Alcoholism and depression.

Alcoholism and depression are entangled in many ways and appear in many combinations. In spite of this fact, to this problem is rarely given sufficient attention which results in poor diagnostic and inadequate therapeutic approach with all the consequences this engenders. The frequency of depression in alcoholics is investigated here with the object of finding out to what extent it can be successfully diagnosed and medically treated. The research was carried out in the Psychiatric Clinic of the Clinical Hospital "Split" and the sample of examinees included the patients treated in the stationar…

Neuropathology of alcoholism.

There are wide ranging effects of alcohol on the nervous system. Some interfere with physiological and neurochemical functions but ultimately structural damage occurs. During life one of the most impressive changes is brain shrinkage which can be visualized using neuroradiological imaging techniques. This article reviews the pathological explanations for brain shrinkage and addresses the question of the pathogenesis of the reversible component of this damage in relation to prolonged abstinence from alcohol. This shrinkage seems to relate to a loss of white matter. However, the cortex is also a

Alcoholism and smoking.

A case-control study compared the natural history of cigarette smoking in alcoholic and nonalcoholic populations to determine why alcoholism and smoking are closely associated. Eighty-three percent of alcoholics were smokers compared to 34% of the nonalcoholic subjects. Compared to other children, children who later became alcoholic were more likely to become regular users of tobacco once they had tried it. Only 7% of alcoholic smokers were successful in their attempts to quit smoking compared to 49% of the nonalcoholic smokers. More research comparing the smoking cessation efforts of alcoholi…

Anxiety and alcoholism.

Patients with alcohol dependence commonly experience symptoms of anxiety, depression, and insomnia. It is essential that clinicians recognize and treat anxiety disorders in alcoholic patients. Panic attacks with and without agoraphobia are especially prevalent among alcoholics and their families. Treatments of choice for panic disorder are the monoamine oxidase inhibitors, as well as tricyclic antidepressants and the benzodiazepine alprazolam. Benzodiazepines seem to be effective in controlling two pathophysiologic characteristics of alcohol withdrawal--noradrenergic and hypothalamic-pituitary

[Tiapride and alcoholism].

The physician-patient relationship plays a major part in the management. The consultations should be extended and fruitful for the patient, … ication in the treatment. Tiapride therefore proves a highly effective therapeutic agent in chronic alcoholism, providing rapid action, good results and excellent tolerance with small dosages.

The alcoholism generator.

Alcohol exposure largely affects 3 populations: fetuses, adolescents, and adults. These 3 developmental stages are inextricably intertwined such that elevated alcohol exposure at any time increases the probability of exposure at the others. This circular interdependency is called the alcoholism generator. Furthermore, exposure to large amounts of alcohol at these 3 times can cause cognitive dysfunction, largely through mechanisms of alcohol-induced perturbations in neurogenesis and synaptogenesis. Breaking this cycle is key to reducing problem alcohol drinking and the associated sequelae.

[Treatment of alcoholism].

In view of the many non-specific symptoms a diagnosis of alcoholism can only be made by means of a detailed history and a medical checkup. Simple, standardized questions may often be sufficient. After a diagnosis a consultation with a family physician and drugs, such as disulfiram, acamprosate and naltrexone have been proved to be a highly efficient therapy. Therefore, it is worthwhile to treat alcoholics in the general practice setting.

[Alcoholism: relapse prevention].

Prevention of alcoholism relapse implies an early detection of the problem (abuse or dependence). This can be achieved through clinical interview, biological evaluation, psychological assessment (i.e. "The Drinking Habits"--I. Pelc) or according to the "Alcoholism Decision Tree" (The Plinius Maior Society). In order to prevent alcoholism relapse, strategies to control the clinical symptoms related to the "Post-Detoxification Syndrome" have recently been highlighted. Further are discussed the benefit of various psychotherapeutic modalities as well as psychopharmacological interventions. Accordi…

Pharmacotherapy in alcoholism.

This review categorizes five main uses of pharmacologic agents in the treatment of alcoholism: reversing the active pharmacologic effects of alcohol; controlling withdrawal symptoms; blocking the desire for alcohol use; treating psychiatric symptoms induced by alcohol and other drugs; and treating independent, but concurrent, psychopathologic conditions. No medication, including stimulants such as caffeine, has been found to actually reverse the action of alcohol. Because of their cross-tolerance and dependence with alcohol, benzodiazepines--especially intermediate acting preparations such as …

Anxiety and alcoholism.

The prevalence and the co-occurrence of anxiety disorders and alcoholism in individuals, families, and the general population have been documented in both epidemiologic and clinical investigations. In patients who fulfill the criteria for one of those conditions, clinicians should consider the possibility of the presence of the other disorder--currently, at a previous time, or at some point in the future. A lifetime history of alcohol abuse and anxiety disorders obtained from both the patient and the family members may assist in determining the appropriate treatment.
