Extrinsic risk factors for tendinopathy: bridging the basic and clinical research divide.

PURPOSE: Tendon injuries frequently affect the Achilles, rotator cuff, and the tendons of the wrist extensors originating at the lateral epicondyle. Until recently, terms for tendon degeneration, disease, and injuries were debated between the basic research and clinical communities, creating barriers to advances in knowledge. Aside from acute traumatic rupture in healthy tendon, in the current 'omic age, most tendon disease, including ruptures in chronic disease, exists along a tendinopathy spectrum. While intrinsic factors such as sex, genetics, and aging are unmodifiable, extrinsic factors i

Clinical and ultrasonographic evaluation of uninjured dominant shoulder in amateur rugby players vs a control group: a pilot study.

BACKGROUND: Rugby is a sport involving a great number of shoulder collisions. Traumatic stress of the shoulder can weaken the static stabilizers and promote major injuries as dislocation or full-thickness tears of the rotator cuff. The goal of this study is to evaluate the clinical and ultrasonographic dominant shoulder factures in a group of amateur rugby players, with no history of shoulder injuries, and to compare them with those of a control group. METHODS: 52 male subjects join in the study: 26 amateur rugby players and 26 subjects, which did not practice rugby or competitive sport. Clin

Inferior Glenohumeral Ligament (IGHL) Injuries: A Case Series of Magnetic Resonance (MR) Imaging Findings and Arthroscopic Correlation.

INTRODUCTION: The inferior glenohumeral ligament (IGHL) complex commonly is assessed by both magnetic resonance imaging (MRI) and magnetic resonance (MR) arthrogram. Our study compared the accuracy of MR arthrogram compared to MRI using arthroscopic correlation as the gold standard. METHODS: A retrospective review of cases reporting an IGHL injury was performed. Seventy-seven cases met inclusion criteria, while five had arthroscopic reports that directly confirmed or refuted the presence of IGHL injury. Two arthroscopic reports confirmed concordant IGHL injuries, while three arthroscopic repo

Management of recent first-time anterior shoulder dislocations.

The management of a first episode of anterior shoulder dislocation starts with an analysis of the causative mechanism and a physical examination to establish the diagnosis. Based on the findings, the case can be classified as simple or accompanied with complications, most notably vascular or nerve injuries. Two radiographs perpendicular to each other should be obtained to confirm the diagnosis then repeated after the reduction manoeuvres. Additional imaging studies may be needed to assess concomitant bony lesions (impaction lesions or fractures). External reduction should always be attempted a

Severe heterotopic ossification after arthroscopic acromioplasty: a case report.

Heterotopic ossification is a well-recognized complication of spinal cord injury, closed head injury, total hip arthroplasty, burns, and other trauma and has been observed in various tissues such as muscles, tendons, ligaments, and menisci. Complications of arthroscopic acromioplasty are relatively uncommon and include hematoma, traction neuropathy, infection, acromial fracture, reflex sympathetic dystrophy, and instrument breakage. However, little has been reported on heterotopic ossification of the shoulder, particularly after arthroscopic surgery. Recurrent rotator cuff impingement symptoms

Impact of preoperative shoulder osteoarthritis severity score on outcomes after rotator cuff repair: A correlation study.

Arthroscopic rotator cuff repair (ARCR) generally improves clinical outcomes, but the impact of pre-existing glenohumeral osteoarthritis (GHOA) severity on these outcomes remains unclear. This study aims to evaluate the correlation between preoperative GHOA severity, assessed via the Shoulder Osteoarthritis Severity (SOAS) score, and postoperative patient-reported outcomes (PROs) following ARCR with a minimum of two years follow-up. In this retrospective cohort study, 150 patients who underwent ARCR between 2018 and 2023 were included. Preoperative GHOA severity was assessed independently by t

Role of Hypoxia-Inducible Factor-1α in Regulating Muscle Degeneration After Rotator Cuff Tears.

BACKGROUND: Secondary muscle degeneration after a rotator cuff tear (RCT) critically affects clinical outcomes. Vascular compromise after a tendon injury creates a complex microenvironment that may be associated with the degeneration of rotator cuff muscle. The role of hypoxia-inducible factor-1α (HIF-1α), a master regulator of cellular stress responses to hypoxia, in modulating muscle abnormalities after an RCT remains undefined. PURPOSE: To define the role of HIF-1α in stem cell differentiation and muscle degeneration after an RCT in a murine model. STUDY DESIGN: Controlled laboratory stud

Lower socioeconomic status is correlated with worse outcomes after arthroscopic rotator cuff repair.

