PURPOSE: It was the purpose of this study to identify and document normal and abnormal supraspinatus tendon function in vivo using real-time ultrasound.
METHODS: We defined 4 groups of 20 individuals each: partial tear (group 1), full-thickness tear (group 2), successfully repaired tear (group 3), and healthy asymptomatic controls (group 4). Except for group 4, all patients underwent magnetic resonance arthrography to confirm the diagnosis. All underwent ultrasound imaging of the supraspinatus tendon with the adducted arm at rest and under maximal isometric abduction. Tendon deformation was d

In 100 patients the fulcrum axis which is the line connecting the anterior tip of the coracoid and the posterolateral angle of the acromion, was used to position true anteroposterior radiographs of the shoulder. This method was then compared with the conventional radiological technique in a further 100 patients. Three orthopaedic surgeons counted the number of images without overlap between the humeral head and glenoid and calculated the amount of the glenoid surface visible in each radiograph. The analysis was repeated for intraobserver reliability. The learning curves of both techniques were

BACKGROUND: The use of suture anchors has made arthroscopic repair of the torn rotator cuff possible. However, objective evaluations have demonstrated high failure rates. The goal of this study was to compare the modes and rates of failure of two double-row arthroscopic repair techniques and the mini-open double-row technique.
METHODS: Thirty pairs of fresh-frozen human shoulders were used in this study. The specimens were prepared to simulate a cuff defect, which was then repaired. The repairs were done with three different lateral row techniques (Mason-Allen sutures passed through transosse

In 13 patients, the development of supraspinatus muscle atrophy and fatty infiltration after rotator cuff tendon repair was quantified prospectively via magnetic resonance imaging. Intraoperative electrical nerve stimulation at repair showed that the maximal supraspinatus tension (up to 200 N) strongly correlated with the anatomic cross-sectional muscle area and with muscle fatty infiltration (ranging from 12 N/cm(2) in Goutallier stage 3 to 42 N/cm(2) in Goutallier stage 0). Within 1 year after successful tendon repair (n = 8), fatty infiltration did not recover, and atrophy improved partiall

RATIONALE AND OBJECTIVES: Ultrasound exhibits comparable accuracy to magnetic resonance imaging (MRI) for assessing many common shoulder pathologies. Despite this, many patients are being recommended for follow-up MRI after undergoing shoulder ultrasound. The purpose of this study is to offer an initial benchmark for an acceptable rate of recommendation for follow-up MRI following shoulder ultrasound.
MATERIALS AND METHODS: During a two-month period all diagnostic shoulder ultrasound imaging reports from a community radiology site were isolated (n = 350). Referrals were from a mixed base but

A common shoulder problem that significantly detracts from patients' quality of life is shoulder instability (SI). Abnormal scapular positioning and movement are closely associated with rotator cuff injuries and SI, as shown by several studies. The aetiology of scapular dyskinesia (SD) adversely affects shoulder stability and function, including postural abnormalities, musculoskeletal problems, and neurological conditions. Presently, there is a paucity of studies on scapular kinetic alterations in SI rehabilitation. This paper rigorously examines the correlation between SI and scapular kinetic…

Elderly of ages greater than or equal to 65 years represent nearly 20% of the US population. Physiological functions of nearly all organ systems decline and the prevalence of major diseases increase with aging. Progressive musculoskeletal disabilities of the elderly with vestibular and proprioceptive neural dysfunction lead to poor balance, unsteady gait, and frequent falls that are the leading cause of fatal and non-fatal injuries in those greater than or equal to 65 years of age. Osteoporosis, defined by a marked decrease in mineral bone density, as quantified by dual-energy x-ray absorptiom

Shoulder injuries can pose as a diagnostic challenge for clinicians. Owing to the wide variety of pathologies and the fact that they frequently coexist, diagnosis can be difficult. Accurate diagnosis and management not only require a thorough understanding of each condition but the ability to narrow down the broad differential through detailed physical examination. This review article provides an in-depth examination of common shoulder conditions and outlines key physical examination techniques for upper extremity surgeons and other clinicians who manage these conditions. It begins by providin

Complications regarding inferior shoulder dislocations (ISD) are predominately rotator cuff injuries, neuropathies, and vascular insults. To our knowledge, there are no studies regarding the delayed appearance of neuropathies with inferior shoulder dislocations. A 32-year-old previously healthy male presented with an inferior shoulder dislocation that required open reduction and internal fixation after failed attempts at closed reductions in the emergency room and operating room. At the time of discharge, the patient had no neurologic complaints, which continued to the two-week follow-up appoi…

Anterior shoulder instability is the most common form of shoulder instability and is usually because of a traumatic injury. Careful patient selection is key to a favorable outcome. Primary shoulder stabilization should be considered for patients with high risk of recurrence or for elite athletes. Soft-tissue injury to the labrum, capsule, glenohumeral ligament, and rotator cuff influence the outcome. Glenoid bone loss (GBL) and type of bone loss (on-track/off-track) are important factors when recommending treatment strategy. Identification and management of concomitant injuries are paramount.

