Postural asymmetries in transfemoral amputees.

BACKGROUND: Postural asymmetries are thought to lead to impairment of body structure and function such as muscle imbalance, gait asymmetry and possible chronic conditions, which result in limitation of mobility and restriction of daily activity for transfemoral amputees (TFAs). Despite the potential clinical impact, postural asymmetries have not been confirmed or quantified in TFAs. OBJECTIVES: To identify the presence of postural asymmetries in TFAs utilizing clinical evaluation measures. STUDY DESIGN: An observational cross-sectional study in which participants were evaluated at a single t…

Stump overgrowth in juvenile amputees.

Stump overgrowth requiring surgical revisions occurred in 58 of 1,015 (5.7%) skeletally immature patients with congenital and acquired amput … of repeated revisions. Study of the histological changes in eight juvenile congenital and acquired amputees revealed two patterns: reactive connective tissue stratification and a bursa.

Advances in amputee care.

UNLABELLED: This self-directed learning module highlights the recent innovations in amputee care. It is part of the study guide on limb deficiency in the Self-Directed Physiatric Education Program for practitioners and trainees in physical medicine and rehabilitation. This focused review article describes the key elements of a successful comprehensive amputee program, the basic surgical considerations for upper- and lower-extremity amputation, and some of the more recent advances in prosthetic components. Further, an update is given on issues such as hand transplantation and the integration of

Differences in whole-body angular momentum between below-knee amputees and non-amputees across walking speeds.

Unilateral, below-knee amputees have an increased risk of falling compared to non-amputees. The regulation of whole-body angular momentum is important for preventing falls, but little is known about how amputees regulate angular momentum during walking. This study analyzed three-dimensional, whole-body angular momentum at four walking speeds in 12 amputees and 10 non-amputees. The range of angular momentum in all planes significantly decreased with increasing walking speed for both groups. However, the range of frontal-plane angular momentum was greater in amputees compared to non-amputees at …

Vascular Amputees: A Study in Disappointment.

Despite optimistic reports about the results of amputation for advanced vascular disease, the patient's assessment of advantages and disadvantages is seldom acknowledged. A detailed social study of 67 amputees has revealed considerable disparity between the patient's views and those of the medical staff. About a third of the patients are forced to retire from active work by the amputation; about three-quarters report a serious decline in their social activities; only about half are really independent with prostheses in the long term; a quarter report severe and intractable symptoms related to …

Amputees Unanimous: A 12-step program.

BACKGROUND: Alongside physical impairment, psychosocial health issues may arise after amputation, impacting quality of life. Traditional psychosocial care models provide services in a linear fashion, with limited supports upon discharge. A novel program entitled Amputees Unanimous is a 12-step program provided for amputees by amputees. OBJECTIVE: To shape the final content of Amputees Unanimous by identifying personal beliefs and opinions of healthcare professionals and amputees. STUDY DESIGN: Qualitative design utilizing a phenomenological approach. METHOD: Five focus groups were held: fou…

Prosthetic rehabilitation of elderly bilateral amputees.

A retrospective evaluation of vascular amputees, all over 55 years of age, found that 18 underwent bilateral lower limb amputation. The ages of patients ranged from 55 to 83 years, mean age at the time of second amputation was 65.9 years, 13 men and 5 women. Out of the 18 patients, 12 underwent bilateral below knee amputation (BK-BK), 3 below knee and above knee (BK-AK), and 3 bilateral above knee (AK-AK). Six patients (50%) of the 12 with BK-BK were fitted with prostheses. They therefore became users of two prostheses and have achieved a high functional level of rehabilitation despite a high …

Body sway in below-knee amputees.

The purpose of the present study was to test the hypothesis that below-knee amputees have less standing stability than normal persons. Twenty below-knee amputees were tested with the quantified Romberg test. All amputees below 59 years and all women above 59 years had a decreased sway compared with matched control groups of normal persons. Amputated men above 59 years did not show any difference in sway compared with the matched control group. Presuming that small sway excursions can be interpreted as a stable standing position, the study shows that a well fitted PTB-amputee stands at least as…

Stump fractures in lower extremity amputees.

A series of 23 above-knee and below-knee amputees who sustained an ipsilateral lower extremity fracture was qualitatively evaluated with respect to the nature of the injuries sustained, associated problems, and results of fracture treatment. The overall incidence of these fractures was 3% in a population of lower-extremity amputees. The stump fractures encountered were distal femur fractures and fractures about the hip. More than one fourth of these fractures had an initially missed or 'delayed' diagnosis. Most femur fractures were successfully treated by nonoperative means, and most of the hi…

Rehabilitation of elderly lower-extremity amputees.

Eighty elderly amputees who were seen at the Rehabilitation Centre of the Royal South Sydney Hospital during a five-year period between 1974 and 1978, have been reviewed. These included 44 unilateral below-knee amputees, 21 unilateral above-knee amputees, and 15 bilateral amputees. Seventy-nine per cent of these patients were able to use their prostheses successfully, and 84% returned to their own homes after discharge from hospital. Some details of the rehabilitation programme, and types of prosthesis prescribed are discussed.

Functional capabilities of lower extremity amputees.

One hundred thirty-four lower extremity amputees were evaluated from six months to 12 years postamputation by means of retrospective questionnaires. Patient population was similar to that of the "Amputee Census" in terms of sex, amputation level and cause of amputation. Information was gathered on activities generally considered essential for daily living, vocation and recreation, living arrangements and adjustments therein, as well as feedback on the patients' beliefs concerning what rehabilitation personnel should be doing to improve amputees' lifestyle. The relationship of functional outcom…

[Muscles of the below-knee amputees].

OBJECTIVE: The objective of this article is to review anatomical, histological and physiological muscle changes following below-knee amputation. MATERIALS AND METHODS: We searched the PubMed and Reedoc databases for studies evaluating modifications of the below-knee stump and changes over time in its anatomy, volume and histology. We also looked at postamputation modifications in gait and balance. RESULTS: Below-knee amputees show muscular atrophy on both the amputated side and nonamputated side, with fewer and smaller muscle fibres (particularly slow-twitch fibres). This amyotrophy varies i…

Exercise performance of lower-extremity amputees.

A comparison of the literature quantifying the energy expended during ambulation of healthy individuals and those with amputation of the lower extremity is difficult as study parameters and methods are inconsistent. However, the energy cost of ambulation is greater for amputees than for nonamputees. Ascending level of amputation appears to be associated with increasing metabolic demand. There appears to be a difference in energy cost of ambulation following different surgical procedures. The literature regarding energy cost of ambulating with different lower-extremity prostheses is equivocal, …

Functional capabilities of lower limb amputees.

Five hundred lower limb amputees were evaluated by personal interview at the Artificial Limb Centre, Pune, India. Information thus obtained on activities of daily living and functional capabilities with their prostheses was analysed. The analysis showed that 55% of the amputees were totally independent, 40% had to use a crutch or cane and only 5% were solely dependent on a wheelchair for ambulation. Further, it was also confirmed that as age increased, functional independence decreased, and that below-knee amputees were more independent than above-knee and bilateral amputees. When compared wit…
