Successful conversion of percutaneous transhepatic gallbladder drainage to endoscopic ultrasound-guided hepaticogastrostomy combined with antegrade stenting for a malignant distal biliary obstruction due to invasive intraductal papillary mucinous carcinoma: a case report.

BACKGROUND: Endoscopic retrograde cholangiopancreatography is the gold-standard treatment for malignant distal biliary obstruction. However, when endoscopic retrograde cholangiopancreatography cannot be successful, endoscopic ultrasound-guided rendezvous and endoscopic ultrasound-guided antegrade stenting can be performed as salvage. Nevertheless, endoscopic ultrasound-guided hepaticogastrostomy is widely performed in cases of inaccessible papilla due to duodenal obstruction, although only at high-volume centers owing to its technical challenges and the high risk of complications. There are so

Modified Sullivan-type segment III hepatojejunostomy for radiologically presumed unresectable hilar cholangiocarcinoma: a technical case report.

Hilar cholangiocarcinoma may present with advanced biliary obstruction requiring palliation. An 80-year-old frail woman with jaundice and severe pruritus had contrast-enhanced computed tomography findings radiologically consistent with Bismuth-Corlette type IV hilar cholangiocarcinoma. No tissue diagnosis was obtained. Cancer antigen 19-9, carcinoembryonic antigen, alkaline phosphatase, magnetic resonance cholangiopancreatography, endoscopic retrograde cholangiopancreatography, and percutaneous transhepatic cholangiography were unavailable at the treating hospital and were not performed. [A case where phenobarbital was remarkably effective against drug-induced liver injury presenting with prolonged intrahepatic cholestasis caused by Ashwagandha].

This case concerns a 39-year-old male with a history of adjustment disorder, and he had been using Ashwagandha-self-prescribed and self-imported online-for stress relief and vitality enhancement for the past 3 months. He visited the emergency department with symptoms of hyperventilation, and blood tests showed total bilirubin of 2.37mg/dL, ALT of 384U/L, and ALP of 92U/L, indicating hepatocellular-type liver dysfunction, thereby raising suspicion for drug-induced liver injury (DILI). Although ursodeoxycholic acid was initiated after referring to our hospital, his condition showed minimal impro

Distal cholangiocarcinoma arising in a Choi Type I septate common bile duct: a case report.

Septate common bile duct (CBD) is a rare congenital anomaly resulting from incomplete recanalization of the primitive biliary tract. Although often asymptomatic, it predisposes to bile stasis, chronic inflammation, and potential malignant transformation. Distal cholangiocarcinoma arising in association with a septate CBD is exceptionally rare. We report the case of an 84-year-old male presenting with progressive jaundice, right hypochondrial pain, anorexia, and 20 kg weight loss over 2 months. Examination revealed icterus and a palpable non-tender mass. Laboratory studies showed marked hyperbi

[Acute cholangitis due to Candida glabrata and Klebsiella pneumoniae].

Acute cholangitis is a bile duct infection associated with bile duct obstruction. Bile culture is positive in most cases, and the most frequent etiological agent is Escherichia coli. Candida sp acute cholangitis is a rare finding, which is more common in patients with immunosuppression, use of corticosteroids, prolonged antibiotic treatment or surgical procedures of the bile duct. We present the case of a 67-year-old woman with none of the above-mentioned history who consulted for fever, abdominal pain and jaundice. MRI of the abdomen revealed a lithiasic image in the common bile duct with dil

A rare case report of Mirizzi syndrome type III treatment algorithm in situs inversus totalis, large ventricular septal defect and transposition of great arteries in a young diabetic patient.

Situs inversus totalis (SIT) is a rare condition in which cardiac and abdominal organs are inverted from their normal left-sided orientation. Mirizzi syndrome, characterized by the obstruction of the common hepatic duct or the common bile duct by gallstone, is a rare condition. Mirizzi syndrome co-occurrence in SIT patients is rare. Gallbladder in sinistroposition is extremely uncommon in SIT patients. We report a known case of diabetes, ventricular septal defect with transposition of the great arteries in a 32-year-old female who presented with jaundice, cholangitis, chills, and fever that ha

A Case Report of Primary Unresectable Hilar Cholangiocarcinoma Causing Colonic Obstruction Due to Peritoneal Dissemination.

