Nicorandil-induced Isolated Abducens Nerve Palsy.

Nicorandil is commonly used to treat angina pectoris. However, the adverse neurological effects of nicorandil remain unclear. Herein, we report the case of a patient who presented with isolated abducens nerve palsy associated with the administration of nicorandil. A 74-year-old man presented to our hospital with abducens nerve palsy and headaches. The patient had started nicorandil treatment one week prior. No abnormalities were observed in the laboratory tests or neuroradiological findings. The patient recovered immediately after the discontinuation of nicorandil treatment. In cases of crania…

Isolated Abducens Nerve Palsy Following Pembrolizumab.

Pembrolizumab is a checkpoint inhibitor targeting the programmed cell death 1 receptor of lymphocytes and is used in the treatment of solid tumours including melanoma. The authors report a 64-year-old man treated with pembrolizumab for stage IV cutaneous melanoma (primary cutaneous melanoma of the right lower back) with liver metastases. The patient developed a horizontal binocular diplopia due to an isolated unilateral cranial nerve VI palsy. Following 1 week of high dose oral steroid therapy and cessation of the drug, the patient's nerve palsy and associated diplopic symptoms improved dramat

Horner's syndrome with abducens nerve palsy.

A 68-year-old male patient presented with a week of sudden diplopia. He had been diagnosed with nasopharyngeal cancer 8 months prior and had … invasion of nasopharyngeal cancer. Lesions on the cavernous sinus need to be considered in cases of abducens nerve palsy and ipsilateral Horner's syndrome.

Benign recurrent abducens (sixth) nerve palsy.

Benign recurrent abducens nerve palsy is rare. Twenty-three cases in children have been reported in the literature and many of these cases followed immunization or were associated with viral illness. Most of the reported patients share the following features: spontaneous recovery within 6 months, ipsilateral recurrence, and painless palsy. The authors describe a Turkish child with recurrent abducens nerve palsy with no obvious etiology.

Posttraumatic abducens to oculomotor nerve misdirection.

INTRODUCTION: Paradoxical patterns of extraocular muscle, eyelid, or pupillary movements can occur following injury between divisions of the oculomotor nerve, trigeminal and abducens nerves, and trigeminal and oculomotor nerves. We report three cases of unusual ocular motility and eyelid movements that are a result of aberrant connections between the abducens and oculomotor nerves. METHODS: Three patients with unusual eye movement abnormalities after trauma were studied. A complete ophthalmic examination plus neuroradiologic evaluation were performed. RESULTS: Each patient manifested an aber…

Benign recurrent unilateral abducens nerve palsy.

The recurrence of an isolated unilateral left abducens nerve palsy on five occasions over five years is described in an adult. No evidence of any systemic or intracranial disorder has been identified. The aetiology remains uncertain and the clinical course is benign. The diagnosis of 'benign' abducens nerve palsy is one of exclusion and is made retrospectively after an adequate period of serial evaluations.

Abducens-oculomotor synkinesis associated with internuclear ophthalmoplegia and acquired abducens nerve palsy.

We report a case of acquired abducens-oculomotor synkinesis, internuclear ophthalmoplegia, and abducens nerve palsy following brainstem hemorrhage. Weakening surgery on the medial rectus muscle did not eliminate the synergistic movement. The mechanism responsible for the abducens-oculomotor synkinesis is discussed.

Bilateral Abducens Paralysis Secondary to Compression of Abducens Nerve Roots by Vertebrobasilar Dolichoectasia.

Vertebrobasilar dolichoectasia is characterized by dilatation, tortuosity, and marked elongation of the vertebral and basilar arteries. Doli … authors present a case in which vertebrobasilar dolichoectasia led to the development of bilateral abducens nerve paralysis in a 9-year-old girl.

Supernumerary Abducens Nerves: A Comprehensive Review.

BACKGROUND: Branching and/or replication of the abducens nerve is not an uncommon occurrence. Although numerous variations have been documented, the rarest forms are duplicated or triplicated nerves, where multiple nerve roots originate from the brainstem, travel intracranially, and attach to the lateral rectus as separate entities. METHODS: We conducted a systematic literature search on the topic of supernumerary abducens nerve, using PubMed and Google Scholar. RESULTS: After screening, 16 studies were included: 11 cadaveric studies and 6 case reports. CONCLUSIONS: In this paper, we review…

The abducens nerve: microanatomic and endoscopic study.

OBJECTIVE: Only a few anatomic studies concerning the intra- or extracranial course of the abducens nerve (Cranial Nerve VI) have been reported. This is likely because the nerve passes through anatomically intricate areas, making its neurovascular relationships complex. Here we provide an anatomically and surgically oriented classification of the abducens nerve, analyze the microanatomy of the nerve and the surrounding connective and/or neurovascular structures, and provide measurements and anatomic topography. PATIENTS AND METHODS: A microsurgical anatomic dissection of 55 cadaveric human he…

Bilateral congenital absence of the abducens nerve.

A patient with lifelong, bilateral horizontal gaze palsies is presented. The anatomical findings of bilateral absence of the abducens nerve as it affected the brainstem, the course of the cranial nerves to the extraocular muscles, and muscle innervation are described. The possible relationship of these findings to Duane's syndrome and Möbius syndrome is discussed.

Invasive fungal sinusitis with abducens nerve palsy.

BACKGROUND: Though rare, sphenoid sinusitis can cause abducens nerve palsy because of the anatomical proximity of the sphenoid sinus and the abducens nerve. CASE PRESENTATION: A male patient in his late seventies presented with double vision and left abducens nerve palsy. Imaging revealed sinus opacifications later identified as due to Scedosporium apiospermum, a rare fungal pathogen. Despite being immunocompetent, the patient developed invasive fungal sinusitis involving the sphenoid sinus. Treatment included systemic antifungal therapy with voriconazole and sinus surgery. Follow-up imaging …
