Strongyloides stercoralis is a nematode that causes severe infections in immunocompromised patients. The low parasitic burden of chronically infected patients makes diagnosis difficult to achieve by conventional methods. Here, an in-house (IH) method for the isolation of parasite DNA from stools and a PCR assay for the molecular diagnosis of S. stercoralis were optimized. DNA yield and purity improved with the IH method which included a step of incubation of stool samples with a glycine-SDS buffer and mechanical disruption prior to DNA extraction. For the PCR assay, the addition of bovine seru

Abstract : Seven cases of parasitism by Strongyloides cebus were identified in Lagothrix cana from Brazil. Aspects of the clinical presentation, treatment, pathology, and parasitic biology of these infections are described. Moderate to severe disease was observed, requiring hospitalization of 3 primates, and diarrhea was the most common clinical sign described. One L. cana individual died, for which ulcerative enteritis was the major finding upon histopathological analysis. The use of ivermectin in these atelids was safe and effective against the parasite. Parallel attempts to experimentally

Strongyloides stercoralis hyperinfection poses a great challenge to physicians in the tropics due to their non-specific symptoms and signs. Early identification and initiation of treatment improves mortality rates. Reports of Strongyloides infection in Hansens disease, systemic lupus erythematosus (SLE), organ transplant recipients and malnourished individuals have been reported in the published literature. The outcomes were variable, which may be attributed to a lack of treatment protocol. Treatment has been individualized and reports available are based on individual case reports and small c

An AIDS patient was diagnosed with Strongyloides stercoralis hyperinfection complicated by ileus. Serum ivermectin concentrations were very low after rectal administration, but increased after subcutaneous doses. Absorption of rectal ivermectin appears poor, and subcutaneous administration can increase serum levels quickly, which may improve treatment in critically ill patients.

BACKGROUND: We investigated whether luciferase immunoprecipitation systems (LIPS) can be the basis for a more rapid, specific, and standardized assay for the diagnosis of Strongyloides stercoralis infection.
METHODS: A LIPS assay was developed based on immunoglobulin (Ig) G or IgG4 antibody to a recombinant Strongyloides antigen (NIE) and was compared with an NIE enzyme-linked immunosorbent assay (ELISA). A second antigen, S. stercoralis immunoreactive antigen (SsIR), was tested alone and in combination with NIE. The assays were tested using serum samples from patients with parasitologically

Strongyloidiosis is widely distributed in tropical and subtropical areas but is a rarely reported parasitic infestation in Turkey. Disseminated strongyloidiosis may develop in patients with immunodeficiencies. We report a case of Strongyloides stercoralis infection and Loeffler's syndrome that developed in a patient who had received systemic prednisolone. The patient was a 20 year-old man, born in Hatay, Turkey. The patient presented at our department complaining of abdominal pain and leg pain. After he was admitted for further examination; headache, sore throat and cough developed. The differ

Minnesota accepts more refugees per capita than any other state. For that reason, primary care physicians throughout the state are likely to encounter refugees in their practices. This article describes 5 infectious diseases that are common among refugees and what physicians need to know about them in order to effectively screen and treat patients who are newcomers to this country. The diseases discussed are strongyloides, schistosomiasis, malaria, hepatitis B, and tuberculosis.

We report strongyloides hyperinfection in two patients with generalized hypogammaglobulinemia from multiple myeloma and nephrotic syndrome, despite a significant strongyloides-specific immunoglobulin G (IgG) response. In contrast to reports on animals, where human IgG was shown to be a protective antibody, our observation suggests that in humans, immunity to the infective-stage larvae is not protective against the autoinfective larvae, which are the causative agents of strongyloides hyperinfection.

We report the case of a 77-year-old man who developed a Strongyloides hyperinfection syndrome following immunosuppressive therapy more than 60 years after he moved away from an area endemic for Strongyloides stercoralis. Successful eradication of the nematode was achieved with an off label subcutaneous formulation of ivermectin. However, the patient subsequently died from acute respiratory distress syndrome (ARDS). Despite a high wormload in the stool and sputum of the patient and delayed infection control measures in the hospital, testing of the medical staff revealed a very low risk of Stron

Strongyloides stercoralis hyperinfection is a rare but know complication in patients with intestinal parasitism. Immunosuppressive therapy and acquires immunodeficiency syndrome (AIDS) are major risk factors for dissemination. Diagnosis requires identification of larvae in body fluids or biopsy and the mortality rates can be as high as 80%. The enteric bacteria are carried by invasive larvae and this can result in septicemia, pneumonia, meningitis, and disseminated bacterial infection in many parts of the body. We present a case of Strongyloides stercoralis hyperinfection complicated by Escher

The negative effects of nematodes in small ruminants can be reduced by use of dewormers but their effectiveness is increasingly limited by the emergence of anthelmintic resistance. The efficacy of benzimidazole (BZ) anthelmintics in the Philippines was estimated by an in vitro larval development assay using worm eggs recovered from faeces collected from goats and sheep. Two hundred and eighteen farms were selected to represent areas of the country with high goat and sheep populations and the full range of farm sizes, from smallholders with just a few animals to commercial and institutional far

Intestinal parasitic infection among five interior communities at Bakun Valley, upper Rejang River, Sarawak, Malaysia, was investigated as part of a public health impact assessment of the proposed US$ 3 billion Bakun Hydroelectric Project. Coproparasitological examination of 355 stool samples from 7 of 16 villages representing 5 of 7 tribes in the area revealed infection rate of 41%. A higher infection rate was found among the settled Kayans (56%) than the seminomadic Penans (29%). Infection rate was high (68%) among children less than 14 years old. Trichuris trichiura accounted for more than

With particular reference to parts of eastern and southern Africa the authors outline the dire effects of parasitic worms on people in the tropics and subtropics. Helminth infections can be controlled with drugs but in the long term a more comprehensive approach is required, dealing with poverty, health care and education, living conditions, sanitation and water supplies.

OBJECTIVE: To analyse the efficacy of ELISA serology in patients with Strongyloides infection acquired during World War II and maintained through repeated autoinfection.
DESIGN: Descriptive.
SETTING: Laboratory of Parasitology, Faculty of Medicine, Leiden, the Netherlands.
METHODS: Parasitological and clinical data on 193 ex-prisoners of war (South-east Asia) were presented previously (1990) by Verburg and De Geus. ELISA using L-3 S. ratti antigen was carried out with sera of these patients and the results were compared with those of repeated stool examinations using Baermann's method.

The course of a subcutaneous weight-related infection with Strongyloides ratti was followed in rats fed diets containing either 3 mg Zn/kg diet [zinc deficient (Zn-)] or 40 mg Zn/kg diet [zinc adequate (Zn+)]. At 19 d postinfection (dpi) the proportions of larvae persisting in the intestines as adult worms were 52 +/- 2% (means +/- SEM) for Zn-, 39.5 +/- 2.5% for pair-fed Zn- (Zn-PF), and 31.6 +/- 3.2% for Zn+ (p less than 0.001, analysis of variance); some Zn- rats were then transferred to the zinc-adequate diet [This was the zinc-repleted group (ZnR).] Both groups retained a group of pair-fe

After the resection of a localized lung carcinoma, a 65-year-old veteran developed jejunal perforation and pneumonia, both of which subsequently were shown to be caused by infection with Strongyloides stercoralis. The patient recovered after jejunal resection and treatment with thiabendazole. Several unusual aspects of the case are discussed.
