An audit of treatment outcome in acromegalic patients attending our center at Bergamo, Italy.

BACKGROUND: Acromegaly is a chronic disease impacting on morbidity and mortality. Increased mortality is reverted after the achievement of hormonal targets. The relative role of treatment options is still matter of debate. METHODS: A retrospective chart review was performed on all the acromegalic patients attending our center along the last 20 years. RESULTS: Data about 159 patients (83 F) were retrieved and analyzed: 18% had been lost to follow-up, while follow-up was >5 years in 79%. Growth hormone (GH) at diagnosis was 24 microg/L (median, range 3-239). Pituitary MRI showed a macro-, micr…

Genetic variation at the growth hormone (GH1) and growth hormone receptor (GHR) loci as a risk factor for hypertension and stroke.

An increased prevalence of both hypertension and cerebrovascular stroke is apparent in growth hormone (GH) deficiency whilst hypertension is a frequent complication in acromegaly. This has suggested a possible link between GH, stature and arterial function. Since the risk of both hypertension and stroke also appears to be inversely correlated with adult height, we have instigated an exploratory study to assess whether inter-individual variation in the genes encoding human growth hormone (GH1) and the GH receptor (GHR) might be associated with an increased risk of hypertension and stroke. GH1 p…

Hormonal crossroads of the heart: from classic endocrine regulation to cardiac hormone secretion: an updated review.

BACKGROUND: The concept of the heart as both a target and source of hormones has reshaped cardiovascular and endocrine medicine. Classic endocrine axes—including pituitary, thyroid, adrenal, and gonadal hormones, govern cardiac growth, contractility, metabolism, and stress adaptation. Endocrine disorders such as thyroid dysfunction, Cushing’s syndrome, acromegaly, diabetes, and obesity frequently present with distinct cardiovascular phenotypes. Conversely, cardiac disease induces systemic endocrine disturbances, from low T3 syndrome to insulin resistance and hypogonadism, underscoring a bidire…

Multi-omics Investigations in Endocrine Systems and Their Clinical Implications.

Innovative techniques such as the "omics" can be a powerful tool for the understanding of intracellular pathways involved in homeostasis mai … tions, including Cushing's syndrome and Addison's disease. Pituitary tumors and disorders including acromegaly and hypopituitarism have been studied using proteomics to examine changes in protein expression. Reproductive problems such as polycystic ovarian syndrome and endometriosis are two examples of conditions where alterations in protein expression have …

Painless thyroiditis associated with SARS-CoV-2 and influenza infections in a patient with central hypothyroidism after pituitary surgery.

SUMMARY: We present the case of a 50-year-old Japanese woman who was transferred to our hospital with a 2-day history of fever, sore throat, and malaise. She was diagnosed with acromegaly 9 months ago while being treated for diabetic ketoacidosis, for which she underwent pituitary surgery. She was diagnosed with hypopituitarism postoperatively and was prescribed hydrocortisone and levothyroxine. Her glycemic control was good on metformin. Tests for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and influenza were positive in the emergency room. Other laboratory findings included …

Depression and Impulsivity Self-Assessment Tools to Identify Dopamine Agonist Side Effects in Patients With Pituitary Adenomas.

Background: Dopamine agonists (DA) are the first line therapy for prolactinoma and symptomatic hyperprolactinemia; use as an adjuvant treatment for acromegaly and Cushing's disease is rare. Some patients develop de novo psychiatric symptoms or have exacerbation of pre-existing conditions during DA therapy. A practical, clinically sensitive depression and impulse control disorders (ICD; particularly hypersexuality and gambling disorders) detection tool is important for identifying at risk patients. The Barratt Impulsivity Scale (BIS-11) and the 9-item Patient Health Questionnaire (PHQ-9) are se…

Significance of postoperative fluid diuresis in patients undergoing transsphenoidal surgery for growth hormone-secreting pituitary adenomas.

OBJECT: Following successful transsphenoidal surgery in patients with growth hormone (GH)-secreting pituitary adenomas, a characteristic fluid diuresis has been described. In this paper the authors aimed to further analyze the degree of fluid diuresis as it relates to postoperative GH levels. METHODS: Between 2000 and 2008, 85 patients underwent transsphenoidal surgery for a GH-secreting adenoma at the USC University Hospital. A retrospective chart review was conducted. Postoperative fluid intake, output, and balance within 48 hours following surgery, as well as endocrinological data were rec

Radiographic evaluation of osteoarthritis.