BACKGROUND: Socioeconomic status has been recognized as a crucial social determinant of health influencing patient outcomes. Area Deprivation Index (ADI) is a validated measure of an area's socioeconomic status. Limited data exists on the impact of ADI and clinical outcomes and complications following rotator cuff repair (RCR). The purpose of this study was to investigate the impact socioeconomic factors have on outcomes following primary arthroscopic RCR. METHODS: This is a retrospective cohort study with 1-year follow-up. Patients who underwent primary rotator cuff repair at a single instit

Can inadequate acromiohumeral distance improvement and poor posterior remnant tissue be the predictive factors of re-tear? Preliminary outcomes of arthroscopic superior capsular reconstruction.

PURPOSE: The factors affecting the anatomical and functional outcomes of arthroscopic superior capsular reconstruction (ASCR) were investigated in this study. Continuity between the posterior remnant tissue and graft, as well as medial-to-lateral anatomical graft continuity, might play a vital role in shoulder stability and functional recovery, which could be correlated with postoperative factors such as the acromiohumeral distance (AHD). METHODS: Thirty-two patients (36 shoulders) who underwent ASCR were included. The follow-up period was 24.8 ± 6.9 months. The relationship between graft con

Irreparable rotator cuff tears: long-term follow-up, five to ten years, of arthroscopic debridement and tenotomy of the long head of the biceps.

PURPOSE: Rotator cuff tears (RCT) are a common source of pain and disability of the shoulder and are frequently combined with pathology of the long head of the biceps tendon (LHBT). The aim of this study was to evaluate the long-term results (5-10 years) of arthroscopic debridement of the rotator cuff with or without tenotomy of the LHBT in patients with irreparable rotator cuff tears. METHODS: Patient files between January 2005 and December 2010 were retrospectively reviewed. Suitable patients were contacted and invited to the outpatient clinic for assessment of their shoulder function by th

Long-term clinical and radiographic outcome of rotator cuff repair with a synthetic interposition graft: a consecutive case series with 17 to 20 years of follow-up.

BACKGROUND: Treatment options for irreparable cuff tears include synthetic interposition grafts, but whether such grafts can maintain acceptable shoulder function and prevent cuff tear arthropathy in the long-term is unknown. METHOD: This was a retrospective case series of 13 consecutive patients treated with a synthetic interposition graft made of Dacron (DuPont, Wilmington, DE, USA). Patients were examined with bilateral ultrasonography, bilateral x-ray imaging, Constant-Murley score, and Western Ontario Rotator Cuff score. RESULTS: After a mean of 18 years (range, 17-20 years), 1 patient

[Mid-Term Clinical Results after Open Rotator Cuff Reconstruction in Double-Row Technique with Titanium Anchor Screws].

BACKGROUND: The double-row rotator cuff repair is discussed controversially. Despite improved biomechanical properties, reduced re-tear rates and higher costs, no significant difference compared to single-row fixation in the clinical results is found. Mid-term results of an open double-row fixation with titanium anchor screws are presented. MATERIAL AND METHODS: 237 patients (m = 142, f = 95, median age: 56.3 years) were operated in 2007 with this technique by the senior author (M. G.). Preoperatively, 2 years and 4,5 years postoperatively a subjective shoulder score (SSG) with follow-up rate

Is rotator cuff repair appropriate in patients older than 60 years of age? prospective, randomised trial in 103 patients with a mean four-year follow-up.

BACKGROUND: The objective of this randomised trial was to compare surgical rotator cuff repair to simple decompression by acromioplasty and biceps tenotomy in patients older than 60 years of age with a mean follow-up of 4 years. HYPOTHESIS: Tendon repair produces better functional outcomes than simple decompression and prevents progression towards cuff tear arthropathy in the longer term. PATIENTS AND METHOD: Of 130 initially included patients older than 60 years of age and having rotator cuff tears deemed amenable to surgical repair, 103 (79%) were evaluated after a mean of 4 years. The intraoperative use of ultrasound facilitates significantly the arthroscopic debridement of calcific rotator cuff tendinitis.

INTRODUCTION: During arthroscopy, the localization of calcific deposit in patients suffering from calcifying tendinitis can be demanding and time consuming, frequently using ionizing radiation. Intraoperative ultrasound has been recently promoted, facilitating deposit localization and reducing radiation dose. MATERIAL AND METHODS: In this prospective, randomized, controlled and clinical observer-blinded pilot trial, 20 patients with calcific tendinitis were operated. In group I, the deposit was localized conventionally. In group II, the deposit was localized using intraoperative ultrasound. T
