We performed preoperative proton beam therapy for locally advanced thymoma and subsequently achieved complete resection. The patient was 31-year old woman, in whom chest computed tomography revealed a huge mass at the left anterior mediastinum. We diagnosed locally advanced type B3 thymoma. Because of the potential for complications to the lung and heart, definitive photon radiation therapy would have been difficult to administer. Thymomas rarely present with chest pain due to hemorrhage. This could cause shortness of breath if it ruptures into the pleural space, and is best managed surgically. We describe the case of an 83-year-old woman who presented with such symptoms. Computed tomography showed a ruptured mediastinal mass with pleural effusion. She was managed successfully by thoracotomy with excision of the mass and drainage of the effusion. Histopathology revealed a ruptured thymoma with infarction and necrosis.

Metaplastic thymoma is a rare primary thymic epithelial tumor, and its biologic behavior is uncertain. We report 7 cases of metaplastic thymoma. The spindle components of the tumor seemed to be undergoing epithelial-mesenchymal transition, which is characterized by the loss of E-cadherin expression. Epstein-Barr virus seemed not to have an etiologic role in metaplastic thymoma. No gross genetic alterations were found by using comparative genomic hybridization. We report a rare case of invasive thymoma with endobronchial growth in a 69- year-old woman. Chest CT revealed an ill-defined mass with a calcified lesion in the anterior mediastinum and coin lesion in the bilateral lung fields. We report two cases of primary thymic adenocarcinoma, a very uncommon neoplasm with limited information in the literature. Both patients were men (age 15 and 39 years).
