Assessing plantar pressure distribution in children with flatfoot arch: application of the Clarke angle.

BACKGROUND: Flatfoot, or pes planus, is one of the most common foot posture problems in children that may lead to lower-extremity pain owing to a potential increase in plantar pressure. First, we compared plantar pressure distribution between children with and without flatfoot. Second, we examined the reliability and accuracy of a simple metric for characterization of foot posture: the Clarke angle. Third, we proposed a mathematical model to predict plantar pressure magnitude under the medial arch using body mass and the Clarke angle. METHODS: Sixty children with flatfoot and 33 aged-matched

The farrier's work environment.

The horse industry in Sweden has rapidly expanded in recent years. This increasing number of horses implies a greater need for more farriers. Shoeing a horse is hard physical work, and includes awkward work postures and repetitive movements. It is well known that hard physical work increases the risk of injuries and musculoskeletal problems. The risk is especially high for musculoskeletal disorders when certain movements are constantly repeated. Heavy or repeated unilateral loads lead to considerable stress on the muscles, which can lead to rupture and fatigue that can cause long term problems

Effects of foot orthoses on the work of friction of the posterior tibial tendon.

BACKGROUND: Posterior tibial tendon dysfunction is a significant contributor to flatfeet. Non-operative treatments, like in-shoe orthoses, have varying degrees of success. This study examined changes to the work of friction of the posterior tibial tendon under three conditions: intact, simulated flatfoot, and flatfoot with an orthosis. It was hypothesized that work of friction of the posterior tibial tendon would significantly increase in the flatfoot, yet return to normal with an orthosis. Changes to bone orientation were also expected. METHODS: Six lower limb cadavers were mounted in a foot

Reducing limping by tibial lengthening along nails in adult unilateral developmental dysplasia of the hip with high dislocation.

BACKGROUND/PURPOSE: Treatment of adult neglected developmental dysplasia of the hip (DDH) with high dislocation is still not established. The main concern of young patients is leg-length discrepancy (LLD), which leads to limping gait and impaired body image. Such patients usually ask for a minimally invasive treatment that can improve the LLD. METHODS: Between 1993 and 2003, 17 patients with neglected DDH with high dislocation (mean age, 22.8 years) were treated by tibial lengthening using the Ilizarov external fixator over an intramedullary nail. The inclusion criteria were unmarried young a

Why now? A physiological perspective on the first official sub-2-hour marathon.

The first official sub-2-hour marathon provides a unique real-world case for re-examining the determinants of elite marathon performance. This Perspective argues that the key scientific issue is not whether human physiology can theoretically support sub-2-hour running, but how physiological capacity can be preserved and expressed under late-race fatigue. Sabastian Sawe's 1:59:30 performance was marked by a 59:01 second half and, based on available split data, a 27:36 segment from 30 to 40 km, indicating late-race acceleration rather than simple pace maintenance. The wider front-group pattern,

Autoencoder-Enhanced Convolutional Neural Networks for Plantar Pressure-Based Gait Pattern Recognition: Model Development and Cross-Validated Evaluation Study.

BACKGROUND: Plantar pressure imaging is a stable modality that reflects gait-related biomechanical characteristics and has been used increasingly for gait assessment and recognition. However, plantar pressure images are high dimensional and nonlinear, making manual feature engineering and conventional machine learning insufficient to capture discriminative patterns. OBJECTIVE: This study aimed to develop a gait pattern recognition model based on plantar pressure using an autoencoder (AE)-enhanced convolutional neural network (CNN) and to evaluate its performance against baseline deep learning

Industrially Scalable Production of Triboelectric Nanogenerators for Energy Harvesters and Self-Powered Sensors for Motion Sensing.

Biomechanical energy harvesting based on the triboelectric effect offers great potential in the fields of wearable electronics and smart textile manufacturing processes. While triboelectric nanogenerators (TENGs) show immense promise for self-powered wearable electronics, their transition from laboratory prototypes to commercially available products has been hindered by a lack of scalable, cost-effective manufacturing methods. Here, we address this critical industrial gap by introducing an entirely textile-based TENG production platform that leverages high-speed automated embroidery. This stud

Asymmetric plantar loading during gait is prospectively associated with early changes in knee and hip pain in community-dwelling older adults.

BACKGROUND: Asymmetric mechanical loading during gait alters lower-limb force distribution and may influence joint stress, yet its contribution to early changes in hip and knee pain before clinically apparent osteoarthritis remains poorly understood. This prospective cohort study examined whether plantar loading asymmetry during walking is associated with subsequent changes in joint pain in community-dwelling older adults. METHODS: Community-dwelling adults aged ≥65 years underwent baseline 10-m walking trials using in-shoe loadsol® sensors to quantify plantar force kinetics. Bilateral hip an

Clinical value of fifth metatarsal osteotomy in hallux valgus with concomitant bunionette deformity: a symptom-based comparative study.

BACKGROUND: Hallux valgus is frequently accompanied by bunionette deformity, which may contribute to lateral forefoot pain and increased forefoot width. Although fifth metatarsal osteotomy is an established treatment for symptomatic bunionette deformity, the necessity of this procedure in patients with asymptomatic deformity remains unclear. PURPOSE: This retrospective cohort study aimed to evaluate the clinical necessity of symptom-based fifth metatarsal osteotomy in patients with hallux valgus and concomitant bunionette deformity. STUDY DESIGN: Retrospective comparative cohort study. METH

Laparoscopic transgastric and endoscopic-assisted enucleation of horseshoe shaped leiomyoma of the cardia: surgical technique.

Leiomyomas are rare benign esophageal tumors and represent less than 1% of all esophageal neoplasms. Those located at the esophagogastric junction (EGJ) are uncommon and challenging due to their proximity to the lower esophageal sphincter. Most cases are asymptomatic and discovered incidentally, though symptomatic patients may present with dysphagia, heartburn, regurgitation, or chest pain. Diagnosis typically involves barium swallow studies, upper endoscopy, endoscopic ultrasonography, and computed tomography. Surgical treatment is recommended for symptomatic patients or those with tumors lar

Three-dimensional analysis of trunk compensation during gait after internal hemipelvectomy: A case report.

BACKGROUND: Internal hemipelvectomy is a limb-salvage procedure used to treat aggressive pelvic bone tumors. Depending on the extent of pelvic resection, the procedure can substantially alter pelvic stability, muscle attachment sites, and lower-extremity loading during gait. These structural changes are associated with altered movement patterns during walking, which may influence the ability to maintain functional ambulation. Although gait deviations following hemipelvectomy have been reported, the role of trunk motion within these altered gait patterns remains poorly understood. RESEARCH QUE