Osteoarthritis is the most widespread form of arthritis in the United States. Classically, osteoarthritis has been grouped into primary and secondary types. Primary or idiopathic osteoarthritis is believed to be a sequela of altered biomechanical stresses across joints in susceptible individuals. Secondary osteoarthritis is a consequence of underlying cartilage damage, such as from preceding inflammatory arthritis, metabolic abnormality, or injury. The radiographic hallmark of osteoarthritis is asymmetric loss of cartilage space. Osteophytosis bony eburnation, subchondral cysts, and eventual s

Synchronous subarachnoid drop metastases from a pituitary adenoma with multiple recurrences. Case report.

The authors report the case of a 49-year-old man with synchronous drop metastases from a multiply recurrent somatotroph pituitary adenoma. The metastatic lesions were found in the subarachnoid space of the cauda equina and foramen magnum 18 years after the initial diagnosis of the disease. Five transsphenoidal resections had previously failed to cure the sellar tumor. Two of these, performed 4 and 5 years before the patient's current presentation, had been complicated by cerebrospinal fluid rhinorrhea that necessitated lumbar drainage. Resections of the two subarachnoid lesions, separated by 1

Radioimmunoassay of growth hormone-releasing hormone (GHRH) with a polyclonal antibody against synthetic GHRH(1-29)-Gly4-Cys-NH2: method and clinical studies.

A radioimmunoassay (RIA) for growth hormone-releasing hormone (GHRH) using a polyclonal antibody against synthetic GHRH(1-29)-Gly4-Cys-NH2 has been developed. The antiserum (RBM105) showed full cross-reactivity with GHRH-(1-44)NH2, GHRH-(1-40)OH, GHRH-(1-37)OH and GHRH-(3-44)NH2, and probably recognized the region of Ala4 to Lys12 of GHRH. Since the sensitivity of the GHRH RIA was 1.5 pg/tube, the lowest detectable plasma level was 5 ng/l when an extract of 0.3 ml of plasma per tube was used. On gelfiltration chromatography, the GHRH immunoreactivity of normal plasma was eluted in the same pos

Radiation therapy of pituitary tumors: results in 95 cases.

Between 1967 and 1985 95 patients with pituitary adenoma received radiation therapy at the University of California, Los Angeles (UCLA), and the West Los Angeles Veterans' Administration (VA) Medical Center. Seventy of these patients received radiation therapy immediately as adjuvant therapy following incomplete resection, and 25 were irradiated as primary treatment or after surgical failure. The majority of cases (87%) presented clinically with macroadenomas. Two-thirds of the patients were treated with parallel opposed lateral portals to a midline total dose of 50 Gy. The mean follow-up was

Short-term endocrinological results after gamma knife surgery of pituitary adenomas.

We report our preliminary results after the radiosurgical treatment of 25 secreting pituitary adenomas with a mean follow-up of 20 months (range 6-36 months). Fifteen acromegalic patients showed a decrease of 65% in mean growth hormone (GH) levels after 6 months and of 77% after 12 months. Only 3 patients (20%) are considered to be in remission (mean GH and IGF1 level into the normal range). A decrease of prolactin (PRL) was noted in 46% and 64% at 6 and 12 months after radiosurgery in 4 patients with prolactinomas. There was no case of PRL normalizaion. At present 3/4 patients have individual

Studies on "big" growth hormone from human plasma and pituitary.

Most of the immunoreactive growth hormone (IRGH) in human plasma elutes from Sephadex G-75 as "little" GH (LGH), mol wt 22,000, but 14-39% elutes earlier ("big" GH, BGH). In saline extracts of human pituitary, 11-17% of IRGH eluted as BGH. On gel filtration of pituitary and plasma BGH in 8 M urea, 59-81% ran as LGH, but when the remaining BGH was refiltered in urea, all ran as BGH. Thus there is a "urea-stable" and a "urea-labile" form of BGH. SImilarly, freezing and thawing converted over half of pituitary and plasma BGH to LGH, but when the "freeze-stable" BGH was again frozen, thawed, and r