Although surgical resection is the only available treatment to achieve long-term survival in biliary tract cancer, many cases are often identified at an advanced stage at the time of diagnosis. Radiotherapy may be an alternative option to prolong survival in cases with locally advanced unresectable disease. While there are some reports of long-term survival after radiotherapy for unresectable biliary tract cancer, it is rare that clinical symptoms are exhibited by peritoneal dissemination more than 8 years after radiotherapy and that resection can be performed. Our case was a 55-year-old femal

Rare Association of Budd-Chiari Syndrome With Celiac Disease: A Case Report.

Budd-Chiari associated with celiac disease is a rare phenomenon in the medical literature with annual incidence of less than five per million. The majority of the cases are reported from the North African region. Our patient presented in the out-patient department with symptoms of progressive abdominal distension, diffuse abdominal pain and shortness of breath for one year. She was a known case of celiac disease for the last three years. The clinical examination revealed ascites, jaundice, decreased air entry in basal segments bilaterally, and multiple hemangiomas all over the body. Haematolog

Uncommon cause of acute encephalopathy in liver cirrhosis.

A 49-year-old woman with a medical history of alcoholic cirrhosis status post-transjugular intrahepatic portosystemic shunt (post-TIPS) in 2012, and ongoing alcohol abuse, presented to the hospital, with haematuria. CT intravenous pyelogram (IVP) was normal except for 'a large intrahepatic cystic mass adjacent to the TIPS, causing intrahepatic biliary duct dilation'. The patient also presented with acute encephalopathy, jaundice, right upper quadrant abdominal pain and hyperbilirubinaemia (total bilirubin of 8.1 mg/dL with direct bilirubin of 3.0 mg/dL). She remained encephalopathic despite ad

Acute Budd-Chiari syndrome resulting from a pyogenic liver abscess.

Budd-Chiari syndrome is a rare condition resulting from outflow obstruction of the liver. This syndrome due to a pyogenic abscess is rarely documented in the English literature. Here a male patient with acute Budd Chiari syndrome is presented. A 21-year-old male patient was admitted to the hospital because of severe right upper quadrant pain, jaundice, hepatomegaly and fever. The examination of liver by computerized tomography and ultrasound revealed a large lesion 120 x l00 mm in size located in the right lobe of liver, which was compressing the inferior vena cava, the right and middle hepati

Acute acalculous cholecystitis.

Thirty-two patients with acute acalculous cholecystitis are presented. The age of the patients ranged from 1 to 80 years, with an average of 46.3 years. Acute acalculous cholecystitis occurred during the postoperative period in only four patients. Three patients were receiving total parenteral nutrition and 16 patients had one or more associated medical diseases. One patient had acute acalculous cholecystitis due to mechanical obstruction of the cystic duct caused by a diaphragmatic hernia. The most frequent signs and symptoms were right upper quadrant abdominal pain, nausea, vomiting, fever,

The obstructed pancreatico-biliary drainage limb: presentation, management, and outcomes.

INTRODUCTION: Obstruction of the pancreatico-biliary (PB) drainage limb following major PB operations creates unique diagnostic and management dilemmas. We describe the etiology and prevalence, as well as diagnostic and therapeutic approaches for this challenging problem. METHODS: Individuals with PB limb obstruction were identified from a cohort of 477 patients undergoing major PB resections or bypasses for benign and malignant (N = 265) diseases from September 2000 to January 2010. Their presentation, management, short-term outcomes, and survival were analyzed. RESULTS: Thirteen patients d

Primary Extrahepatic Biliary Mucinous Cystic Neoplasm Suspected Preoperatively and Treated by Robotic Pancreaticoduodenectomy: A Case Report.

