Spinal muscular atrophy: survival pattern and functional status.

OBJECTIVE: Spinal muscular atrophy (SMA) is common. The prevalence of SMA in southern Chinese is 1 in 53,000. The clinical course is variable. The traditional classification of SMA includes age of onset, age of death, achievement of motor milestones, and ambulatory status as criteria. There was a lack of inclusion of the best lifetime functional status of any child with SMA. With the advances in medical care, the life expectancy and ambulatory status of patients with SMA have improved. The objective of this study was to assess the survival pattern, ambulatory status, and functional status of c

Diagnostic Challenges and Surgical Outcomes of Miyazaki Syndrome: A Report of Two Cases and a Systematic Review.

Miyazaki syndrome is a rare and underrecognized complication of long-standing cerebrospinal fluid (CSF) diversion. It usually occurs in patients with chronic CSF overdrainage and is caused by enlargement of the cervical epidural venous plexus, which may compress the spinal cord and cause progressive myelopathy or radiculopathy. Because symptoms develop slowly and may resemble degenerative cervical myelopathy or other neurological conditions, diagnosis is often challenging. We report two illustrative cases from our institution. The first patient was a 57-year-old man with a ventriculoperitoneal

An Observational Study to Evaluate the Risk Factors and Quality of Life in Individuals With Spinal Cord Injury and Pressure Injuries.

Background Spinal cord injury (SCI) is a debilitating condition often associated with many complications, of which pressure injury (PI) is one of the most prevalent. Individuals with SCI are at an increased risk of developing PIs due to impaired protective sensations, dependence for mobility, and co-morbidities. Individuals with PIs often report an increase in morbidity, healthcare-related expenditure, and reduced quality of life (QoL). However, there is a paucity of data on PI-related risk factors and QoL outcomes in SCI individuals in developing countries like India. This study aimed to asse

Successful Acute-Phase Rehabilitation Leading to Activities of Daily Living Recovery in a Patient With Hypoxic Encephalopathy Following Prolonged Cardiopulmonary Arrest: A Case Report.

Hypoxic encephalopathy due to prolonged cardiopulmonary arrest is associated with poor neurological prognosis, often leading to significant impairment in activities of daily living (ADL) and social reintegration. Rehabilitation for patients experiencing cardiac arrest lasting over 15 minutes is rarely successful. This case report describes a 29-year-old male patient with hypoxic encephalopathy following a 19-minute cardiopulmonary arrest while in a sauna, who achieved substantial ADL recovery through intensive acute-phase rehabilitation. Cardiopulmonary resuscitation (CPR) was initiated 13 min

Ocrelizumab in highly disabled progressive multiple sclerosis patients

BACKGROUND: Ocrelizumab is a commonly used anti-CD20 monoclonal antibody with efficacy in the treatment of both relapsing-remitting (RRMS) and primary progressive (PPMS) multiple sclerosis. Real world use of ocrelizumab in MS patients with higher levels of motor disability requiring a walker or a wheelchair is not well characterized as these populations were excluded from initial studies. Higher levels of disability may be a barrier to treatment access. This study aimed to describe the access to, and tolerability and therapeutic outcomes of ocrelizumab in highly disabled MS patients in a real-

Underuse of indicated medications among physically frail older US veterans at the time of hospital discharge: results of a cross-sectional analysis of data from the Geriatric Evaluation and Management Drug Study.

BACKGROUND: Medication underutilization, or the omission of a potentially beneficial medication indicated for disease management, is common among older adults but poorly understood. OBJECTIVES: The aims of this work were to assess the prevalence of medication underuse and to determine whether polypharmacy or comorbidity was associated with medication underuse among physically frail older veterans transitioning from the hospital to the community. METHODS: This was a cross-sectional analysis of patients who were discharged from 11 US veterans' hospitals to outpatient care, based on data from t

[Special outpatient services at 5 and 8 years in very-preterm children in the EPIPAGE study].

