The onset of the pain, tenderness and swelling in the scrotal content is a common complaint in the urological outpatient. A child with those symptoms should be looked on as an emergency sign requiring emergency surgical exploration as a testicular torsion. Testicular torsion represents a surgical emergency because the likelihood of testicular salvage diminishes with the duration of torsion. Therefore, we must act quickly to identify this condition in any patient who presents with an acute scrotum. The history, physical examination and patient age can provide important clues as to the exact cau

Of the many causes of acute chest pain, it is important to delineate the life threatening causes, such as myocardial infarction, dissecting aneurysm and pulmonary embolism. It is important to consider the cause as 'myocardial infarction until proved otherwise'. Common causes in general practice are musculoskeletal disorders and functional problems. A diagnostic strategy is outlined.

A 52 year old patient was admitted for retrosternal pain not responding to nitroglycerin. Two years before he had suffered myocardial infarction. He had known cholecystolithiasis. Reinfarction was excluded, but the patient developed right upper quadrant abdominal pain with rebound tenderness, fever and leukocytosis. Abdominal sonography supported the diagnosis of acute cholecystitis. Acute illness resolved rapidly without complications under treatment with antibiotics. The patient underwent cholecystectomy during the free interval four weeks after discharge from the hospital. Intraoperative di

The case of a patient presenting to the Emergency Department with acute head pain is presented in this article. The case is discussed from the standpoint of the meaning of the historical features in the case, and the physical examination features. The differential diagnosis of acute head pain in the Emergency Department is discussed. Potential diagnostic strategies and initial therapeutic regimens are covered.

Acute abdominal pain is a daily problem in a busy pediatric practice and in hospital emergency rooms. An initial, carefully performed history and physical examination should differentiate the child who has a surgical disease from one who requires medical therapy or only reassurance. Whenever there is doubt about the diagnosis, the child must be admitted to the hospital for observation and appropriate laboratory examinations. This will allow for the early diagnosis of appendicitis and other diseases requiring an operation and will prevent needless operations on children with medical illnesses.

BACKGROUND: Analgesia by specialists with formal training in pain management could be more effective, to find out, the results of a team of an acute pain service will be determined.
METHODS: Retrospective study (n = 108) of post-operative (POP) analgesia; two evaluations were taken: before starting analgesics in the immediate POP period and the second at 24 h. A multivariate analysis was performed to establish independent risk factors associated with the effectiveness of the treatment.
RESULTS: The effectiveness was 81.48% at 24 h. The risk factors associated with poor management effectivene…

The thesis comprises an overview and four papers, all published or submitted for publication in international peer-reviewed scientific journals. Chronic pain after surgery is a common and debilitating complication after many types of surgery. The cause and pathology behind is still mainly uncovered, though several risk factors have been proposed. One of the strongest risk factors for persistent postsurgical pain is the intensity of the acute pain response though the mechanisms involved remain unsettled. The acute pain response consists of several different types of pain (i.e. somatic pain, v…

Acute pain is a common and nearly universal experience that usually has a sudden onset and is limited in duration. It is a normal physiologic response to a noxious stimulus that can become pathologic if untreated or not treated effectively. Acute pain has a limited duration (<1 month) and often is caused by injury, trauma, or medical treatments such as surgery. Primary care practitioners (PCPs) who encounter patients with acute pain can help preserve function and quality of life and prevent progression to chronic pain by implementing appropriate management strategies. PCPs in rural settings ma…

BACKGROUND: Acute postoperative pain is common. Nearly 20 per cent of patients experience severe pain in the first 24 h after surgery, a figure that has remained largely unchanged in the past 30 years. This review aims to present key considerations for postoperative pain management.
METHODS: A narrative review of postoperative pain strategies was undertaken. Searches of the Cochrane Library, PubMed and Google Scholar databases were performed using the terms postoperative care, psychological factor, pain management, acute pain service, analgesia, acute pain and pain assessment.
RESULTS: Infor…

BACKGROUND: Back pain has many causes. In Germany, about 70% of adults have at least one episode of back pain per year.
METHODS: This review is based on a selective literature search and on the German National Disease Management Guideline for Low Back Pain.
RESULTS: The physician taking the history from a patient with back pain should ask about the nature, onset, course, localization, and radiation of the pain and its dependence on physical activity and/or emotional stress. In the differential diagnosis, neurologic deficits and any "red flags" suggesting dangerous conditions such as spinal f
