randomized controlled trials.

Radiofrequency (RFA) and cryoballoon (CBA) catheter ablation are the primary thermal ablation techniques used for catheter-based ablation of atrial fibrillation (AF). Recently, pulsed field ablation (PFA), which is a nonthermal ablation technique, has been studied; however, it is unclear if PFA can improve atrial arrhythmia freedom compared to thermal ablation. A meta-analysis of randomized controlled trials comparing PFA to thermal ablation techniques (RFA and/or CBA) was conducted. The primary outcome was atrial arrhythmia freedom. The secondary outcome was a composite of serious adverse eve

Efficacy and safety of pulsed field ablation versus conventional thermal techniques for patients with atrial fibrillation: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Atrial fibrillation (AF) is the most common cardiac arrhythmia and a leading cause of stroke. Pulsed field ablation (PFA) emerges as a novel, non-thermal energy source offering myocardial-selective ablation with a potentially improved safety and efficacy profile. OBJECTIVES: To compare the safety and efficacy of PFA versus RFA/CBA ablation in patients with AF. METHODS: Multiple databases were systematically searched from inception to May 2025. Randomized Controlled Trials (RCTs) comparing PFA versus thermal ablation (TA) were analyzed using risk ratio (RR) and mean difference (MD)

Exploratory phase angle assessment after atrial fibrillation ablation in overweight patients: a pilot study.

BACKGROUND: The phase angle (PhA), derived from bioelectrical impedance analysis (BIA), is an indicator of cellular health and nutritional status. Its role in patients with atrial fibrillation (AF), particularly after catheter ablation, is not well-established. This study aimed to investigate changes in PhA and their correlation with cardiac remodeling in overweight patients with persistent AF after cryoballoon ablation. METHODS: Thirteen overweight patients (BMI ≥ 23 kg/m²) with persistent AF scheduled for ablation were prospectively enrolled. One patient was excluded due to sick sinus syndro

Management of Vasoproliferative Tumor in a Patient With Coats' Disease: Multiple Therapeutic Approach in Current Era.

This case report describes a vasoproliferative tumor in a 16-year-old male patient with Coats' disease, treated with laser photocoagulation, cryoablation, and anti-vascular endothelial growth factor injections therapies, that developed an epiretinal membrane as a complication. The surgical treatment in association with the retinal laser treatment greatly improved visual acuity. At 8 months follow-up from surgery, visual acuity had significantly improved, and macular anatomy recovered. Vasoproliferative tumors may occur in patients with Coats' disease, and in such a clinical scenario, a multipl

Efficacy and safety in synchronous core-needle biopsy and cryoablation for highly suspicious malignant pulmonary nodule.

BACKGROUND: Percutaneous computed tomography (CT)-guided biopsy and cryoablation are commonly used techniques for diagnosing and treating pulmonary malignant tumors. Performing these procedures simultaneously allows for tissue diagnosis while potentially offering therapeutic benefits. This study aimed to evaluate whether the efficacy and safety of simultaneous percutaneous CT-guided biopsy and cryoablation in managing pulmonary tumors suspected of malignancy are comparable to those of sequential procedures. METHODS: This retrospective study involved 124 patients with 131 highly suspicious mal

Magnetic Resonance Imaging-Guided Cryoablation of Prostate Cancer Lymph Node Metastasis.

PURPOSE: To evaluate the safety and effectiveness of magnetic resonance (MR) imaging-guided cryoablation of prostate cancer metastatic lymph nodes (LNs). MATERIALS AND METHODS: Fifty-two patients with prostate cancer who underwent MR imaging-guided LN ablation from September 2013 to June 2022 were retrospectively reviewed. Of these, 6 patients were excluded because adequate ablation margins (3-5 mm) could not be achieved secondary to adjacent structures. The remaining 46 patients (mean age, 70 years [SD ± 7]) underwent 55 MR imaging-guided cryoablation procedures of metastatic LNs (25 in the

Thirty-day mortality risk in patients following radiofrequency and cryoballoon ablation for atrial fibrillation across the entire nation of Poland: An 8-year analysis from the National Health Fund of Poland.

INTRODUCTION: Atrial fibrillation (AF) is the most common sustained arrhythmia, and catheter ablation (CA) is a primary therapeutic option. However, the 30-day all-cause mortality risk associated with CA for AF may be underestimated due to selection bias. Our study aimed to assess 30-day mortality in an unselected cohort of patients. METHODS: Data from the National Health Fund-Poland covered over 99% of nationwide radiofrequency ablation (RF) or cryoballoon ablation for AF, with 100% recorded deaths. The study included consecutive CA procedures in adult patients between 2012 and 2019. Thirty-

Cryoablation combined with transarterial infusion of pembrolizumab (CATAP) for liver metastases of melanoma: an ambispective, proof-of-concept cohort study.

BACKGROUND: The presence of liver metastasis correlates with poor therapeutic response of PD-1 blockade therapy in melanoma. A novel treatment protocol by combining cryoablation with transarterial infusion of pembrolizumab (CATAP) was proposed, and its feasibility and safety was assessed among this group of patients. METHODS: This registered ambispective cohort study enrolled fifteen melanoma patients with multiple hepatic metastases who received planned two-stage CATAP therapy: in the combined stage, subtotal cryoablation on day 1, in which one to two intrahepatic lesions were ablated comple

Cryoballoon ablation for persistent atrial fibrillation in patients without left atrial fibrosis.

INTRODUCTION: The role of cryoballoon (CB) pulmonary vein isolation (PVI) for patients with persistent atrial fibrillation (AF) is controversial, since long-term success can be poor. We performed left atrial voltage mapping before CB PVI and determined AF-free survival depending on the extent of low-voltage areas (LVAs). METHODS AND RESULTS: We consecutively enrolled 60 patients with persistent AF (average age, 60.6 ± 12.9 years; CHA2 DS 2 VASc score, 2.3 ± 1.6; and left atrial size 46.0 ± 5.2 mm) who were planned for CB PVI. Before ablation, we performed left atrial voltage mapping (Abbott E

Cryoballoon pulmonary vein ablation and left atrial appendage closure combined procedure: A long-term follow-up analysis.

BACKGROUND: The combined left atrial appendage closure (LAAC) and cryoenergy pulmonary vein isolation (PVI) procedure has been proven safe and effective in managing stroke in patients with nonvalvular atrial fibrillation (AF), although most data refer to procedures performed using radiofrequency as the main energy source. OBJECTIVE: The purpose of this study was to evaluate long-term follow-up of patients with AF undergoing concomitant LAAC and cryoenergy PVI. METHODS: Patients undergoing LAAC and cryoballoon PVI at our institution were enrolled. At 3, 6, and 24 months from the index procedu

Concomitant pulmonary vein isolation and percutaneous closure of atrial septal defects: A pilot project.

BACKGROUND: Patients with an atrial septal defect (ASD) are at increased risk of developing atrial fibrillation (AF). Currently percutaneous ASD closure is the preferred therapeutic strategy and although pulmonary vein isolation (PVI) for AF is feasible after ASD closure, the transseptal puncture can be technically challenging and probably increases the perioperative risk. A staged approach, with PVI several months before ASD closure, has been recommended for patients already scheduled for closure, but no data are available on combined procedures. PURPOSE: This pilot study evaluates the feasi