INTRODUCTION: Biliary mucinous cystic neoplasm (MCN) is a cystic tumor characterized by ovarian-like stroma and is recognized as a neoplasm with the potential for invasive carcinoma. While MCNs predominantly occur in the pancreas, primary occurrence in the extrahepatic bile duct is exceedingly rare. Most reported cases are diagnosed postoperatively due to the lack of specific clinical symptoms, the difficulty in preoperative differentiation from other cystic diseases, such as intraductal papillary neoplasm of the bile duct, and the strict requirement of pathological evaluation for definitive d

Rhabdomyosarcoma of the Biliary Tract in Children: Analysis of Single Center Experience.

Rhabdomyosarcoma (RMS) of the biliary tract is a rare tumor in children, constituting 0.5-0.8% of all pediatric RMS. Still, it is the most common malignancy in this location in children. Due to its rarity and location, it may cause diagnostic and treatment difficulties. Above all, there are no therapeutic guidelines specific for this tumor location. The aim of the study was to present an analysis of our experience with the treatment of children with biliary tract rhabdomyosarcoma (RMS) and discuss clinical recommendations for this specific location published in the literature. A retrospective

Stepwise approach to curative surgery using percutaneous transhepatic cholangiodrainage and portal vein embolization for severe bile duct injury during laparoscopic cholecystectomy: a case report.

Laparoscopic cholecystectomy (LC) has been recently adapted to acute cholecystitis. Major bile duct injury during LC, especially Strasberg-Bismuth classification type E, can be a critical problem sometimes requiring hepatectomy. Safety and definitive treatment without further morbidities, such as posthepatectomy liver failure, is required. Here, we report a case of severe bile duct injury treated with a stepwise approach using (99m)Tc-galactosyl human serum albumin ((99m)Tc-GSA) single-photon emission computed tomography (SPECT)/CT fusion imaging to accurately estimate liver function.A 52-year

Symptomatic outcome after laparoscopic cholecystectomy.

AIMS: Patients with gallstones often present with multiple complaints. We wanted to study the major complaints of our patients undergoing laparoscopic cholecystectomy and the symptomatic relief afforded by the operation. METHODS: We studied 113 patients with symptomatic gallstone disease who underwent laparoscopic cholecystectomy in a single surgical unit. Patients with proven common bile duct stones, obstructive jaundice, cholangitis, present or past associated abdominal pathology or cholecystoenteric fistula were excluded from the study. The mean follow up period was 18 months (range 10-22

Feline gastrointestinal eosinophilic sclerosing fibroplasia associated with Candida albicans.

Feline gastrointestinal eosinophilic sclerosing fibroplasia (FGESF) is a rare and poorly understood disease characterised by the infiltration of eosinophils and the development of fibrous tissue within the gastrointestinal tract of cats. A 2-year-old female neutered Ragdoll was presented for signs consistent with extrahepatic biliary tract obstruction (EHBO), including jaundice, hyporexia and lethargy. Marked progressive hyperbilirubinemia and mild anaemia were also present. Abdominal ultrasonography suggested a duodenal mass and pancreatitis as the cause of EHBO. Cytopathological results from

Review of imaging findings in hepatic veno-occlusive disease.

Hepatic veno-occlusive disease (VOD), also known as sinusoidal obstruction syndrome (SOS), is a potentially life-threatening complication of hematopoietic stem cell transplantation. Patients present with right upper-quadrant abdominal pain, jaundice, weight gain, and conjugated hyperbilirubinemia. Early diagnosis of VOD is essential to promptly initiate defibrotide therapy, which has been demonstrated to enhance survival and achieve complete resolution of disease in some patients. Historically, VOD was diagnosed by the modified Seattle or Baltimore criteria, which are both based on clinical sy

Giant Echinococcosis of the Liver with Suppuration: A Case Report and Review of the Literature.

Purpose: Cystic echinococcosis (CE) is a common, complex parasitic disease that constitutes a major public health concern. CE demonstrates high endemicity in areas where dogs are used for herding or where animal husbandry practices involve close contact with livestock. It can clinically manifest with a variety of signs and symptoms, such as cholangitis, jaundice, pancreatitis, external biliary fistula, inferior vena cava obstruction, portal hypertension, and superinfection. The latter can notably be related to suppuration, either by rupture or bacteremia. The aim of this study is to report our
