Abacavir and cardiovascular disease: A critical look at the data.

Most HIV-infected subjects will receive a treatment regimen including abacavir or tenofovir. Therefore, clarifying if there is an increased risk of acute myocardial infarction (AMI) among those exposed to abacavir is of the utmost importance. Due to the low frequency of AMI in this young population (2-5 per 1000 patients/year), efforts to clarify this have been quite controversial. While some observational cohorts have found a statistically significant association, others have not. Meta-analysis of randomized clinical trials offering the highest scientific evidence found no association at all,…

Zidovudine, tenofovir or abacavir? Different adverse effect profiles.

Current guidelines on first-line treatment of HIV infection recommend a combination of at least three antiretroviral drugs from two different pharmacological classes: at least two nucleoside or nucleotide reverse transcriptase inhibitors plus either a protease inhibitor or a non-nucleoside reverse transcriptase inhibitor. Among the available nucleoside or nucleotide inhibitors, some guidelines recommend the tenofovir + emtricitabine combination for adults. The abacavir + lamivudine and zidovudine + lamivudine combinations have similar efficacy but are only considered alternative options. What …

It's about time: The association between abacavir and cardiovascular disease.

Data from the REPRIEVE trial have been used to study the association between treatment with abacavir and cardiovascular disease in people with HIV. Past exposure to abacavir before the trial was found to be just as risky as exposure during the trial. This calls into doubt the widely accepted hypothesis that recent exposure to abacavir increases the risk of cardiovascular disease but that risk rapidly attenuates after exposure ends. The evidence for this hypothesis is weak while evidence for an alternative hypothesis has been neglected. An alternative hypothesis is that risk from exposure to ab…

An Animal Model of Abacavir-Induced HLA-Mediated Liver Injury.

Genome-wide association studies indicate that several idiosyncratic adverse drug reactions are highly associated with specific human leukocyte antigen (HLA) alleles. For instance, abacavir, a human immunodeficiency virus reverse transcriptase inhibitor, induces multiorgan toxicity exclusively in patients carrying the HLA-B*57:01 allele. However, the underlying mechanism is unclear due to a lack of appropriate animal models. Previously, we developed HLA-B*57:01 transgenic mice and found that topical application of abacavir to the ears induced proliferation of CD8+ lymphocytes in local lymph nod…

Once vs twice-daily abacavir and lamivudine in African children.

BACKGROUND: Antiretroviral therapy (ART) adherence is critical for successful HIV treatment outcomes. Once-daily dosing could improve adherence. Plasma concentrations of once-daily vs twice-daily abacavir + lamivudine are bioequivalent in children, but no randomized trial has compared virological outcomes. METHODS: Children taking abacavir + lamivudine-containing first-line regimens twice daily for more than 36 weeks in the ARROW trial (NCT02028676, ISRCTN24791884) were randomized to continue twice-daily vs move to once-daily abacavir + lamivudine (open-label). Co-primary outcomes were viral …

Identification of abacavir hypersensitivity reaction in health care claims data.

PURPOSE: Abacavir is associated with an infrequent but potentially serious hypersensitivity reaction (HSR) that can include a wide range of signs and symptoms. Identification of this reaction through medical insurance claims could provide a simple and efficient means of monitoring the incidence of abacavir hypersensitivity in large populations of patients. METHODS: Using data from a safety study of 948 abacavir users with 22 hypersensitivity events identified from claims and validated through medical record review, we used a recursive partitioning analysis to construct an algorithm to differe…

Abacavir: a review of its clinical potential in patients with HIV infection.

UNLABELLED: Abacavir is a carbocyclic 2'-deoxyguanosine nucleoside analogue. It is metabolised intracellularly to a 2'-deoxyguanosine nucleoside analogue which competitively inhibits HIV reverse transcriptase and terminates proviral DNA chain extension. In double-blind trials in antiretroviral therapy-experienced or -naive patients, reductions in HIV RNA levels were greater and more prolonged in patients receiving abacavir in combination with other antiretroviral drugs than in those receiving placebo in combination with the same agents. Furthermore, abacavir in combination with lamivudine and …

An up-to-date evaluation of dolutegravir/abacavir/lamivudine for the treatment of HIV.

