Endoscopic third ventriculostomy in patients with a diminished prepontine interval.

OBJECT: Fenestration of the floor of the third ventricle is vital to the success of endoscopic third ventriculostomy (ETV) in treating patients with noncommunicating hydrocephalus. A generous prepontine interval (PPI) is generally accepted as one anatomical feature that may affect the safety and functionality of ETV. Whether a diminished PPI influences the safety or success of ETV, however, has not been adequately assessed. METHODS: A review was conducted on the last 100 ETV procedures performed by the first author (M.M.S.). From archived preoperative MR imaging studies, the PPI was measured

Supracerebellar-supratrochlear and infratentorial-infratrochlear approaches: gravity-dependent variations of the lateral approach over the cerebellum.

OBJECTIVE: Lateral supracerebellar-infratentorial approaches are established for lesions in ambient cistern and posterolateral midbrain, but published surgical experiences do not describe results with this approach in the sitting position. Gravity retraction of the cerebellum opens this surgical corridor and dramatically alters exposure, creating 2 variations of the lateral supracerebellar-infratentorial approach: the supracerebellar-supratrochlear approach and the infratentorial-infratrochlear approach. METHODS: We reviewed our experience treating cavernous malformations and arteriovenous ma

A comparison between transport and diffusion calculations using a finite element-spherical harmonics radiation transport method.

Most researchers choose the diffusion approximation to the transport equation as the model to describe photon migration in biological tissues. However, the applicability of this approximation is limited and, in certain cases, invalid. In this paper we introduce a two-dimensional, finite element-spherical harmonics (FE-P(N)) radiation transport method for the simulation of light propagation in tissue. The propagation of light is investigated first in a layered cylinder, which can be seen as a very simplistic approximation of a human head. Effects of the anisotropy factor g on the photon migrati

Choroid plexus epithelium (normal and neoplastic) expresses synaptophysin. A potentially useful aid in differentiating carcinoma of the choroid plexus from metastatic papillary carcinomas.

As an incidental finding in paraffin sections of brain tissue used as positive controls for synaptophysin immunostain, the cytoplasm of choroid plexus epithelium present was found to stain strongly positively for this substance. This was subsequently found to be the case in normal choroid plexuses in autopsy material from infancy to old age, as well as in epithelial cells of papillomas and carcinomas of the choroid plexus. The latter findings may prove useful in differentiating choroid plexus carcinomas from metastatic papillary carcinomas of extracerebral origin with the exception of neuroend

Syringomyelia-Chiari complex: magnetic resonance imaging and clinical evaluation of surgical treatment.

Thirty patients with syringomyelia-Chiari complex who underwent posterior fossa decompression or syringosubarachnoid shunting were studied clinically and by magnetic resonance (MR) imaging to assess the changes in the syrinx and in their clinical picture after surgery. When symptoms of posterior fossa compression were present, posterior fossa decompression was performed; however, when symptoms of posterior fossa compression were absent, the choice of posterior fossa decompression or syringosubarachnoid shunting depended, respectively, on whether the syrinx was narrow or wide on MR imaging. At

Symptomatic cerebrospinal fluid dissemination of cerebral glioblastoma. Computed tomographic findings in 11 cases.

Computed tomography (CT) findings in eleven patients with symptomatic cerebrospinal fluid (CSF) dissemination from cerebral glioblastoma were analyzed and, in seven cases subsequently autopsied, they were compared with histological observations. Each patient had multiple CT abnormalities including periventricular enhancement (5/11), subarachnoid enhancement (10/11) and progressive hydrocephalus (7/9) by cranial CT, and small filling defects with or without block (5/5) by CT myelography. The areas that showed periventricular or subarachnoid enhancement on CT were confirmed to have macroscopical

Raised intracranial pressure and cerebral blood flow. 4. Intracranial pressure gradients and regional cerebral blood flow.

Intracranial pressure was raised by expansion of a supratentorial subdural ballon in anaesthetized baboons. Pressures were measured at several sites, both supratentorial and infratentorial, and cerebral blood flow was measured in each cerebral hemisphere separately. Pressures recorded from the right and left lateral ventricles corresponded closely throughout. Highly significant correlations were also obtained between the pressures in the right and left subdural spaces and the mean intraventricular pressure. There was, thus, no evidence of intracompartmental pressure gradients within the suprat

Radiological DESH score cannot predict outcome after shunt surgery: A Swedish prospective follow-up study.

