Solid variant of aneurysmal bone cysts in long tubular bones: giant cell reparative granuloma.

OBJECTIVE: The purpose of this study was to determine the age distribution, location, and imaging features of histologically proven solid variants of aneurysmal bone cysts in long tubular bones. MATERIALS AND METHODS: We performed a retrospective review of imaging studies of histologically proven solid aneurysmal bone cysts in long bones between 1961 and 2001. There were 30 cases comprising 29 radiographic, six CT, and eight MR imaging examinations. The lesions were evaluated for bone involved, location within a long bone, matrix, size, soft-tissue mass, and MR imaging characteristics. The im

[Old Monteggia lesions in children. Apropos of 14 cases].

Fourteen patients between the ages of 3.5 years and 11 years, with an old Monteggia lesion were managed in the Orthopaedics department between 1968 and 1997. Dislocation of the radial head was not diagnosed at the time of the initial trauma in 9 out of 14 cases and the diagnosis was made an average of 14 months after the trauma (range: 5 weeks to 84 months). Twelve of the 14 patients underwent surgical reduction of chronic dislocation of the radial head, mostly associated with proximal ulnar osteotomy and/or a procedure on the extensor retinaculum. The mean follow-up is 4 years 9 months. There

Operative treatment of post-traumatic proximal radioulnar synostosis.

The results of operative resection of a post-traumatic proximal radioulnar synostosis performed by one surgeon in eighteen limbs of seventeen consecutive patients during an eight-year period were reviewed retrospectively. The resection was performed an average of nineteen months after the injury; eight limbs had the resection less than twelve months after the injury. A free fat graft was used in the first eight patients. No adjuvant non-steroidal anti-inflammatory medication or low-dose radiation was used postoperatively as prophylaxis against heterotopic ossification. We classified the proxim

Survival with an arterial pH of 6.57 following major trauma with exsanguinating haemorrhage associated with traumatic amputation.

We report the survival of a multiply injured patient with exanguinating haemorrhage and an arterial pH of 6.5, following a road vehicle crash. The previously healthy 38 years old male driver veered off the motorway and collided with a tree. The ambulance arrived at the scene 9 min after being called by an eyewitness and, following rapid extrication from the wreckage; the patient arrived in hospital 27 min later (with a GCS of 6), and was immediately intubated. The patient had suffered near-complete amputation of the left leg at upper femoral shaft level, along with multiple distal fractures an

Insufficiency fractures of the tibia and fibula.

OBJECTIVE: Insufficiency fractures (IF) occur when normal or physiological muscular activity stresses a bone that is deficient in mineral or elastic resistance. IF of the tibia and fibula are probably less common than IF of the ribs, vertebrae, hip, pelvis, and distal ulna, and therefore they are frequently underrecognized and mistaken for other conditions. Our aim was to analyze the main features and outcome of IF of the tibia and fibula in patients attending our Rheumatology Service. METHODS: IF was considered when occurring spontaneously or with minimal trauma. Between January 1984 and Jul

Distal radius morphometry of volar curvature along with scaphoid and lunate facet inclinations and ulnar variance in the Anatolian population.

This study evaluates the morphometric characteristics of the volar distal radius, specifically volar curvature, scaphoid and lunate facet inclinations, and ulnar variance, in the Anatolian population. By identifying these population specific morphological variations, the study seeks to support improved anatomical compatibility of volar implants and enhance surgical planning for distal radius fracture fixation. Morphometric measurements were analyzed retrospectively on three-dimensional computed tomography images of 103 intact distal radii. Standardized sagittal sections were created through th

Influence of distal radioulnar joint subluxation on restricted forearm rotation after distal radius fracture.

PURPOSE: To analyze the influence of subluxation of the distal radioulnar joint (DRUJ) on restricted forearm rotation after distal radius fracture. METHODS: Twenty-two cases of healed unilateral distal radial fracture with restricted forearm rotation were included in the study. The subluxation of the DRUJ was evaluated using helical computed tomography scan at neutral, maximum pronation, and maximum supination and presented as the percent displacement of the ulnar head in both the injured and uninjured sides. The radiographic parameters of palmar tilt, radial inclination, dorsal shift, radial

Failed distal ulna resections.