Open fractures, transverse, short oblique and spiroid fractures of the humeral shaft, as well as comminuted fractures with radial palsy or vascular injury, mostly lead to bad end-results if treated conservatively. The same is valid in the case of bilateral humeral shaft fractures, multiple injuries, polytrauma, pathologic fractures and pseudarthrosis. Good end-results and a low rate of complications in the operative procedure require an adequate approach to the fractured limb as well as a meticulous care of the soft tissues. In plate osteosynthesis, the anterolateral approach for the proximal

OBJECTIVES: This study aimed to determine and compare the prevalence, patterns and factors associated with injury between elite Malaysian able-bodied and para-badminton players.
METHODS: Medical records from July 2007 to December 2017 were reviewed.
RESULTS: Among 209 able-bodied players, 1010 injuries were reported. The injuries affected the lower limb (67.2%), sustained during training (94.2%), overuse in nature (38.7%), and involving mostly junior players (62.4%). The injury rate was 94/year, lower than previously reported. Patellar tendinopathy and muscle strain to the upper limb and tor

BACKGROUND: Scapula body fractures are rare injuries with an incidence of 1% of all fractures accounting for 3% to 5% of all upper extremity fractures. Fractures of the scapula commonly result from high-energetic trauma and fall from great height. While several studies focused on concomitant injuries of chest and head as well as the cervical spine, up to now in the common literature, no study exists analyzing the prevalence of concomitant intra-articular glenohumeral injury following extra-articular scapular fracture.
OBJECTIVES: The aim of this study was to analyze the prevalence of concomit

BACKGROUD: Collagen-inducing injections are emerging as new treatments for partial-thickness rotator cuff tears (PTRCTs). This study aimed to directly compare the effects of atelocollagen and acellular dermal matrix (ADM) injections on PTRCTs.
METHODS: Between July 2019 and December 2021, 183 patients with PTRCTs who were treated with ultrasound-guided collagen injections were retrospectively enrolled. Two injections were administered at 3-month intervals at the supraspinatus tear site, and tendon integrity was evaluated 6 months after the first injection using magnetic resonance imaging (MRI

CASE: A 75-year-old woman on long-term hemodialysis sustained anterior shoulder dislocation without a fracture. Four months later, she presented with persistent shoulder pain and limited motion. Imaging revealed humeral head osteonecrosis (Cruess stage V) and a massive rotator cuff tear. Reverse shoulder arthroplasty was performed with favorable outcome.
CONCLUSION: Osteonecrosis of the humeral head following anterior shoulder dislocation without a fracture is rare. To our knowledge, this is the first reported case in a dialysis-dependent patient. This case highlights the need for increased c

BACKGROUD: Surgeons face challenges in selecting cost-effective and biologically active agents for rotator cuff healing, given the numerous commercial products available, such as polydeoxyribonucleotide (PDRN) and atelocollagen (ATC). However, the precise efficacy of PDRN and ATC in rotator cuff healing remains debatable, and there is currently a lack of studies directly comparing the effects of the 2 agents on repaired cuff tendons. Therefore, the purpose of this study was to compare the efficacy of PDRN and ATC on bone-to-tendon interface (BTI) healing using a chronic rotator cuff tear (RCT)

BACKGROUND: Surgical technique has been shown to influence risk of surgical site infection following rotator cuff repair (RCR). Few studies have reported the rate of infection associated with mini-open RCR. The goal of this study was to report the postoperative infection rate and risk factors for infection among patients undergoing RCR performed by a single surgeon using a modified mini-open technique. Our hypothesis was that the rate of infection after mini-open RCR would be lower than previously reported for this surgical approach.
METHODS: We retrospectively reviewed an institutional shoul

PURPOSE: The purpose of this study was to evaluate clinical outcomes and survivorship of isolated biceps tenodesis (BT) at a minimum of 2 years and to identify patient-specific factors associated with these outcomes in patients undergoing BT without concomitant rotator cuff repair (RCR). We hypothesized that patient-reported outcomes would be significantly improved on American Shoulder and Elbow Surgeons Survey (ASES) and Single Assessment Numeric Evaluation (SANE), with a high rate of survivorship (>90%) at 2-year follow-up.
METHODS: A retrospective review of an institutional registry was pe