BackgroundDisproportionately enlarged subarachnoid-space hydrocephalus (DESH) has been proposed as a radiological predictor of shunt responsiveness in idiopathic normal pressure hydrocephalus (iNPH), but previous studies have reported inconsistent results. We investigated the association between preoperative DESH score and postoperative outcome after shunt surgery in a prospective iNPH cohort.MethodsThis prospective study included 101 consecutive patients with iNPH from the Gothenburg Prediction of Outcome in iNPH (POiNT) study. Preoperative DESH scores were assessed on MRI using previously pu

Predictors of shunt responsiveness and outcomes in idiopathic normal pressure hydrocephalus: a retrospective cohort study.

BACKGROUND: Idiopathic Normal Pressure Hydrocephalus (iNPH) remains a challenging clinical diagnosis with variable treatment response. OBJECTIVES: This study aimed to identify clinical, imaging, and CSF biomarkers associated with favorable outcomes following shunt placement. METHODS: All patients evaluated for iNPH at the Tel-Aviv Medical Center (TLVMC) between 2020 and 2022 were included. Participants underwent clinical, cognitive, and imaging assessments, high-volume lumbar puncture (LP). LP responders were referred for shunt placement, and outcomes assessed at one year. RESULTS: 183 pati

The microtubule-binding protein EML3 is required for mammalian embryonic growth and cerebral cortical development, and Eml3 null mice are a model of cobblestone brain malformation.

The cerebral cortex is a multi-layered structure generated through the migration of neural precursors from their birthplace in the ventricul … rive. The reduced structural integrity of the PBM leads to focal over-migration of neurons into the subarachnoid space. These findings strengthen the link between the EML protein family and cortical neuronal migration defects by identifying Eml3 as the first EML family member whose absence leads to over-migration of neuroblasts. Moreover, we report the first COB-lik …

High Evans Index in progressive supranuclear palsy may mimic normal pressure hydrocephalus: a retrospective cross-sectional study.

BACKGROUND: Differentiating true normal pressure hydrocephalus (NPH) from neurodegenerative disorders with ex-vacuo ventriculomegaly remains a clinical challenge. Misdiagnosis can lead to inappropriate and risky shunt surgeries. OBJECTIVE: To evaluate the diagnostic utility of NPH-specific magnetic-resonance-imaging (MRI) markers, namely Evans Index (EI), callosal angle (CA), and disproportionatelyenlarged subarachnoid-space hydrocephalus (DESH) pattern, across different movement disorders. METHODS: In this retrospective cross-sectional study, we evaluated 125 consecutive patientsdiagnosed w

Hemorrhagic complication of arachnoid cyst: A case report and literature review.

Arachnoid cysts (ACs) are congenital malformations that can form anywhere in the subarachnoid space along the cerebrospinal axis. While intracystic hemorrhage and subdural hematoma (SDH) are rare, they can be urgent complications that may require emergency craniotomy. This report aims to present a case of an AC complicated by intracystic hemorrhage and SDH. A 63-year-old man presented after a fall, reporting a mild headache, right-sided weakness, confusion, speech difficulties, and gait ataxia. Brain magnetic resonance imaging (MRI) revealed an acute or early subacute SDH. A cystic structure m…

A nomogram for neurosurgical intervention in children with mild traumatic brain injury and minor subdural hematoma under 3 years.

Infants and toddlers with mild traumatic brain injury (mTBI) and minor subdural hematoma (SDH) were found to have a higher risk of requiring … tion curves, the Hosmer-Lemeshow test, and decision curve analysis. Immediate seizures, anemia, and subarachnoid space depth were identified as significant predictive factors by the BSR, leading to the development of a nomogram. The AUC for this nomogram, obtained through bootstrap validation (resampling = 500), was 0.893 (95% CI, 0.844-0.942). The model demonstrate …

Dynamic transition from spinal arachnoid diverticulum to syringohydromyelia in a cat: Correlation between computed tomography myelography and magnetic resonance imaging.

BACKGROUND: Diseases characterized by impaired spinal cord function due to fluid accumulation include syringohydromyelia (SHM) and spinal arachnoid diverticulum (SAD). SHM refers to fluid-filled cavities within the spinal cord, whereas SAD involves fluid accumulation in the subarachnoid space. Although the true incidence of SHM and SAD in cats is unknown, both conditions are rare based on the limited number of published case reports. Computed tomography (CT) myelography and magnetic resonance imaging (MRI) are regarded as the diagnostic gold standards. CASE DESCRIPTION: An approximately 1-yea…
