BACKGROUND: Chylothorax occurs when chyle from the thoracic duct leaks into the pleural space. While majority of cases are iatrogenic, traumatic chylothorax can occur when rib or vertebral fractures disrupt the thoracic duct. These occurrences are exceedingly rare, particularly following blunt traumatic insult.
METHODS: We performed a retrospective review of a case of chylothorax following blunt trauma. Data was extracted from the electronic medical record.
CASE: A 60-year-old female presented to the trauma bay after a motor vehicle crash as a restrained driver with bilateral chest pain. Of

OBJECTIVE: The frequency, operative management, and outcomes for patients with traumatic vascular injury in the UK are unknown. The Vascular Interventions and Surgery in Trauma Audit (VISTA) aimed to describe the contemporary landscape of UK vascular trauma compared with retrospective data from the National Vascular Registry (NVR).
METHODS: A prospective, resident led service evaluation was conducted across UK major trauma centres (MTCs) delivered by the National Trauma and Research Innovation Collaborative and the Vascular and Endovascular Research Network. The evaluation included patients w

Post-traumatic stress disorder (PTSD) is caused by exposure to a traumatic or stressful event. Symptoms related to this disorder include persistent re-experiencing of memories and fear of generalization. Current pharmacological treatments for PTSD are insufficient, with fewer than 30% of patients reporting symptom remission. This study aims to determine the efficacy of acute (R,S)-ketamine and chronic fluoxetine (FLX) in reducing fear memory and fear generalization. In rodents, fear conditioning (FC) is commonly used in the literature to induce behaviors related to symptoms of PTSD, and the op

Patients with severe trauma or polytrauma frequently acquire alterations in immune functions which are correlated to dysbalanced cytokine synthesis. In these settings the role of polymorphonuclear neutrophil granulocytes (PMN) as cytokine-producing cells is less well characterized. The immunosuppressive role of interleukin (IL)-10 is well known, and increased systemic IL-10 levels are related to the severity of injury and to posttraumatic complications. We determined concentrations of IL-10 in culture supernatants of 30 individual PMN fractions isolated from 18 severely traumatized patients (1

We investigated the effect of carbachol (CAR, a cholinergic agent) on intestinal mucosal blood flow (IMBF), activity of Na-K-ATPase, expression of aquaporin (AQP)-1, and intestinal absorption rate during enteral resuscitation of a 35%TBSA scald in rats with a glucose electrolyte solution (GES). One hundred male Wistar rats were randomly divided into five groups: sham scald (N group); scald without fluid resuscitation (S group); scald resuscitated with enteral GES alone (GES group); scald resuscitated with enteral CAR alone (CAR group); and scald resuscitated with enteral CAR plus GES (GES/CAR

The records of 154 patients with 254 abdominal vascular injuries seen over 5 years (1980 to 1985) were reviewed. The overall mortality rate (MR) was 46%. This included 100% (5/5) for blunt injuries, 49% (59/119) for gunshot wounds, and 23% (7/30) for stab wounds. The most common venous injuries and the MRs were: inferior vena cava, 59% (33/56) and iliac veins, 65% (10/16). The most common arterial injuries and the MRs were: aorta, 68% (15/22), iliac artery, 57% (12/21), and superior mesenteric artery, 67% (8/12). Of 84 patients who presented to the emergency department (ED) with a blood pressu

Posttraumatic stress disorder (PTSD) memory processes are often investigated using fear conditioning experiments. Based on the premises of Ehlers and Clark's cognitive model of PTSD, we previously found that an additional stimulus (the CSX) that was presented simultaneously with aversive stimulation became a stronger cue for conditioned fear compared to a standardly presented predictive conditioned stimulus (CS+). In the current study, we tested whether an additional CSX with higher semantic similarity to the aversive stimulus resulted in a stronger cue than the semantically dissimilar CSX (th

Visual system damage and dysfunction caused by exposure to a blast wave has been described in both clinical studies and in pre-clinical models. Within the retina, retinal ganglion cells (RGC) exhibit sensitivity to mild blast-mediated traumatic brain injury (bTBI), which can result in progressive neurodegeneration. The purpose of this study was to determine if repetitive hypoxic preconditioning (HPC) can prevent bTBI-mediated RGC damage and death. This study utilized clinically relevant outcomes of RGC structure and function, supported by histological analysis of the surviving RGCs. Mice were

This case report presents the successful surgical management of a 34-year-old male who had blunt chest trauma resulting in a left atrial appendage tear after a traffic accident. He arrived at the emergency department 30 minutes post-injury in a gasping state, showing signs of hypovolemic shock, tachycardia (132 bpm), hypotension (blood pressure 88/48 mm Hg) and decreased breath sounds on the left side. The initial management included fluid resuscitation and the insertion of an intercostal drain, which drained 1.2 litres of blood. After the initial fluid resuscitation, haemodynamic stability wa

Exposure to repeated mild blast traumatic brain injury (mbTBI) is common in combat soldiers and the training of Special Forces. Evidence suggests that repeated exposure to a mild or subthreshold blast can cause serious and long-lasting impairments, but the mechanisms causing these symptoms are unclear. In this study, we characterise the effects of single and tightly coupled repeated mbTBI in Sprague-Dawley rats exposed to shockwaves generated using a shock tube. The primary outcomes are functional neurologic function (unconsciousness, neuroscore, weight loss, and RotaRod performance) and neuro

In the literature, burns are understood as traumatic events accompanied by increased morbidity and mortality among affected patients. Their characteristic feature is the formation of swelling and redness at the site of the burn, which indicates the development of inflammation. This reaction is not only important in the healing process of wounds but is also responsible for stimulating the patient's innate immune system. As a result of the loss of the protective ability of the epidermis, microbes which include bacteria, fungi, and viruses have easier access to the system, which can result in inf

BACKGROUND: The phrase "floating knee is a flail knee joint," referring to ipsilateral femoral and tibial fractures, was first used by Blake and McBryde in 1975. This condition is often caused by a high-energy trauma with often extensive injury to the soft tissues, and is accompanied by life-threatening systemic complications, including head, chest or abdominal injuries and a high incidence of fat embolism. Floating knee is a severe and uncommon injury pattern.
CASE SUMMARY: A 27-year-old man sustained multiple injuries when the electric motorcycle he was riding was hit by a van. His injuries

BACKGROUND: Intensive care unit (ICU) patients can experience emotional distress and post-traumatic stress disorder when they leave the ICU, also referred to as post-intensive care syndrome. A deeper understanding of what patients go through and what they need while they are transitioning from the ICU to the general ward may provide input on how to strengthen patient-centred care and, ultimately, contribute to a positive experience.
AIM: To describe the patients' experience while transitioning from the ICU to a general ward.
DESIGN: A descriptive qualitative study.
METHOD: Data were gathere
