Pharmacologic interrogation of patients with esophagogastric junction outflow obstruction using amyl nitrite.

BACKGROUND: The Chicago Classification of esophageal motility includes a group of patients who show evidence of esophagogastric junction outflow obstruction (EGJOO) as demonstrated by elevated integrated relaxation pressure (IRP) and preserved peristalsis. Our aim is to classify EGJOO patients based on response to amyl nitrite (AN) during high-resolution manometry. METHODS: Patients were considered to have true EGJOO if elevated IRP during supine swallow persisted in the upright position and was associated with high intrabolus pressure. The EGJ response to AN was compared between patients wit…

Effects of amyl nitrite on circulation, respiration and blood homoeostasis in cyanide poisoning.

The effects of intravenously (i.v.) administered or inhaled amyl nitrite (AN) were followed under chloralose anaesthesia in intact and cyanide-poisoned, spontaneously breathing beagles. The i.v. doses of AN were 0.03 and 0.15 mmol/kg and the i.v. dose of KCN was 0.06 mmol/kg. AN was inhaled in a closed system at 0.15 mmol/kg without previous poisoning and, in addition, at 0.074 mmol/kg (two ampoules at 0.3 ml AN) during artificial ventilation after poisoning with 0.045 mmol KCN/kg i.v.. Mean arterial pressure decreased by 15 and 40 mmHg, respectively, after i.v. injection of AN, associated wit…

The effect of amyl nitrite on the mitral valve echocardiogram in presumably healthy young adults.

We analyzed the use of amyl nitrite as a provocative factor in the diagnosis of mitral valve prolapse in a population of healthy young adults. Sixty-five men and 11 women underwent continuous M-mode echocardiographic and phonocardiographic monitoring before, during and after the administration of inhaled amyl nitrite. All of the 76 subjects had normal baseline echocardiograms, and all had a satisfactory hemodynamic response to amyl nitrite. Mitral valve prolapse, defined by echocardiography and phonocardiography, was not provoked in any of the subjects. Therefore, we concluded that, although t…