Twenty patients with complaints of pain and limitation of activities for more than 1 year after ulnar head resection (Darrach) were studied. Their ages ranged from 18 to 60 years (mean, 38 years). The initial indications for operation were posttraumatic derangement in 18 patients, radial growth anomaly in 1, and a lesion of the ulna in 1. Follow-up ranged from 29 to 135 months (mean, 61 months). The patients had an average of 2.2 additional operations, with up to seven procedures per patient. Radiographs were compared with those from an equal number of successful Darrach resections chosen rand

A Radiation Safety Training Program Results in Reduced Radiation Exposure for Orthopaedic Residents Using the Mini C-arm.

BACKGROUND: Fluoroscopy during fracture reduction allows a physician to assess fractures and immediately treat a pediatric patient. However, concern regarding the effects of radiation exposure has led us to find ways to keep radiation exposures as low as reasonably achievable. One potentially simple way, which to our knowledge has not been explored, to decrease radiation exposure is through formal education before mini C-arm use. QUESTIONS/PURPOSES: We questioned whether a radiation safety educational program decreases radiation (1) time and (2) exposure among residents and patients. PATIENT

Comparison of short and long arm plaster casts for displaced fractures in the distal third of the forearm in children.

BACKGROUND: Various methods of cast immobilization have been recommended for the treatment of distal forearm fractures in children. The purpose of this study was to determine if short arm casts are as effective as long arm casts in the treatment of displaced fractures of the distal third of the forearm in these patients. METHODS: In a prospective randomized trial, consecutive patients, four years of age or older, who presented to The Women's and Children's Hospital of Buffalo with a displaced fracture of the distal third of the forearm were randomized to treatment with a short or long arm pla

Advantages of expulsion-proof pins in the treatment of olecranon fractures with tension band wiring: Comparison with a control group.

INTRODUCTION: Tension band wiring is considered the standard treatment for transverse olecranon fractures. Its main complications are pin migration and discomfort caused by the hardware. We have designed and used "expulsion-proof" pins (EPP) that are shaped to prevent migration and reduce discomfort. This study compared the complication rate between our device and Kirschner pins (controls). HYPOTHESIS: We hypothesised that EPP would have lower migration rates and fewer complications than standard Kirschner pins. MATERIALS AND METHODS: This retrospective, single-center, multi-operator, observ

Open reduction combined with CORA-based osteotomy of the ulna in the treatment of missed Bado type I Monteggia injury: A retrospective study of 5 cases.

Open reduction combined with ulnar osteotomy is the most common approach to treating missed Monteggia injuries. The osteotomy is usually performed at the proximal ulna to ensure better healing and fewer complications. The purpose of this study is to present a center of rotation angulation (CORA)-based osteotomy of the ulna for treating Bado type I Monteggia injuries. We retrospectively reviewed the cases of patients who were treated with open reduction combined with a CORA-based ulnar osteotomy between February 2014 and December 2016. Each patient provided his or her internal control, and paire

Treatment of fractures of the ulnar coronoid process.

OBJECTIVE: To discuss the classification, management and outcome of fractures of the ulnar coronoid process. METHODS: Retrospective analysis was carried out in 31 patients (19 men and 12 women of average age 29.8 years [range, 18-52 years]) with fractures of the ulnar coronoid process. The fractures were classified into four major groups based on the extent of injury to the ulnar coronoid process, the state of the anterior bundle of the ulnar collateral ligaments (UCL) and elbow stability. A fracture of the coronoid process less than halfway up was defined as type I (eleven cases); of the mid

[Relative value of internal fixation with K-wires in the treatment of fractures of the distal radius. Study in patients affected by multiple morbidities or dementia].

The age distribution of patients with distal radius fractures shows a clear predominance of patients over 60. In such patients fractures must be treated with due consideration for the general condition, accompanying illnesses, such as osteoporosis, and the often lesser ambitions concerning the function of the wrist joint. Three cases of patients over 70 years of age are presented: two of these patients were suffering from dementia and one, from multiple morbidities and poor general condition: In all three, severe osteitis of the distal radius developed after K-wire pinning to treat distal radi