INTRODUCTION: There are more than 30 agents available for the treatment of HIV with guidelines shifting toward integrase strand transfer inhibitors (INSTIs) as part of first line therapy. The fixed dose combination of dolutegravir (DTG), abacavir (ABC), and lamivudine (3TC) is a convenient, well tolerated, and highly effective option for treating HIV infection and remains a first line therapy across several prominent guidelines. AREAS COVERED: In this drug evaluation, the authors provide a comprehensive overview of DTG/ABC/3TC for the treatment of HIV including the pharmacokinetics, pharmacod…

Dolutegravir plus abacavir-lamivudine for the treatment of HIV-1 infection.

BACKGROUND: Dolutegravir (S/GSK1349572), a once-daily, unboosted integrase inhibitor, was recently approved in the United States for the treatment of human immunodeficiency virus type 1 (HIV-1) infection in combination with other antiretroviral agents. Dolutegravir, in combination with abacavir-lamivudine, may provide a simplified regimen. METHODS: We conducted a randomized, double-blind, phase 3 study involving adult participants who had not received previous therapy for HIV-1 infection and who had an HIV-1 RNA level of 1000 copies per milliliter or more. Participants were randomly assigned …

Abacavir-reactive memory T cells are present in drug naïve individuals.

BACKGROUND: Fifty-five percent of individuals with HLA-B*57:01 exposed to the antiretroviral drug abacavir develop a hypersensitivity reaction (HSR) that has been attributed to naïve T-cell responses to neo-antigen generated by the drug. Immunologically confirmed abacavir HSR can manifest clinically in less than 48 hours following first exposure suggesting that, at least in some cases, abacavir HSR is due to re-stimulation of a pre-existing memory T-cell population rather than priming of a high frequency naïve T-cell population. METHODS: To determine whether a pre-existing abacavir reactive m…

Forced degradation study of abacavir using the electrochemistry-mass spectrometry setup.

This article investigates the electrochemical oxidation of abacavir using a simple online setup, coupling an electrochemical flow cell with a mass spectrometer, as an alternative approach to forced oxidative degradation studies. Electrochemical oxidation of abacavir was performed on a glassy carbon electrode in only 3 min, using four different solvents: three containing 0.5 mol/L ammonium acetate, pH 7.0, with 0, 50, and 90% (v/v) methanol, and one containing 0.1 mol/L NaClO4 in pure methanol. A total of seven degradation products were observed, with detector signals in different ratios depend…

CYP1A2 and ADH1C contribute to abacavir aldehyde intermediate formation in the human liver.

Abacavir, an anti-human immunodeficiency virus agent, is metabolized to the pharmacologically inactive abacavir carboxylate via an aldehyde intermediate, abacavir aldehyde (abacavir-CHO). Because aldehyde formation is generally rate-limiting, the enzymes responsible for abacavir-CHO formation may significantly influence drug efficacy and toxicity. In this study, we aimed to identify the enzymes involved in abacavir-CHO formation in the human liver. Using 5 recombinant human NAD+-dependent alcohol dehydrogenase (ADH) isoforms, which are well known to catalyze aldehyde formation from alcohols, A…

Abacavir Exposure in Children Cotreated for Tuberculosis with Rifampin and Superboosted Lopinavir-Ritonavir.

In children requiring lopinavir coformulated with ritonavir in a 4:1 ratio (lopinavir-ritonavir-4:1) and rifampin, adding ritonavir to achieve a 4:4 ratio with lopinavir (LPV/r-4:4) overcomes the drug-drug interaction. Possible drug-drug interactions within this regimen may affect abacavir concentrations, but this has never been studied. Children weighing <15 kg needing rifampin and LPV/r-4:4 were enrolled in a pharmacokinetic study and underwent intensive pharmacokinetic sampling on 3 visits: (i) during the intensive and (ii) continuation phases of antituberculosis treatment with LPV/r-4:4 an…

Use of Abacavir and Risk of Cardiovascular Disease Among HIV-Infected Individuals.

BACKGROUND: Evidence is conflicting about the association of abacavir use and cardiovascular disease (CVD) among HIV-infected individuals. Previous studies may have been biased by the preferential initiation or continuation of abacavir in patients with renal dysfunction. METHODS: We conducted a cohort study in Kaiser Permanente California during 1998-2011, following HIV-infected adults initiating antiretroviral therapy until the earliest of CVD (ie, coronary heart disease or ischemic stroke), health plan disenrollment, death, or end of study. We used inverse-probability weighting to fit margi…
