[Decreased plasma fibronectin (pFN) level in preterm infants with infections].

Plasma fibronectic (pFN) is a high molecular weight multifunction glycoprotein, which augments neutrophil and macrofage phagocytosis and acts as a nonspecific opsonin for the reticuloendothelial system. In this study we have determined pFN concentrations in fifty eight preterm infants to discriminate infected from non infected ones. Concentrations of pFN decreased from baselin in babies with early or late onset infections. The changes in pFN concentrations were not found before sepsis, but on day 1. By day 5 pFN concentrations have increased and have been no longer different from controls. We

Strategies for the prevention of early-onset neonatal group B streptococcal sepsis: a decision analysis.

OBJECTIVE: To perform a decision analysis to understand better the implications of 19 potential group B streptococcus screening and treatment strategies. METHODS: We searched the literature to locate appropriate articles from which to derive probability estimates. Using decision analysis, we determined the likely outcomes of 19 group B streptococcus screening and treatment strategies and focused on three main outcomes: 1) number of expected cases of early-onset neonatal group B streptococcal sepsis, 2) percentage of gravidas treated with intrapartum antibiotics, and 3) total costs. RESULTS:

Group B streptococcus induces tumor necrosis factor in neonatal piglets. Effect of the tumor necrosis factor inhibitor pentoxifylline on hemodynamics and gas exchange.

Group B streptococcus (GBS), a common neonatal gram-positive pathogen, causes similar pathophysiologic features in human newborns and neonatal animal models of sepsis. Previous reports suggest that mediators in addition to TxA2 and PGI2 contribute to the late effects of GBS infusion (2 to 4 h), which include persistent increases in Ppa, hypoxemia, systemic hypotension, and a progressive fall in CO. Tumor necrosis factor (TNF) infusion in animals produces several of the late GBS effects. We hypothesized that GBS causes increased serum TNF levels 2 to 4 h into infusion in neonatal piglets. We al

Cord blood gas patterns identifying newborns at increased risk of group B streptococcal sepsis.

The effect of group B streptococcal sepsis acquired in utero on umbilical cord gas values is not known. Hypothesizing that fetal acid-base balance may be affected, we sought to identify a pattern of cord gas values that might detect newborns at increased risk of group B streptococcal sepsis. This review encompassed all newborns from January 1, 1986 to March 31, 1990 who manifested group B streptococcal sepsis as confirmed by a positive blood culture. An increased-risk cord gas profile was identified as an arterial pH less than 7.18 with either an arterial carbon dioxide pressure less than 59 m

Incidence of early onset group B streptococcal septicemia in Sweden 1973 to 1985.

A retrospective investigation was made to determine the incidence of early onset group B streptococcal (GBS) septicemia in Sweden in relation to perinatal data. During the period 1973-1985 the yearly incidence of GBS septicemia increased from 0.1 to around 0.5 cases per 1,000 live births. In 416 cases verified by blood culture, the overall mortality was 15% 62/416). In babies with a birth weight of greater than 2,500 g this figure was 9.6% (29/303). In babies with a birth weight of less than 2,500 g the mortality was 29.2% (33/113). Males had a worse prognosis than females and were also more o

Incidence and etiology of neonatal septicaemia and meningitis in western Sweden 1975-1986.

In a retrospective study of neonatal septicaemia and meningitis in a defined region of western Sweden 1975-1986, 231 cases were identified. The incidence was 2.8/1000 live births. The case-fatality rate was 15%. thirty-three patients had meningitis. Only 55 patients (24%) had no known risk factors. Preterm delivery was a most important risk factor for both morbidity and mortality. The most common causative organisms were group B streptococci, Staphylococcus aureus and aerobic Gram-negative rods, together isolated from 82% of the patients. The cases were approximately equally divided between ve

Group B streptococcal bacteremia in men.

Twenty-three episodes of bacteremia due to group B Streptococcus (GBS) in men were reviewed for both clinical and microbiologic features. The commonest sources of bacteremia were infections of the respiratory tract (seven cases), skin (five cases), urinary tract (four cases), and intravenous access sites (four cases). Sixteen (70%) of the cases of bacteremia were hospital acquired, and 10 (43%) were polymicrobial (seven with Staphylococcus aureus). GBS bacteremia was generally low-grade and brief as assessed by the percentage and persistence of positive blood cultures. Among patients bacteremi

Maternal carriage and neonatal acquisition of group B streptococci.

Vaginal, cervical, and rectal swabs were obtained from 762 women early in pregnancy to culture group B streptococci (GBS). The overall carrier rate was 13.9% and the rectal, vaginal, and cervical carrier rates were 10.6%, 7.9%, and 6.3%, respectively, GBS were isolated only from the rectum in 33%, so the rectum is an important reservoir. Seventy-five percent of the culture-positive women were permanent or intermittent carriers. Ninety percent of all women positive at labor were positive before the 20th week; acquisition of GBS late in pregnancy was rare. The frequency of transmission to the ne

Immunofluorescence in group B streptococcal infection and idiopathic respiratory distress syndrome.

Immunofluorescence was performed on lung tissue obtained at necropsy from 18 newborn infants, including five with group B streptococcal (GBS) sepsis, seven with idiopathic respiratory distress syndrome (IRDS), and six control infants who died from other causes. Deposits of C3, IgG, and fibrin were found within hyaline membranes of infants who died with GBS sepsis or IRDS within 48 hours after birth. In some cases C4, factor B, and IgM were also observed. In five infants with IRDS who died more than five days after birth, immunofluorescent lung findings were less common and less intense. Hyalin

The Selective Interaction of Pistacia lentiscus Oil vs. Human Streptococci, an Old Functional Food Revisited with New Tools.

