Willingness to pay for prevention and treatment of lymphatic filariasis in Leogane, Haiti.

BACKGROUND: Lymphatic filariasis (LF) affects more than 120 million people worldwide. Efforts to eliminate this disease require sustained community participation. This study explores community valuation of LF elimination efforts by estimating household and community willingness to pay (WTP) for the prevention of transmission and treatment of filarial lymphedema in the community of Leogane, Haiti. METHODS: A contingent valuation survey was used to assess individual WTP for specific prevention and treatment interventions. A 2-dimensional Monte Carlo simulation was developed to estimate confidenc

Modelling exposure opportunities: estimating relative risk for motor neurone disease in Finland.

This paper addresses the issues surrounding an individual's exposure to potential environmental risk factors, which can be implicated in the aetiology of a disease. We hope to further elucidate the 'lag' or latency period between the initial exposure to potential pathogens and the physical emergence of the disease, with specific reference to the rare neurological condition, motor neurone disease (MND), using a dataset obtained from the Finnish Death Certificate registry, for MND deaths between the period 1985-1995. A space-time approach is adopted, whereby patterns in both time and space are c

The Genomic Equity Accessibility Framework (GEAF): A dimensionally consistent framework for modeling affordability and equity in genomic and regenerative therapies.

BACKGROUND: Advances in CRISPR-based gene editing and stem cell-derived embryo models have created substantial biomedical opportunity alongside significant ethical and distributive concerns. Personalized genomic and regenerative therapies approved in the United States as of this writing carry list prices spanning approximately $2.2 million (Casgevy) to $4.25 million (Lenmeldy), with several other approved products priced between these figures (e.g., Lyfgenia, $3.1 million; Hemgenix, $3.5 million). This raises the question of how affordability, accessibility, ethical compliance, and distributio

Dosing Regimen Optimization of Aztreonam/Avibactam According to Renal Function Stratification: A Population Pharmacokinetic-Guided Simulation Study.

Background/Objectives: Aztreonam/avibactam is a promising treatment option for serious infections caused by metallo-β-lactamase-producing carbapenem-resistant Enterobacterales (MBL-CRE). However, the labeled regimen is operationally demanding because it requires frequent, prolonged infusions, and the recommended loading dose does not match the commercially available vial strength. This population pharmacokinetic (PopPK)-based Monte Carlo simulation study aimed to optimize aztreonam/avibactam dosing across renal function strata while maintaining pharmacokinetic/pharmacodynamic (PK/PD) target at

Comprehensive validation of the rapid tick exposure test (RaTexT®): accuracy, reproducibility and comparative performance to detect acaricide resistance in livestock ticks.

BACKGROUND: RaTexT®, an innovative rapid tick exposure test, was recently developed to provide farmers with quick, on-site results to improve their acaricide resistance management. This was achieved by exposing partially engorged adult ticks to a specially designed acaricide-impregnated matrix fitted inside a transparent polypropylene box. Each RaTexT® box contains six strips of four small, interconnected compartments, in which ticks are exposed immediately after removal from cattle in the field. In this study, we assessed whether a single strip of four interconnected compartments, instead of

Impact of Febrile State on Vancomycin Clearance in Pediatric Patients: Insights From Population Pharmacokinetic Modeling.

OBJECTIVE: Augmented renal clearance is increasingly recognized in pediatric patients with febrile neutropenia (FN), leading to enhanced elimination of renally cleared antimicrobials including vancomycin (VCM). Although fever‑associated hemodynamic changes may accelerate VCM clearance, their quantitative impact in the hospital-based general pediatric cohort remains unclear. The primary objective of this study was to quantify the effects of FN and infection-associated fever on VCM clearance in pediatric patients and to identify optimal dosing regimens across age and renal function strata. Individual target pharmacokinetic/pharmacodynamic attainment rates among cefepime-treated patients admitted to the ICU with hospital-acquired pneumonia with and without ECMO.

Cefepime (FEP) is used for hospital- and ventilator-associated pneumonia when Pseudomonas aeruginosa is involved. However, its pharmacokinetics (PK) in severe pneumonia necessitating extracorporeal membrane oxygenation (ECMO) remain unclear. This single-center, prospective study enrolled 70 mechanically ventilated patients with suspected pneumonia (n = 9 on ECMO), excluding those on renal replacement therapy. Dosing followed institutional renal function-based protocols. Plasma concentrations were quantified by liquid chromatography-tandem mass spectrometry, and a two-compartment PK model was d

In Silico Interim Adaptation of Proton Therapy in Head and Neck Cancer by Simultaneous Dose and Linear Energy Transfer Escalation.

PURPOSE: The outcome of proton therapy for head and neck cancer (HNC) varies considerably. We investigated the feasibility of adapting proton therapy plans based on 18F-fluorodeoxyglucose-positron emission tomography-defined biologic tumor volumes (BTVs) reflecting remaining aggressive tumor subvolumes 2 weeks into treatment (interim). Recognizing the potential to improve proton therapy response with increasing linear energy transfer (LET), we simulated a combined dose-LET escalation to the BTVs and compared it to pure dose escalation. In addition, the impact of relative biological effectivene

Adverse events during immunotherapy in Slovenian patients with metastatic melanoma reveal a positive correlation with better treatment outcomes.

BACKGROUND: Immunotherapy with CTLA-4 inhibitors and PD1 checkpoint inhibitors has initiated a breakthrough in the treatment and prognosis of patients with metastatic melanoma. The survival of these patients has increased from the expected survival time of less than 12 months to at least forty months. However, immunotherapy with either anti-CTLA-4 antibodies or PD1 inhibitors alone or in combination has a broad palette of significant immune-related adverse events. The aim of the study was to assess the correlation of immune-related adverse events with treatment outcomes defined as significant

Impact of decontamination on individual radiation doses from external exposure among residents of Minamisoma City after the 2011 Fukushima Daiichi nuclear power plant incident in Japan: a retrospective observational study.

Following the Fukushima incident, radiation doses from external exposure accounted for the majority of the total doses. Although countermeasures are being implemented, with the aim of reducing radiation exposure, little information is available on the effects of decontamination on individual doses among the residents of radioactively contaminated areas. To evaluate the effectiveness of the decontamination measures in reducing individual doses, and to examine the influence of the timing of decontamination and the district, data were analysed for 18 392 adults and 3 650 children in Minamisoma Ci

The Potential Impact of Alzheimer's Disease Early Treatment on Societal Costs of Care in Czechia: A Simulation Approach.

BACKGROUND: In Czechia, only about a quarter of people suffering from the Alzheimer's disease (AD) receive (usually belated) treatment. Because of their more rapid cognitive decline, untreated patients require extensive assistance with basic daily activities earlier than those receiving treatment. This assistance provided at home and nursing homes represents a substantial economic burden. AIMS OF THE STUDY: To calculate lifetime costs of care per AD patient and to evaluate potential care savings from early treatment. METHODS: We use Monte Carlo simulation to model lifetime societal costs of
