Crystals and arthritis.

Monosodium urate, calcium pyrophosphate dihydrate, and basic calcium phosphate (carbonate-substituted hydroxyapatite and octacalcium phosphate) crystal aggregates are associated with gout, pseudogout, and cartilage degeneration (osteoarthritis, Milwaukee Shoulder/Knee Syndrome), respectively. Hyperuricemia is a frequent but nonspecific and inconstant feature of gout just as an elevated synovial fluid inorganic pyrophosphate level is an inconstant feature of pseudogout. Monosodium urate, calcium pyrophosphate dihydrate, or basic calcium phosphate crystals can cause acute inflammation associated…

Fructose-induced aberration of metabolism in familial gout identified by 31P magnetic resonance spectroscopy.

The hyperuricemia responsible for the development of gouty arthritis results from a wide range of environmental factors and underlying genetically determined aberrations of metabolism. 31P magnetic resonance spectroscopy studies of children with hereditary fructose intolerance revealed a readily detectable rise in phosphomonoesters with a marked fall in inorganic phosphate in their liver in vivo and a rise in serum urate in response to very low doses of oral fructose. Parents and some family members heterozygous for this enzyme deficiency showed a similar pattern when given a substantially lar…

Renal function in children undergoing cardiac operations.

Because we sometimes observed large amounts of uric acid crystals in the urine of infants and children after open-heart operations and since … ndard error of the mean) and 10.4 +/- 1.7 mg/dl (range, 8.0 to 15.5 mg/dl), respectively. Since the hyperuricemia in the cyanotic children could not be related to increased exogenous administration or decreased renal excretion, it is probably caused by increased endogenous production and may be related to the resolution of the cyanotic state.

Pathogenesis of acidosis in hereditary fructose intolerance.

An 18-yr-old man with a classical history of hereditary fructose intolerance (HFI) developed typical biochemical changes following an oral fructose load: fructosemia, hypoglycemia, hypophosphatemia, hyperuricemia, and metabolic acidosis. Hypokalemia (3.1 meq/liter) was also noted. Three aspects of this case expand the published literature on this syndrome: (1) Metabolic acidosis was found to be due to both lactic acidosis and proximal renal tubular acidosis (RTA). We could quantitate the relative contribution of each, and found that urinary bicarbonate loss due to proximal RTA accounted for le…

KLF transcription factors in metabolic kidney disease: from podocyte protection to tubulointerstitial fibrosis.

Metabolic kidney disease comprises renal disorders driven or worsened by systemic metabolic abnormalities, including diabetic kidney disease, obesity-related kidney injury, hyperuricemia-associated nephropathy, and metabolic syndrome-associated chronic kidney disease. Despite different triggers, these disorders share podocyte injury, glomerular endothelial dysfunction, tubular lipotoxicity, mitochondrial impairment, oxidative stress, inflammation, and tubulointerstitial fibrosis, indicating a common program of metabolic stress-induced renal cell reprogramming. Krüppel-like factors (KLFs) are z…

Tumour lysis syndrome in the setting of a high-grade B-cell lymphoma with primary bone marrow localization - a case report.

Tumour lysis syndrome (TLS) is an oncologic emergency usually arising after chemotherapy administration, caused by a massive tumour breakdown and characterized by a specific laboratory and clinical findings. We present a case of TLS, both spontaneous and treatment related, in a 59-year-old woman with high grade B-cell lymphoma without apparent bulky disease on radiologic scans. The patient presented with acute kidney failure with marked hyperuricemia (2764 µmol/L) and extremely high lactate dehydrogenase (LD) activity (13,770 U/L). She was treated with rasburicase, corticosteroid pre-phase the…

Multifunctional cellulose-rich bagasse magnetic biochar capable of efficient elimination and separation of purine compounds in beer: DFT calculation and practical system applications.

Purine compounds in beer significantly increase the risk of hyperuricemia and gout, creating a strong demand for low-purine and healthy beer. A multifunctional cellulose-rich bagasse (cellulose content: 40 %-50 %) magnetic porous carbon (MABB) was constructed by employing alkali-modified bagasse and intercalating with Fe3+ to effectively remove three purine nucleosides (NUCs) in beer, namely, guanosine (GUA), adenosine (ADE), and inosine (INO). Characterization results showed that alkalization increased the specific surface area of MABB and iron oxide acted as an active site to facilitate NUC …

Development of P(NVCL)-based transdermal patches for sinomenine hydrochloride delivery in gouty arthritis.

Gout is a metabolic disease characterized by hyperuricemia and acute arthritis, with global incidence continuing to rise. This study developed a novel transdermal patch utilizing poly(N-vinyl caprolactam) [P(NVCL)] as a carrier matrix for sinomenine hydrochloride. The optimized patch formulation contained 5 % P(NVCL), 1.0 % glycerol, 1.5 % sodium polyacrylate, and 4.0 % sinomenine hydrochloride. A synergistic permeation enhancer system comprising 3 % azone, 6 % borneol, and 3 % menthol significantly improved transdermal absorption efficiency, achieving 248.6 ± 15.7 μg/cm² cumulative penetratio…

Atypical presentation of HNF1β gene mutation mimicking type 2 diabetes: Case report and literature review.

Hepatocyte nuclear factor 1β (HNF1β)-related disease is a rare autosomal dominant genetic disorder. It presents with diverse clinical phenotypes and involves multiple systems. We report a heterozygous c.544C>T mutation in the HNF1β gene identified in three members of a Han Chinese family. All patients presented with renal cysts, renal impairment, hyperuricemia, hypomagnesemia, and hyperparathyroidism. Only the proband exhibited hyperglycemia, pancreatic dysplasia, and genital malformations. The older son presented with asymptomatic elevation of liver enzymes, while the younger son had left ren…

Teicoplanin total trough concentration and adverse effects in patients with hypoalbuminemia.

CONCLUSIONS: Our findings suggest that a TEIC Cmin ≥ 21.1 μg/mL may increase the risk of TEIC-induced hepatotoxicity in patients with hypoalbuminemia. These findings may need to be validated in further studies.

Umbilical Cord Blood Transplantation in Lesch-Nyhan Syndrome: A Case Report and Literature Review.

Lesch-Nyhan syndrome (LNS) is a rare X-linked disorder caused by hypoxanthine phosphoribosyltransferase 1 (HPRT1) gene mutations, leading to HPRT1 deficiency, hyperuricemia, and severe neurological dysfunction, including self-injurious behavior. Hematopoietic stem cell transplantation (HSCT) has been applied to some inborn errors of metabolism (IEM), yet only a few cases with limited success in LNS have been reported. This case was diagnosed with LNS at nine months of age due to developmental delays. He subsequently received 5/6 human leukocyte antigen-matched umbilical cord blood transplantat…