Pistacia lentiscus berry oil (LBO) represents a typical vegetal product of the Mediterranean basin that has been formally used in traditional cuisine for 100s of years. In addition to its interesting alimentary properties, this product could represent an interesting candidate in the field of research on the study of new anti-infective agents. In fact, in Mediterranean countries, lentisk oil still continues to be widely used in folk medicine for oral and skin affections, in particular, acute gingivitis, pediatric skin infections such as impetigo and foot plaques, and biofilm related infections

Maternal age‑stratified associations between Group B Streptococcus colonization and prenatal glucose metabolism and hemoglobin levels in pregnancy: a retrospective case‑control study.

OBJECTIVE: Group B Streptococcus (GBS) colonization during pregnancy is associated with adverse outcomes. The associations between GBS colonization status and clinical parameters measured at different antenatal time points, and whether these differ by maternal age, remain unclear. This study aimed to explore age-stratified associations between GBS colonization and clinical parameters. METHODS: A retrospective case-control study was conducted at a single center. Cases were GBS-positive women (n = 110), and controls were GBS-negative women (n = 155) selected from the same cohort and matched on

Assessment of dried blood spot and umbilical cord blood samples for the quantification of maternally transferred IgG against the group B streptococcal alpha-like surface proteins in neonates at Kolding Hospital, Denmark: a proof-of-concept, cross-sectional study.

BACKGROUND: Maternally transferred IgG against group B Streptococcus (GBS) reduces the risk of GBS disease in infants. Previous studies have used umbilical cord serum as a proxy for neonatal serum to quantify GBS-specific IgG in neonates. The maternal GBS-AlpN vaccine targets four N-terminals of the alpha-like surface proteins (AlpNs; RibN, AlphaCN, Alp1N, and Alp2/3N), which are present in 92-99% of invasive GBS isolates. Early vaccine licensure could rely on the serological threshold of risk reduction (SToRR) based on neonatal AlpN-IgG concentrations. Dried blood spot samples (DBSSs), routin

Gut microbiota and short-chain fatty acids in pregnant women with group B Streptococcus colonization and their impact on neonatal outcomes.

BACKGROUND: Group B Streptococcus (GBS) frequently colonizes the gastrointestinal and lower reproductive tracts in pregnant women. However, research has predominantly addressed its influence on the vaginal microbiota, leaving its effects on the gut ecosystem and the consequent implications for neonatal outcomes largely unexplored. METHODS: In this prospective study, 87 pregnant women were enrolled at the Women and Children's Hospital of Xiamen University from March 2022 to October 2023 and divided into GBS-positive and GBS-negative cohorts according to their colonization status. Fecal samples

Maternal 6-valent group B Streptococcus vaccine in non-pregnant and pregnant females: a randomized phase 1/2 trial.

Group B Streptococcus (GBS) disease is a major cause of infant morbidity and mortality. Capsular polysaccharide conjugate GBS vaccines have been evaluated for decades; however, none is licensed. This phase 1/2 trial evaluated an investigational maternal hexavalent polysaccharide-protein conjugate GBS vaccine (GBS6) in non-pregnant females and pregnant participants and their infants. In stage 1 (conducted in South Africa), healthy non-pregnant participants received GBS6, GBS6 + aluminum phosphate (AlPO4) or placebo; a subset received GBS6 + AlPO4 booster approximately 2 years later. Impact of combined vaginal group B Streptococcus screening (culture and rapid PCR) in pregnant women on neonatal morbidity and mortality.

BACKGROUND: Vaginal colonization by group B Streptococcus (GBS) in pregnant women is a major risk factor for early-onset neonatal infection, by vertical transmission during birth. Current guidelines recommend universal screening via vaginal swab and culture in the third trimester for antibiotic prophylaxis. Since 2013, the Antoine Béclère maternity hospital has offered combined screening for GBS carriage using culture at the third trimester and PCR on vaginal swabs in the delivery room. The primary aim was to assess the added value of PCR in culture-negative or unscreened women. METHODS: This

Multidrug resistance in group B Streptococcus causing urinary tract infection exposes an erythromycin-driven protective effect against oxidative stress.

Multidrug resistance has been reported in group B Streptococcus (GBS) from various origins, but rates among urinary tract infection (UTI) isolates are largely unknown. Erythromycin, a second-line antibiotic for GBS for which high rates of resistance have been reported, was recently shown to support the resistance of Staphylococcus to oxidative stress. To survey multidrug-resistant (MDR) GBS from UTI and to investigate the effect of erythromycin exposure on the bacteria's ability to resist oxidative stress, we determined the antibacterial activity of 18 antibiotics against 292 GBS UTI isolates

High prevalence of penicillin-resistant group B Streptococcus among pregnant women in Northwest Ethiopia.

Group B Streptococcus (GBS) is a normal constituent of the female genital and gastrointestinal flora but remains a leading cause of perinatal bacterial infections, including endometritis, bacteremia, chorioamnionitis, and urinary tract infections in pregnant women. In Ethiopia, reported GBS colonization rates among pregnant women range from 7.2 to 25.5%. This study aimed to determine the prevalence, antimicrobial susceptibility patterns, and associated factors of GBS colonization among pregnant women attending antenatal care (ANC) at Debre Markos Comprehensive Specialized Hospital, Northwest E
