[Subacute leukoencephalopathy after radio- and chemotherapy of lymphoblastic lymphoma].

In a patient aged 34 with lymphoblastic lymphoma lymphomatous infiltrations developed in the leptomeninges but were controlled with radiotherapy and chemotherapy. Recurrence of neurological signs after 7 months was recognized as relapse of lymphoma in the central nervous system, but in the brain extensive demyelination and necrotic changes were found. The authors relate the development of leucoencephalopathy to the hypersensitivity of the nervous tissue to chemotherapy after previous radiotherapy.

Clinical spectrum of infections in patients with HTLV-III-associated diseases.

Infectious complications are the cause of death in the overwhelming majority of patients with the acquired immunodeficiency syndrome (AIDS). These infectious complications are outstanding in severity and number. Infecting organisms are mainly those which take advantage of defective T-cell function. Many cause latent infections which can reactivate and cause disease in AIDS patients because of immunosuppression. Serological diagnosis of infections in AIDS patients is complicated because of the defect in B-cell function; rising antibody titers are not observed in most patients with infectious co

Investigation on the role of cell transcriptional factor Sp1 and HIV-1 TAT protein in PML onset or development.

JC virus (JCV) causes progressive multifocal leukoencephalopathy (PML), characterized by multiple areas of demyelination and attendant loss of brain function. PML is often associated with immunodepression and it is significantly frequent in AIDS patients. The viral genome is divided into early and late genes, between which lies a non-coding control region (NCCR) that regulates JCV replication and presents a great genetic variability. The NCCR of JCV archetype (CY strain) is divided into six regions: A-F containing binding sites for cell factors involved in viral transcription. Deletions and en

Detection of JC virus DNA in CSF of pediatric patients with non-PML neurological disorders.

BACKGROUND: JC virus (JCV) is a human polyomavirus classically associated with progressive multifocal leukoencephalopathy (PML). Its role in other central nervous system (CNS) disorders - and the contribution of related polyomaviruses such as BK virus (BKV) and simian virus 40 (SV40) - remains poorly defined in pediatric populations, especially among immunocompromised patients. METHODS: We analysed 64 cerebrospinal fluid (CSF) specimens collected from children evaluated for suspected viral encephalitis or meningoencephalitis between March and October 2024. Broad-range and virus-specific PCR a

Effectiveness of 6-week dosing of Natalizumab versus continued 4-week treatment for Multiple Sclerosis: An observational registry-based study.

BACKGROUND: Multiple sclerosis (MS) impacts quality of life. Every-4-week (Q4W) regimen of natalizumab (NTZ) is approved for relapsing-remitting multiple sclerosis (RRMS) but increases progressive multifocal leukoencephalopathy (PML) risk. This study aimed to assess the impact of switching to an every-6-week (Q6W) dosing of NTZ in RRMS patients previously treated with Q4W dosing for ≥12 months. METHODS: This observational, registry-based, two-armed, retrospective comparative study included 243 patients (69 males and 174 females; mean age=35.7 ± 8.8 years) from the Kuwait National Registry bet

Cladribine tablets after treatment with natalizumab (CLADRINA) - rationale and design.

BACKGROUND: Individual disease modifying therapies approved for multiple sclerosis (MS) have limited effectiveness and potentially serious side effects, especially when administered over long periods. Sequential combination therapy is a plausible alternative approach. Natalizumab is a monoclonal therapeutic antibody that reduces leukocyte access to the central nervous system that is associated with an increased risk of progressive multifocal leukoencephalopathy and disease reactivation after its discontinuation. Cladribine tablets act as a synthetic adenosine analog, disrupting DNA synthesis a

Hematopoietic Stem Cell Transplantation in ARPC1B Deficiency.

Mutations in the ARPC1B isoform component of human actin-related protein 2/3 complex have been recently associated with an inborn error of immunity characterized by combined immunodeficiency, allergies, autoinflammation, and platelet abnormalities. Currently, indications on the management of this novel disease and information on its outcome are lacking. We report the first case series of 7 children with a homozygous mutation in ARPC1B gene who underwent allogeneic-HSCT (allo-HSCT). All patients presented an early clinical onset, characterized by recurrent infections, failure to thrive and gast

The oncogenic roles of JC polyomavirus in cancer.

JC polyomavirus (JCPyV) belongs to the human polyomavirus family. Based on alternative splicing, the early region encodes the large and small T antigens, while the late region encodes the capsid structural proteins (VP1, VP2, and VP3) and the agnoprotein. The regulatory transcription factors for JCPyV include Sp1, TCF-4, DDX1, YB-1, LCP-1, Purα, GF-1, and NF-1. JCPyV enters tonsillar tissue through the intake of raw sewage, inhalation of air droplets, or parent-to-child transmission. It persists quiescently in lymphoid and renal tissues during latency. Both TGF-β1 and TNF-α stimulates JCPyV mu

No evidence for loss of natalizumab effectiveness with every-6-week dosing: a propensity score-matched comparison with every-4-week dosing in patients enrolled in the Tysabri Observational Program (TOP).

BACKGROUND: Extended interval dosing of natalizumab is associated with significantly lower progressive multifocal leukoencephalopathy risk compared with every-4-week (Q4W) dosing in patients with relapsing-remitting multiple sclerosis. Previous studies have suggested that natalizumab effectiveness is maintained in patients who switch from Q4W to extended interval dosing but have been limited by a lack of well-matched patient cohorts. METHODS: Tysabri Observational Program (TOP) data as of November 2019 were used to identify patients with relapsing-remitting multiple sclerosis treated with nat