UNLABELLED: The immature brain is highly susceptible to the consequences of very preterm birth with a high rate of long-term neurodisabilities in survivors and high use of specific outpatient services to limit the functional effects of the disabilities. To assess the economic burden for the social and health care system, it is necessary to inventory the community supports and need for special education or rehabilitation used by preterm children. Such studies are few and were done only in extremely low-birthweight or extremely preterm newborns in the United States. OBJECTIVE: To study the rate

Effect of Vestibular Stimulation Exercises on Level of Consciousness in Patients With Intracerebral Hemorrhage.

Introduction Intracerebral hemorrhage (ICH) is frequently associated with impaired consciousness due to disruption of cortical, subcortical, and brainstem arousal networks. Reduced consciousness limits participation in rehabilitation and highlights the need for safe early interventions to facilitate responsiveness. Conventional multisensory stimulation provides visual, auditory, tactile, and olfactory input to support arousal. Graded vestibular stimulation may offer additional benefit through head movements, positional changes, and visual-vestibular activities; however, its role in patients wi

Allogeneic Hematopoietic Cell Transplantation for Morquio A Syndrome: An International Retrospective Study.

BACKGROUND: Mucopolysaccharidosis IV A (MPS IVA) or Morquio A syndrome is a lysosomal storage disorder that results from a deficiency of N-acetylgalactosamine-6-sulfate sulfatase (GALNS), causing deposition of glycosaminoglycans, keratan sulfate, and chondroitin-6-sulfate in multiple organs. The main clinical manifestations include severe short stature, severe cervical spine stenosis, and progressive skeletal dysplasia, which leads to severe mobility limitation resulting in patients becoming wheelchair-bound in their second decade of life, and overall shortened life span. The current standard

Association of upper-limb neurological recovery with functional outcomes in high cervical spinal cord injury.

OBJECTIVE: High cervical spinal cord injury (SCI) results in complete loss of upper-limb function, resulting in debilitating tetraplegia and permanent disability. Spontaneous motor recovery occurs to varying degrees in some patients, particularly in the 1st year postinjury. However, the impact of this upper-limb motor recovery on long-term functional outcomes remains unknown. The objective of this study was to characterize the impact of upper-limb motor recovery on the degree of long-term functional outcomes in order to inform priorities for research interventions that restore upper-limb funct

Antioxidants and other pharmacological treatments for Friedreich ataxia.

BACKGROUND: Friedreich ataxia is a rare inherited autosomal recessive neurological disorder, characterised initially by unsteadiness in standing and walking, slowly progressing to wheelchair dependency usually in the late teens or early twenties. It is associated with slurred speech, scoliosis and pes cavus. Heart abnormalities cause premature death in 60% to 80% of people with the disorder. There is no easily defined clinical or biochemical marker and no known treatment. This is the first update of a review published in 2009. OBJECTIVES: To examine the efficacy of antioxidants and other phar

Sacsinopathies: sacsin-related ataxia.

Autosomal recessive spastic ataxia of Charlevoix-Saguenay (ARSACS) was originally found among inhabitants of the Charlevoix-Saguenay region of northeastern Quebec in Canada. This disease is a neurodegenerative disorder characterized by early-onset spastic ataxia, dysarthria, nystagmus, distal muscle wasting, finger and foot deformities, and retinal hypermyelination. The principal neuropathology comprises atrophy of the upper vermis and the loss of Purkinje cells in the cerebellum. The SACS gene was originally reported to consist of a single gigantic exon spanning 12.8 kb with an 11.5-kb open r

Improving diagnostic accuracy of multiple system atrophy: a clinicopathological study.

Clinical diagnosis of multiple system atrophy is challenging and many patients with Lewy body disease (i.e. Parkinson's disease or dementia with Lewy bodies) or progressive supranuclear palsy are misdiagnosed as having multiple system atrophy in life. The clinical records of 203 patients with a clinical diagnosis of multiple system atrophy were reviewed to identify diagnostic pitfalls. We also examined 12 features supporting a diagnosis of multiple system atrophy (red flag features: orofacial dystonia, disproportionate antecollis, camptocormia and/or Pisa syndrome, contractures of hands or fee
